Severe chronic pancreatitis versus suspected pancreatic disease: dynamic MR cholangiopancreatography after secretin stimulation.

PURPOSE: To assess whether secretin stimulation improves visualization of the pancreatic ducts at magnetic resonance (MR) cholangiopancreatography (MRCP) in patients with severe chronic pancreatitis or suspected pancreatic disease. MATERIALS AND METHODS: Thirty-one patients (group 1) with chronic pancreatitis and 84 patients (group 2) with clinical and/or laboratory findings suggestive of pancreatic disease who did not have ductal alterations at ultrasonography (US) and/or computed tomography (CT) underwent MRCP before and up to 10 minutes after secretin stimulation. Size of the main pancreatic duct (head, body, tail) and duodenal filling before and after secretin stimulation were measured quantitatively. Image quality, number of main pancreatic ductal segments visualized, visualization of side branches, ductal narrowing, endoluminal filling defects, and presence of pancreas divisum were analyzed qualitatively. RESULTS: In both groups, the size of the main pancreatic duct increased significantly 3 minutes after secretin stimulation. Reduced duodenal filling was detected in patients with severe chronic pancreatitis (P < .001). The number of segments of the main pancreatic duct visualized improved from 85 (91%) to 93 (100%) of 93 in group 1 and from 164 (65%) to 245 (97%) of 252 (P < .001) in group 2. Visualization of side branches improved from 22 (71%) to 31 (100%) of 31 in group 1 and from three (4%) to 53 (63%) of 84 (P < .001) in group 2. Pancreas divisum was visualized in one additional patient in group 1 and in six additional patients in group 2. CONCLUSION: The administration of secretin improves visualization of the pancreatic ducts and helps in the evaluation of exocrine reserve.

New prospects for ultrasound contrast agents.

Considering the several suggestions regarding the future developments of echocontrast agents, there is a striking difference between the few compounds actually available on the market and used in clinical practice and those undergoing experimental clinical trials. It is therefore difficult to predict what will be the actual impact of these agents in the next future. Future developments will probably go beyond color enhancement which was the end-point till a very short time ago. They can be schematically summarized as follows: (1) development of new substances which enhance both color and gray scales; (2) use of new-generation substances, such as BR1 (Bracco, Milan, Italy) and EchoGen (Sonus, Bothell, WA), which use a gas other than air, such as perfluorate compounds which are more stable and guarantee longer and stronger effects; (3) use of more complex compounds acting at different levels. For example, SHU 536A (Sonovist) produces resonance phenomena with the second and third harmonics, and also stimulated acoustic emission which permits the morphological study of liver parenchyma. Other promising compounds are liposomes and aerosomes. Among the new possibilities in recording and observing phenomena, we can distinguish two main application fields: one is based on the physics of ultrasound and related to the presence of microbubbles in an acoustic field. These phenomena are generally obtained increasing the emission acoustic pressure, which eventually results in microbubble destruction and they are called nonlinear because there is no direct relationship between emission and return frequencies. These phenomena, which are detectable only with dedicated equipment, include: the resonance phenomenon with harmonic emission; intermittent harmonic emission and stimulated acoustic emission. The other application field is not strictly related to ultrasound physics and includes all the systems which can detect the presence of microbubles qualitatively or quantitatively. Other possible applications are related to the possibility of acquiring not only morphological but also functional data, especially in cardiology and neurology. Finally, targeted agents are potentially capable of demonstrating receptor sites or specific molecules, which may open very interesting therapeutic routes.

[Dynamic pancreatography with magnetic resonance after functional stimulus with secretin in chronic pancreatitis].

