Cluster of cases of haemolytic uraemic syndrome due to unpasteurised cheese.

A cluster of four patients (1 girl, 3 boys) from a French village (2,000 inhabitants) had acute haemolytic uraemic syndrome (HUS) between March 1992 and May 1993. All had prodromes with fever and diarrhoea, then acute renal failure, anaemia, schistocytosis and thrombocytopenia. Peritoneal dialysis was carried out in three children (duration 3-12 days). The verotoxin VT2 gene was identified by polymerase chain reaction in the stools of two children. Some days prior to the diarrhoea, all children had eaten a cheese made with unpasteurised mixed cows' and goats' milk from the same farm. A case control study showed that the occurrence of HUS was linked to the consumption of this milk product (P = 0.006). The VT 2 gene was isolated from the cheese and from the stools of goats and cows from the farm, but not from the stools of farm employees.

[Mucocutaneous lymph node syndrome: case report].

The muco-cutaneous lymphonode syndrome affecting babies and infants has recently been described in Japan. The children present with sub-maxilliary glands, fever and a characteristic desquamating rash. It is important to make the diagnosis because of the rare but dangerous cardiac complications. The aetiology is unknown but the condition may be related to infantile polyarteritis nodosa.

[Pre-, per-, and postoperative resuscitation in thoracic surgery in newborn infants].

The authors report their five year-experience in reanimation during thoracic surgery in the new-born. The report concerns 66 cases (43 atresias of the oesophagus and 23 diaphragmatic hernias). Prognosonis depends on: 1 -- A permanent temam of suitably qualified doctors ready to carry out at any time the required therapy on the infant. 2 -- Free air way (kinesitherapy, continous and prolonged suction of oesophageal atresias and bronchial suction). 3 -- Ventilation (surgery on clean lungs in atresias, diaphragmatic prosthesis and suturing of skin in hernias). Respect of these rules in diaphragmatic hernias saved 12 lives out of 23; in oesophageal atresias, 16 survived out of 16 cases classified as good cases, and the total survival rate was 73%.

[Cardiac malpositions. Apropos of 5 observations in the neonatal period].

Five cases of cardiac malpositions, discovered in the neonatal period, with early death are reported. The determination of the position of the cardiac cavities is essential for appropriate studies of angiocardiograms and appreciation of the possibilities of surgical treatment.

[Unilateral hematocolpos, blind vagina and agenesis of the homolateral urinary tract].

Hematocolpos with homolateral renal agenesis is a rare condition. Dysmenorrhea and paroxysmal abdominal pain are common symptoms in adolescent girls. Following a review of the literature, this article considers those procedures and investigations which can be used to help these patients before arriving at the stage of reconstructive surgery.
