[Multiple bone localizations in hair cell leukemia. Efficacy of treatment with alpha 2 interferon].

We report a case of hairy cell leukemia with multiple bone lesions (skull, tibia, spine, femoral head) clinically, radiologically, and histologically documented. Radiotherapy failed to improve tibial pain. Treatment with alpha interferon resulted in bone pain improvement. Diagnoses and therapy strategies of these unusual features are discussed with respect cases to previously published in the literature.

Squamous cell carcinoma of the esophagus. Treatment by combined vincristine-methotrexate plus folinic acid rescue and cisplatin before radiotherapy.

From January to November, 1981, 28 patients with unresectable squamous cell carcinoma of the esophagus were treated with two cycles of chemotherapy combining vincristine (V), methotrexate (M), folinic acid rescue, and cisplatin (P) on days 1 and 21. Split-course radiation therapy was delivered thereafter from day 42 on. Hematologic, renal, and neurologic tolerance was acceptable, but most of the patients experienced nausea and vomiting. Results evolution at day 40 showed a 61% partial response (PR) rate and a 7% complete response (CR) rate. One month after the end of radiation therapy, 43% PR and 32% CR were obtained. Median response duration was 8 months. Median survival was 11.6 months for patients overall, yielding 12.9 months for responders and 5.9 months for nonresponders. Based on the response rate obtained with combined chemotherapy, a randomized trial of VMP initial chemotherapy is currently being undertaken by our cooperative group to study whether such an initial treatment could improve resectability and radiation-mediated local control along with survival rate.

[Bifractionated radiotherapy and oropharyngeal carcinoma].

The relatively poor results of therapy for epidermoid oropharyngeal carcinoma stages T3 and T4 suggested the need for new treatment regimens. A study involving 24 patients included bifractionated radiation on a daily basis, as used by J. C. Horiot and A. Nabib and then by the E.O.R.T.C. group. The more encouraging results reported by A. Nabib were not obtained. The 5-year survival rate will possibly be no better than after conventional external radiation. Post-radiotherapy complications were observed.

Effects of irradiation on the submandibular gland of the rat. An enzyme histochemical and ultrastructural study.

Single-dose cervical irradiation by cobalt 60 in rats induced lasting functional disturbances of the submandibular gland which were excessive when compared with the relative integrity of the gland as seen under the light microscope. Enzyme histochemical and ultrastructural studies revealed severe damage shortly after exposure with appearance of karyolytic bodies and autophagosomes accompanied by increased hydrolase activity. Mitochondrial alterations were concomitant with diminished ductal oxidative enzyme activity. Although most of these alterations resolved rapidly as a result of acinar and ductal cell repair and regeneration originating in the intercalated ducts, secretory abnormalities were still observed two months after exposure as evidenced by the accumulation of granules in acinar cells and the heterogeneity of ductal cell granules. These anomalies, comparable to those observed in sialadenoses, probably result from persistent alterations of intralobular nerve endings.

[The fate of cancellous bone grafts after irradiation (author's transl)].

It is often necessary to reconstruct the bony defects that result from the resection of bone tumours with cortico-cancellous bone grafts. In addition, post-operative radiotherapy is often given to prevent tumour recurrence. We have attempted to show that the rational use of radiotherapy after the insertion of a bone graft does not endanger the rehabilitation of the graft. We do not know of any clinical publication related to this subject. We are thus reporting eight cases of irradiated cortico-cancellous grafts which were given precise doses of irradiation and we have been able to draw the following conclusions: a) when doses of 4 000 Rads or less were given, there was no delay in graft consolidation, b) when doses of more than 6 000 Rads were used, graft rehabilitation was jeopardised. c) it is possible that 6 000 Rads given over a limited area will not endanger the graft but we have only one example. We have reviewed the literature concerning experimental studies on the progress of bone grafts, the effect of irradiation on normal bone and on cortico-cancellous grafts. The experiments have been performed must frequently in vitro and in vivo on small laboratory animals. The conclusions drawn from these studies agree with our clinical impressions: following irradiation the osteogenic activity of a cortico-cancellous graft and of normal bone are the same. They are subjected to the same risks and the same complications.

[Treatment of Ewing's sarcoma by radiotherapy and adjuvant chemotherapy : results at 3 and 5 years (author's transl)].

The local treatment of Ewing's sarcoma by radiotherapy is classically linked to a very poor prognosis : 10 to 15% of 5 years survival, 75 to 95% of distant metastasis in 2 years. The combination of local radiotherapy to a systemic adjuvant chemotherapy have been recently shown to give a real benefit to this prognosis. The therapeutic trial of the EORTC and GETO, presented here allows us to hope a disease free survival at 5 years for 50% of our patients and a complete survival at 5 years for 56% of them.

Acquired von Willebrand's syndrome with IgM inhibitor against von Willebrand's factor.

This report describes a patient without evident underlying disease, in whom an acquired von Willebrand's syndrome was discovered before surgery. Coagulation abnormalities included a borderline bleeding time, a low retention of platelets on glass beads, decreased levels of factor VIII procoagulant activity (VIIIAHF), factor VIII-related antigen (VIIIAg), and ristocetin-induced agglutination cofactor (VIIIVWF). After cryoprecipitate infusion the patient did not have the expected rise and there was no secondary increment in VIIIAHF. The patient was treated with prednisone for three weeks without significant improvement in the laboratory findings. Spontaneous resolution was observed long after this therapy. The haemostatic abnormalities were attributable to the presence of an inhibitor directed against VIIIVWF. The inhibitor was found in the IgM fraction. Its autoimmune nature is probable although we failed to demonstrate any inhibitory effect of Fab obtained from the patient's purified IgM. Despite the lack of inhibitory effect against VIIIAHF and VIIIAg, the low levels of all three activities of the factor VIII complex could be explained by the short half-life of immune complexes between factor VIII and the inhibitor.

A monoclonal macroglobulin with antinuclear activity.

Serum containing a monoclonal IgM protein from a patient with Waldenstroms' macroglobulinaemia gave intense immunofluorescent staining of kidney nuclei. The Fab mu fragments of this immunoglobulin were obtained. The IgM and Fab fragments reacted in vitro with kidney nuclei using unfixed cryostat sections of rat or mouse kidney. After treatment of the patient with chemotherapy, the monoclonal IgM disappeared, and no more antinuclear activity could be detected in the serum. The results strongly suggest that this IgM protein had antinuclear activity.

[Inflammatory granuloma of the face with a malignant course. Atypical Hodgkin's disease (author's transl)].

A case of Hodgkin's disease beginning in the skin of the face with local lymph node involvement is reported. This was a lymphogranulomatous form without characteristic Reed-Sternberg cells in skin biopsy specimens, rendering the diagnosis difficult. There was a combination of granulomatous inflammation, plaques of necrosis and lesions of fibrinoid necrosis in the vessel walls. This form entered the classification of clinical Stage IV, with its associated serious prognosis.
