A new technical approach for retrograde administration of cardioplegic solutions.

Myocardial protection via the coronary sinus is now currently used by several groups. Although it has generally provided satisfactory results, some of its problems are still not completely resolved. We present a new technique of cardioplegia delivery through the coronary sinus with a Pezzer catheter inserted into it and secured in place by a purse string suture. We believe that this method is safer and more reliable than others.

Hemodynamic changes from spinal cord stimulation for vascular pain.

The hemodynamic changes induced by spinal cord stimulation (SCS) have been studied in a group of 20 patients with peripheral vascular pain. The surgical technique consisted of the introduction of 1 or 2 electrodes in the subarachnoid space up to the level of the painful area, for mono or bipolar SCS. Several techniques have been used for evaluation of hemodynamic changes induced by SCS. Peripheral blood flow speed was measured by means of ultrasound Doppler, showing a raised maximum speed during stimulation, and a tendency of the pulse wave to return to normal. Thermography showed a marked increase of temperature in the painful area. Preoperatively, plethysmography showed an absence of the typical flow waveform, whilst postoperative recordings showed a small wave of progressively increasing amplitude. Scintigraphy with 201TI showed an increase in muscular blood flow in previously hypovascularized areas. The mechanism of action of SCS on peripheral blood flow and vascular pain is discussed.

Effect of perinatal antisteroid treatment on territorial marking behavior in the mongolian gerbil.

Sex differences have been observed in the perinatal sexual differentiation of the neural substrate which regulates territorial marking behavior in the Mongolian gerbil. The present study examines the relative contribution of prenatal and postnatal steroid environment to sexual differentiation in the male and androgenized female using territorial marking behavior as an endpoint. Selective suppression of steroid effects in the pre- and postnatal period was accomplished with the antiandrogen flutamide or the antiestrogen MER-25. Treatment was given prenatally (for 5 days before expected parturition), on the day of birth, and postnatally (to day 10) or prenatally and on the day of birth only. Animals without postnatal antisteroid treatment were intact or were gonadectomized on day 2 and given testosterone propionate (TP) treatment on day 7. (It has previously been shown that day 7 is beyond the period of maximum steroid responsiveness in the male but not in the androgenized female.) MER-25, flutamide, or day-2 ovariectomy had no effect on adult marking behavior responsiveness in females given TP on day 7. All groups marked at normal male frequencies. The presence of flutamide prenatally and on the day of birth in day-2 castrates given TP on day 7 yielded adults with marking responsiveness equivalent to day-7 TP-treated females. In contrast, males given day-7 TP without prenatal and birthday flutamide showed significantly lower marking frequencies, suggesting that the presence of androgen prenatally and on the day of birth rendered day-2 castrates less responsive to TP given on day 7.(ABSTRACT TRUNCATED AT 250 WORDS)

A relationship between frequency of display of territorial marking behavior and coat color in male Mongolian gerbils.

Mongolian gerbils exhibit a territorial marking behavior which is androgen dependent and sexually dimorphic, with males marking more frequently than females. In males a subpopulation of high frequency and low frequency markers was observed in gerbils with brown or black coat color. The present study examines the possibility that this marking frequency difference is genetically determined. After eight generations of selective inbreeding of brown gerbils to maximize the incidence of high marking males, no change in the ratio of high to low marking males was observed. When less common black gerbils were mated with brown gerbils in classic F1 X F2 crosses, a ratio of 3 brown to 1 black resulted, suggesting that black coat color was a Mendelian recessive trait. Marking behavior measured in the offspring of F1 X F2 crosses over five generations revealed significantly higher average marking frequencies in black males than in heterozygous or homozygous brown males. This difference was due to a smaller percentage of low markers among black males than among brown males. Females from these crosses showed no marking frequency differences. Plasma testosterone levels and ventral scent gland size were not different among low or high markers in either black or brown males. These results suggest that there is an androgen-independent genetic component to the expression of territorial marking behavior in the gerbil.

Pre- and postoperative manometric studies in diffuse esophageal spasm.

Since January, 1972, the authors have operated upon 4 patients with idiopathic diffuse exophageal spasm. Clinical details and barium studies are included. Extramucosal myotomy extending from the gastric fundus to the aortic arch was done in each case. Pre- and postoperative manometric studies were carried out in all. After operation in each patient the dysphagia and substernal pain disappeared and in 3 patients radiological patterns changed. The myotomy was associated with marked fall of contractile wave pressures in the body of esophagus. The basal pressures of the esophageal body elevated, in 2 cases fell after the myotomy; in 2 with normal preoperative pressure it remained unchanged. At the lower esophageal sphincter the resting and yield pressures remained similar to the preoperative readings but the myotomy produced a disappearance of the relaxation and contraction pressure. The authors conclude that with myotomy they cannot correct the nature of the functional disorder but, by reducing the amplitude of the waves and lowering the resting pressure if elevated, they can relieve the patient's symptoms.

[Opinion on health priorities in the community of Valencia].

In this study we have used a qualitative approach partially based on the Delphi technique to establish the ten priorities for health in the Valencian Community for the next five years. Two rounds of mail surveys were made sending questionnaires to 1834 people including Health Councillors of the Town Halls, nurses and doctors of the three valencian provinces. The overall response rate was 28.3%. The priorities which were perceived as most important by the respondents were the development of primary care, improvements in the management of health services and the development of preventive medicine. The fourth and fifth places of the ranking were: improvements of emergency services and increasing the budget devoted to hospital care. The 6th priority was preventive measures in cardiovascular diseases, the 7th actions against drug abuse, the 8th special attention to childhood and ageing populations and the last two priorities were protection of the environment and infectious diseases prevention. We consider this approach as useful to obtain additional valuable information complementary to the information obtained from traditional statistics or health surveys specially in the areas of management, diseases prevention and actions towards special groups of population.

[Wernicke-Korsakoff syndrome: malignant tumour as triggering factor].

Gastrectomy, alcoholism and malignant tumour are three predisponing risk factors for the development of Wernicke-Korsakoff syndrome. We described the clinical case of a patient with history of alcoholism that developed Wernicke-Korsakoff syndrome 30 years after undergoing gastrectomy. This patient had, in the last year, a diagnostic for prostatic adenocarcinoma and changes in dietary habits. We presented the clinical and neuropathological features of the Wernicke-Korsakoff syndrome. As well as some aspects in the treatment and prognosis.

[Design of a program to prevent adolescent suicide].

In the last decades a notorious increase in suicidal behaviors has been observed in youth. It has become an international health problem which justifies all prevention initiatives that are put forward in some western countries. The authors, given their experience in assessing and treating suicidal youth, design a prevention program addressed to suicidal risk groups, and whose aim is to curb down suicidal rates among adolescents in their Sanitary Area. This program applied in Child and Adolescent Mental Health Center is structured in five levels: Assessing, initial treatment, therapeutical intervention in the medium and long run, other interventions in the community, and evolution.
