[Spinal anesthesia with bupivacaine. Comparative study of 2 hyperbaric and isobaric solutions].

This study compared two isobaric and hyperbaric solutions of 0.4% bupivacaine used for carrying out spinal anaesthesia in 264 patients more than 70 years old undergoing comparable surgical procedures. With the hyperbaric solution, the mean time of onset of analgesia was shorter: 8.4 +/- 4 min (11.5 +/- 5.1 min with the isobaric solution), the level of analgesia higher by two dermatomes and the mean duration of analgesia longer: 147.3 +/- 24.8 min (121.1 +/- 24.8 min with the isobaric solution). Blood pressure fell more often with the hyperbaric solution. The motor block, assessed by the inability to move the legs and abdominal wall paralysis, was more complete with the hyperbaric solution. Lastly, there were less failures: 3.4% with hyperbaric solution and 12.1% with isobaric solution.

Comparative study of methods chosen by the Association Française de Normalisation (AFNOR) for evaluating sensitizing potential in the albino guinea-pig.

An evaluation was made of the different protocols recommended by the Association Française de Normalisation (AFNOR) for assessing the sensitizing potential of chemicals in the guinea-pig. The methods studied were those of Magnusson & Kligman (J. invest. Derm. 1969, 52, 268), Maguire (J. Soc. cosmet. Chem. 1973, 24, 151), Maurer et al. (Agents & Actions 1975, 5, 174), Klecak et al. (J. Soc. cosmet. Chem. 1977, 28, 53), Dossou & Sicard (1984, to be published) and a hitherto unpublished method of Guillot, Gonnet, Clément & Brulos. Results obtained by each method were compared in tests on the allergenic reference substances dihydrocoumarin, p-phenylenediamine, formalin, penicillin G and benzocaine (ethoform) and on propylene glycol (negative control). The results showed that the number of animals sensitized often varies with the choice of experimental protocol, that inclusion of tests on a control substance is essential, and that while quantitative differences were found, the methods were qualitatively similar in their capacity to identify allergens in the guinea-pig.

[Hemangioendothelioma of the right supra-clavicular region. Clinicopathologic study and management. Report of a case (author's transl)].

The authors present a case of hemangioendothelioma of the right supra-clavicular region in a 14-year-old boy. Interest of scanner in extension bilan and supraselective arteriography with tumor embolisation are stressed. Complete excision was possible and no sign of recurrence is noted 18 months later. Essential characteristics of a such tumor are reviewed.

[Per-operative blood flow measurement, complement to arteriography. Control and prognostic appreciation method (author's transl)].

Per-operative blood flow measurement is studied as hemodynamic control in complement of arteriography (anatomic control). Early (1 month) and late (2 years) results are so reviewed in 217 popliteal or distal revascularisation. The authors attach more importance to the late value of peripheral arterial resistance with appears in relationship with early prognostic and post-operative failure. The authors recommend a vascodilative test to define maximum flow and real value of peripheral arterial resistance. The study of intra-arterial vasodilator response appreciates better the value of distal run-off receptivity and was found to correlate well with clinical results. The present study demonstrates prognostic significance of per-operative flow data.

[Use of energy reserves during the breeding fast of the emperor penguin, Aptenodvtes forsteri].

During the breeding fasting of the emperor penguin, the lipid and protein stores are steadily used to meet the metabolic needs; they represent respectively 93 and 7% of the energy production in the animal. The role of the glucid stores are quantitively negligible. Loss of tissue water represents 35,3% of body weight loss. Increased weight loss below 20 kg a "critical weight", is associated with a conversion to protein catabolism when lipid supplies are exhausted. These results allow the estimation of the metabolism when the body weight loss is considered in this antartic penguin.
