Use of dot immunobinding assay for the rapid diagnosis of human hydatidosis.

A Dot Immunobinding (DIB) assay has been applied to the serodiagnosis of human hydatidosis and its results have been compared with those obtained with the enzyme-linked immunosorbent assay (ELISA). The two techniques have been shown to be closely related (p greater than 0.001), highly sensitive (92.0% of positive results in 75 sera from patients with hepatic or pulmonary hydatidosis) and specific (93.5% of negative results in 31 sera from patients affected by other parasitic diseases and 100% of negative results in 30 normal controls). DIB however is more economical and takes less time (only 4 hours) than ELISA. DIB could be an useful tool in field epidemiological surveys since it is sensitive, specific, simple to perform and it does not require any expensive apparatus.

Clinical experience with a new dynamic display system for ambulatory ECG recordings.

The authors present the results of clinical experience with a new display for ECG signal presentation in ambulatory ECG monitoring. The new method has been devised by adding to contourography the dynamic effect produced by image movement and the perspective effect produced by pseudo-tridimensional presentation. The system has been developed using a microprocessor, a dynamic memory and an arithmetic unit which sends the signal on a CRT screen. Clinical experience, based upon 150 analyses, demonstrates the new method's marked efficiency in presenting rhythm disturbances and particularly ST-T segment alterations. Clinical validation has been performed comparing the results of 50 tapes analysed both with the new method and with different commercial systems. The new method provides useful information as to ST-T segment in 98% of cases and as to rhythm disturbances in 90%, while commercial systems reveal ST-T alterations in 84% and arrhythmias in 70% This method, being devoid of automaticity, does not yield false positives; dynamic and pseudo-tridimensional visualization of ECG signal allow the operator to perceive with high sensitivity alterations of the observed tracing.

[Radial counter-immunoelectrophoresis in the determination of specific anti-hydatidosis antibodies. Preliminary observations].

Radial counter-immunoelectrophoresis was evaluated with sera from surgically-confirmed hydatidosis cases, patients with other parasitic diseases and healthy controls. Radial counter-immunoelectrophoresis was a sensitive as classic counter-immunoelectrophoresis based on the same positivity criterion and was equally specific for the immunodiagnosis of human hydatidosis. The procedure appears to be simple and rapid (the immunoprecipitation step requires just 5 minutes) and merits consideration.

Antibodies

New trends in the rapid serodiagnosis of leptospirosis.

TR/Patoc slide test, ELISA and TIA (Thin-Layer Immunoassay) were evaluated for their use in a rapid serodiagnosis of human leptospirosis. The results obtained indicated that the TR/Patoc slide test is very sensitive and able to detect within a few minutes early antibodies against whichever serovar of leptospires, like the antigens of the Galton test. The value of the ELISA test in detecting antibodies at a low level is confirmed. The TIA assay, employed here for the first time for the diagnosis of leptospirosis, though sensitive seemed too long to perform and consequently it is not a test of choice for a rapid screening of sera.

[Computerized axial tomography in the study of orbital pathology].

In recent years, C.A.T. has revolutionized the diagnostic iter in the field of orbitary pathology. Although echotomography has reached very good results in this field, C.A.T. still remains, according to the Authors the most reliable diagnostic approach, because of its excellent insight of normal and pathological orbital material. the analysis of personal casistics of 120 orbital tomodensitometric examinations has proved this event, especially as retrobulbar expansive pathology is concerned.

[A new system for dynamic electrocardiography. First clinical trial].

The development of a reliable system for analysis of ambulatory ECG recordings, for evaluation of arrhythmias and ST-T segment changes is a great challenge for technology and cardiovascular medicine. Signal superimposition or AVSEP system, conceived by Norman J. Holter, has been widely used and is employed even in recent sophisticated devices for analysis of ambulatory ECG recordings. As an alternative way we have chosen contourography, known since 1966, enhanced by a dynamic and perspective effect (Fig. 1). Figure 2 shows the display mechanism of the ECG signal. Cardiac cycles are continuously sampled, triggered and positioned along the running time axis. The QRS complexes undergo progressive amplitude decrease while "sliding" toward a viewpoint adjustable by the operator (Fig. 3). As memory contains a fixed number of cycles, the latest cycle enters the screen while the oldest falls out. Instead of being superimposed as in AVSEP system, cardiac cycles are displayed separately. In this way it is possible to make comparisons between one cycle and another or among different areas of the ECG tracing. At any instant, the tracing can be stopped, and an intensified area of interest can be recorded on a paper strip in a standard fashion (Fig. 4). A clinical comparison has been performed between this device and two different commercial systems; 25 tapes, 24 hours each, have been separately analyzed by two cardiologists with the Avionics system and 25 with the Pathfinder, and then with the new system. Leishmaniasis in Sardinia: an introductory note.

Preliminary data on a project planned in Sardinia for the epidemiological study of leishmaniases are presented. Twenty-six dogs were found infected in the south-western part of the island. Nine isolates were successfully stabilated . In a canine leishmaniasis focus on the outskirts of Cagliari, Phlebotomus perniciosus was the predominant sandfly species (95%). In the same area, 28 specimens of Rattus norvegicus were trapped and their spleen homogenate was injected into hamsters. No Leishmania has yet been isolated.

[Evaluation of ELIEDA test for immunodiagnosis of hydatid disease].

An ELIEDA test for hydatidosis was evaluated with sera from preoperative, surgically-confirmed hydatidosis cases, postoperative hydatidosis patients, persons with other parasitic disease and healthy donors. ELIEDA was more sensitive than the counter-immunoelectrophoresis test based on the same positivity criterion and was equally specific for the immunodiagnosis of human hydatidosis. The procedure appears to be simple and rapid and merits consideration in that it determines specific classes of antibodies involved in the immune response which appear to be useful in following the evolution of hydatid cysts.

Venous obstruction in permanent pacemaker patients: an isotopic study.

Isotope venography was used to study the venous circulation proximal to the superior vena cava in two groups of pacemaker patients, one with a single endocavitary electrode and the other with multiple pacing catheters. A control group of patients without pacemakers was also studied. Numerous abnormalities were found, especially in the group with multiple electrodes. These findings suggest that venous obstruction is a common complication of endocardial pacing.

Superior vena cava stenosis at site of intersection of two pacing electrodes.

Superior vena cava obstruction is described in a patient with two endocardial pacing electrodes. At necropsy a severe stenosis of the venous lumen was found at the site of intersection of the two catheters. There was no evidence of thrombus formation. Venous wall stenosis is an unusual complication of transvenous pacing and is probably favoured by the presence of two electrodes.