AIM OF THE STUDY: Magnetic Resonance pancreatography (MRP) was performed before and after the intravenous injection of secretin to assess the improvement in pancreatic duct visualization and to perform a dynamic study of the pancreatic exocrine function. MATERIAL AND METHODS: 20 MRP examinations were performed in 18 patients with suspected or known chronic pancreatitis. Coronal T2-weighted half-Fourier SSFSE images were obtained with a phased array surface coil. Images were obtained before and up to 10 minutes after the injection of 1 cu/kg b.w. secretin. Quantitative image analysis included main pancreatic duct enlargement over time after secretin injection and the amount of duodenal filling. Qualitative image analysis included: overall image quality improvement, number of pancreatic duct segments visualized, secondary ducts dilation, intraductal filling defects, the presence of pancreas divisum. RESULTS: After secretin injection the overall image quality was judged sufficient in 2 patients and satisfactory in 18 patients. The number of pancreatic duct segments visualized increased from 40/57 (79%) to 57/57 (100%); secondary ducts were visualized in 4 patients before secretin compared to 18 after secretin. The number of stenosis visualized increased from 6 to 9, while intraluminal filling defects increased from 2 to 6. Pancreas divisum was detected in 2 patients after secretin versus 0 before secretin. The main pancreatic duct enlargement was statistically significant in the head of the pancreas (p < .05). Duodenal filling was normal in 13 patients and decreased in 7. DISCUSSION AND CONCLUSIONS: Secretin injection extends the capabilities of MRP in visualizing the morphologic features of pancreatic ducts. The depiction of pancreatic ducts, stenosis, filling defects and pancreas divisum was improved after secretin injection. The exocrine function of the pancreas can be evaluated analyzing the entity and the timing of the duodenal filling.

[The echo-Doppler evaluation of the effects of glucagon on the portal flow in normal subjects. The premise for the loading test in portal hypertension].

This study was aimed at evaluating the capabilities of Doppler US in the quantitative study of portal blood flow after glucagon administration in normal subjects. While its role in the qualitative study of portal hemodynamics is well known, it remains questionable whether Doppler US may be a valuable tool for the quantitative study of portal blood velocity. This may be solved by introducing, on a routine basis, meal tests which may provide a relative evaluation of the data. To date, these tests were based on the evaluation of blood flow velocity before and after a complete meal. For a more reliable test, we suggest that the meal be substituted with the i.v. administration of glucagon, which is known to play a major role in splanchnic hemodynamics during digestion. We examined 30 healthy volunteers and showed the possibility of a statistically significant evaluation of portal blood flow velocity before and after glucagon administration. After glucagon administration, blood flow velocity increased by 31.3% in 10 minutes, on the average, in all the healthy subjects.

Recurrence of ovarian and uterine neoplasms: diagnosis with transrectal US.

Twenty-one patients with clinically suspected recurrence of ovarian (n = 3) or uterine (n = 18) carcinoma were examined with suprapubic ultrasound (US) and transrectal US with high-frequency linear probes. The examinations were performed 3, 6, 9, and 15 months after surgery and radiation therapy. Eight patients underwent radiation therapy before surgery and ten after surgery; three underwent only surgery. Criteria for recurrence included increased anteroposterior diameter of the vaginal cuff (greater than 2.2 cm); structural alterations or presence of a mass in the vaginal cuff; and infiltration of the rectovaginal septum, bladder, and parametria. Transrectal US findings were true positive for recurrence in nine cases, true negative in ten, and false positive in two. US findings were true positive in three cases, true negative in seven, false positive in two, and false negative in three. In six cases results from US were technically poor, and no diagnosis could be made. Transrectal US was highly sensitive in detection of pelvic recurrent carcinomas, while US had little diagnostic value. The authors believe transrectal US can replace US in the evaluation of patients at risk for recurrent pelvic neoplasm.

[Hemobilia: comparison of diagnostic technics. Considerations on 14 patients].

Fourteen cases of hemobilia of traumatic (six), vascular (six) or neoplastic (two) origin were reviewed. Eleven patients obtained correct diagnosis only be integration of at least two diagnostic procedures out of Ultrasonography, Computed Tomography, arteriography and cholangiography. Although US and CT are extremely useful as screening procedures, correct diagnosis should be only possible by integration of more diagnostic technics. Arterial embolization represents a successful alternative to surgical intervention; moreover US may also be helpful for follow-up of treated patients.

