Development of immunoglobulin M-antibodies after rubella immunization with RA27/3 attenuated live vaccine in teen-agers: remaining problems.

Four-hundred-fourty-two rubella seronegative teen-agers living in Siena and the surroundings were vaccinated with RA 27/3 live attenuated rubella over a two years period (1985-1987). One-hundred-twenty seven vaccinees' sera were randomly selected for the rubella specific antibodies of the IgM class detection with two Elisa Methods; the sera were collected 10 and 30 days after the challenge; in 58 girls a serum sample was also obtained 1 yr later. None of the sera showed reactivity for RF. The comparison between the two Elisa methods gave satisfactory results, the percentage of discrepancies being low (3.9%). Ten days after the immunization the IgM positive sera were only 7.56% (Elisa Abbott) and 5.88% (Elisa Enzygnost Behringwerke). One month later, 86.61% of the specimens showed reactivity for specific IgM antibodies. None of 58 sera gave positive results after 1 yr. Rubella antibodies of the IgG class could be demonstrated in the "screening" sera of 3 IgM negative subjects, when an Elisa method was applied. Nine subjects (7%) repeatedly resulted IgM negative one month after the challenge although a seroconversion was observed using both HAI and EIA. A serum sample was also obtained in 6/9 IgM "non responders" after a rubella epidemic observed in 1988-1989; no subclinical reinfection due to the wild virus was detected in this group, no changes in HAI antibody title nor evidence of IgM class specific response were observed. Although most of the diagnostic aspects related with rubella and rubella vaccination appear to be resolved, it should be underlined that there are several remaining problems especially associated with IgM detection.

[Effects of cholera toxin on the regenerating rat liver (author's transl)].

Two groups of animals were treated with cholera toxin respectively with 10,000 B.D. and 20,000 B.D. soon after hepatectomy. The parameters used were: the percentage of liver regeneration, mytotic activity and the liver protein content. The resulting differences between the controls and the two groups of animals treated were not statistically significant. The Authors conclude that cholera toxin does not affect the liver regeneration in the rat.

[Evaluation of the specific immune response after anti-rubella vaccination].

A selective rubella immunization program was carried out in the Siena (Italy) area since 1984. 912 schoolgirls aged 10 to 18 were screened for rubella antibodies. Seropositivity rates, increasing with age, appeared quite heterogenous among subjects living in different villages. Seronegative girls were partly voluntarily immunized with RA 27/3 live attenuated rubella vaccine. Serological and clinical controls performed on 238 subjects one month after immunization showed a 99.6% vaccine-induced seroconversion rate and mild post-vaccinal reactions in 26% of vaccinees. Serological control performed, on 172 subjects, at 8-12 days after vaccination revealed that in most cases vaccine induced haemagglutination-inhibiting antibodies are not detectable at the time. The results are discussed in relation to a forthcoming introduction of compulsory rubella vaccination in our Country.

[Vaccination against poliomyelitis].

The authors, after a review of some data about the actual poliomyelitis epidemiology in the world, point out the necessity of periodical checks for poliomyelitis vaccination. To this purpose, preliminary data of a research, undertaken in the province of Siena, into the effectiveness and innocuity of oral poliovirus vaccine, are reported. This evaluation has been made through isolation and identification of vaccinal polioviruses from stool after the first, second and then third dose of vaccine, and through titration of serum neutralizing antibodies. Results confirm the high effectiveness and innocuity of oral poliovirus vaccine, but suggest the opportuneness of some changes in the way of giving the oral vaccine.

[Effects of streptococcal toxin on hepatic regeneration in the rat].

The liver regeneration percentage, mytotic activity and protein content were used to demonstrate the effects of streptococcal toxin on regenerating rat liver. No statistically significant difference was showed between controls and toxin treated animals. Serum level of C, resulted on the contrary, considerably reduced in the animals treated with streptococcal toxin.

[Neonatal meningitis (personal contribution)].

The authors, after a review of the literature about etiology, pathogenesis, epidemiology, pathological anatomy and clinic of newborn meningitis, report a personal experience of 15 cases. The more frequent etiological agents were Gram-positive bacteria (60%) and in particular Listeria monocytogenes type 4 and Streptococcus agalactiae. Gram-negative bacteria were isolated in 40% of cases and E. coli presented the highest ratio. All patients were treated with ampicillin-CAF association intravenous for the first 3-5 days and then i.m. or oral. The survival ratio was 77% in case of Gram-positive meningitis and 83% in case of Gram-negative meningitis. Sequeles were observed only among patients with Gram-negative meningitis; it's important to remark that in these cases the etiological agent was CAF-resistant.

[Nucleic acids in the liver of rat treated with "E. coli" O 127 lipopolysaccharide (author's transl)].

The Authors have studied the variations of nucleic acids, proteins and histological aspect in the liver of rats, treated with 1 mg of E. coli ) 127 lipopolysaccharide and killed at 24th, 48th, 72th, 96th hour after the inoculation. The endotoxin determines an increase of nucleic acids and a reduction of hepatic proteic contents. The histological observation shows throw-back phenomenons of the hepatocyte with hyperplasy and hypertrophy of Kupffer cells at 24th hour, while at following intervals the phenomenous of hepatic rigeneration emphasize.