Sonographic patterns in splenic infarct.

The sonographic findings in eight cases of splenic infarct at their onset and at different phases of their development have been retrospectively analyzed. A wide range of appearances was seen (single or multiple, rounded or wedge-shaped, echo-free, hypoechoic, and hyperechoic lesions). In our opinion, such variable appearances are related to the age of the infarct (hypoechoic or echo-free in the earlier stages, hyperechoic in healed infarcts). A presumptive diagnosis is possible in the earlier stages since the detection of changes (in echogenicity and/or in size) over a period of time strongly suggests an infarct. On the other hand, a hyperechoic wedge-shaped lesion is fairly typical of healed infarcts.

[Ultrasonic diagnosis, staging and follow-up of prostatic neoplasms].

120 patients with clinical suspected prostatic neoplasm were evaluate by transrectal linear sonography. 26 normal glands, 57 hypertrophic adenomas and 37 adenocarcinomas were recognized. We had 17 false positive cases for adenocarcinoma in hypertrophic adenomas. We had no false negatives. All diagnosis were confirmed bioptically, by CT or surgically. The findings of 37 adenocarcinomas are examined. The US criteria for early diagnosis (T1-T2), advantages and limits of transrectal linear probe in staging and follow-up, are discussed.

[Comparative study of hepatic metastasis with different modality ultrasound scanners].

57 patients affected by liver metastases were examined with different types of dynamic ultrasound scanners (linear-sector-convex) in order to assess the advantages and the limits of each scanner and to evaluate if a better diagnostic accuracy is possible by using 2 types of scanner in the same patient. The results were estimated at 3 different levels: quantitative analysis where we considered only the presence or not of liver metastases; complex quantitative analysis where we also evaluate number and site of the metastases; qualitative structural analysis where we compared the ultrasonographic appearance of the metastases. All the results were evaluated by a personal computer with a special program and a statistical analysis was elaborated.

Hyperplastic cholecystosis: study by ceruletide-assisted cholecystography.

Five hundred and two patients with good opacification of the gallbladder were studied by means of ceruletide-assisted cholecystography. A high percentage (15.7; 79 patients) was found to have hyperplastic cholecystoses. So far 26 of these patients have been operated upon because of gallstones or painful symptoms. The x-ray diagnosis was confirmed in all cases. Compared with routine cholecystography, the powerful contraction induced by ceruletide appears to lead to a more frequent recognition of hyperplastic cholecystoses.

[Echography inthe study of the gallbladder wall].

In 511 patients with clinical symptoms of chronic or acute biliary disease, gallbladder area was studied by automatic equipments. In 39 patients different features of the gallbladder wall lesions were demonstrated, in some cases associated with gall stones or bile modifications. Gallbladder wall lesions were divided in three different groups, diffuse, localized or both, according to their appearance and extension. Correlations between the echographic patterns and the clinical and pathological pictures were established. The authors concluding stress the diagnostic value of cholecystosonography in detecting wall lesions such as chronic or acute cholecystitis, cholecystosis and gallbladder tumours. The indications of cholecystosonography are in such a way extended well beyond the demonstration of gallstones and bile modifications to the diagnosis of chronic and acute cholecystitis.

Bilateral pseudocystic pheochromocytoma.

A case of bilateral pheochromocytoma presenting as primarily cystic suprarenal masses is reported. The computed tomographic manifestations, possible etiology, and differential diagnosis of these cystic suprarenal lesions are discussed.

[Multi-institution, multidisciplinary radiology and endoscopy trial of the diagnostic accuracy of the double contrast examination of the stomach surface].

In 12 X-ray and gastroenterology departments 1570 patients, clinically suspected to have a gastric pathology, have been examined by double contrast (surface) examination of the stomach followed by gastroscopy. Diagnostic accuracy was 93% in respect of endoscopic data, assumed correct by hypothesis. On the basis of a cost and benefit evaluation, made according to the well known criteria, a wider routine use of the double contrast technique is proposed.
