Macroscopic and histological changes of the ileum HLA-B27 related diseases: therapeutic consequences.

Ileocolonoscopy and microscopic examination of ileum biopsies have been performed on patients with reactive synovitis, ankylosing spondylitis (AS) and on a control group. Histological signs of gut inflammation were present in practically all patients with reactive synovitis, with the exception of the patients with sexually-acquired disease. In the AS group, inflammation of the ileum was observed in the HLA-B27 negative patients and in AS B27 positive patients with peripheral joint involvement. Occasionally, changes of the ileum were seen in the B27 positive AS patients without peripheral joint involvement. Signs of gut inflammation were absent in all controls. These data suggest that chronic inflammation of the ileum could be implicated in the pathogenesis of some cases of reactive synovitis and even in the peripheral joint involvement frequently seen in ankylosing spondylitis. In the second part of the study, sulfasalazine (Salazopyrin) was administered to 15 HLA-B27 positive patients with reactive synovitis, who had failed to respond to non-steroidal anti-inflammatory drugs. In 11 of the 15 patients, a clinical and biological remission occurred 3 to 11 months after the start of sulfasalazine treatment. The frequency of spontaneous remissions in reactive synovitis calls for confirmation of these encouraging results in double-blind controlled studies.

[Results of treating infections of the cervical mucosa with doxycycline polyphosphate in 53 infertile women].

Our study was carried out on 53 women who had been infertile for more than a year and who had cervical mucus infection. In 13 cases (group A) there was no other known associated factor to cause the infertility. In 26 cases (group B) there was an obvious associated factor and in a further 14 cases there was a latent associated factor. The physical characteristics as well as the bacteriological appearances of the mucus (which included a systematic search for aerobic bacteria and for chlamydia trachomatis (CT) and for ureaplasma urealyticum (UU) were studied both before and 6-8 weeks after treatment with doxycycline polyphosphate given in doses of 100 mg twice daily. Before treatment the bacteria most often found were escherichia coli (20 cases) and enterococci (18 cases). CT was only found once and UU six times, usually in association with other bacteria. In 75.5% of cases the treatment made the affecting organism go; and cut down or removed entirely the number of leucocytes in the mucus in 73.5% of cases. But the physical characteristics of the mucus were only improved in 43% of cases, and in 18 cases a new infective organism was found in the mucus after treatment. However, it was in too low a concentration to determine its pathogenicity. There were nine pregnancies within 4 months of the start of treatment, which means that 30% in group A and 19% in group B became pregnant. Infection of the cervical mucus therefore does really seem to be a factor in lowered fertility in certain women CT and UU seem to be rarely the responsible organisms. Doxycycline phosphophate in an efficient and well tolerated treatment in these infections.

Effect of sludge ice cooling on renal function and renal histology in the dog.

The effect of sludge ice surface cooling on the compensatory hypertrophied dog kidney was investigated. Renal function was measured prior to and on days 1, 3 and 7 after the cooling procedure by means of inulin clearance, PAH clearance and sodium excretion capacity during normal hydration and after volume expansion. No alteration in renal function was shown. No freezing lesions or thromboses were seen on histological examination.

[Aspergillosis of the maxillary sinus].

A new case of aspergillosis of the maxillary sinus is presented. The symptoms were those of a chronic unilateral sinusitis, which did not respond to conventional treatment. The diagnosis was suspected at sinoscopy and subsequently confirmed by histo-pathological examination. A Caldwell-Luc operation was performed and proved curative. A review is made of the literature and the epidemiology, symptoms, diagnosis and treatment of the different forms of paranasal sinus aspergillosis are discussed.

[Basal cell nevus syndrome with sinus involvement].

The characteristics of the basal cell naevus syndrome are multiple basal cell naevi and basal cell carcinomas, cysts of the jaws, anomalies of ribs and vertebra, intracranial calcification and striking deformities of the skull, usually with a positive family anamnesis. We will discuss symptomatology, histopathology and therapy. A flemish family, where the syndrome occurs with the mother and her four children, three of whom are admitted in hospital nearly simultaneously with sinusitis maxillaris, are discussed in extenso. Also, the occurrence of the syndrome's stigmata in other members of the family is noted.

A progressive congenital myopathy. Initial involvement of the diaphragm with type I muscle fiber atrophy.

An uncommon case of initial respiratory distress during the first months of life as the result of bilateral diaphragmatic weakness is presented. The biopsy and necropsy findings show a progressive congenital myopathy with type I muscle fiber atrophy and predominant involvement of the respiratory muscles. The lesions, observed in the central nervous system are due to the severe hypoxia. The morphological findings are discussed in relation to the etiology and the clinical picture of the disease.

Nasal polyposis: from cytokines to growth.

Nasal polyposis (NP) is a chronic inflammatory condition that is mostly characterized by an infiltration of eosinophils. How this eosinophilic inflammation leads to polyp formation remains largely unclear. In order to identify the most important factors in polyp growth, first we report the histologic features of two early stage manifestations of eosinophilic nasal polyps compared to their surrounding normal mucosa and mature polyps from the same patients. Histomorphologic analysis of these early stage manifestations of NP showed the presence of eosinophils, forming a subepithelial cap over a pseudocyst area that was filled with albumin. In mature NP, a large pseudocyst area containing albumin was surrounded by subepithelial eosinophilia. Second, in an approach to quantify and to study possible relations between eosinophilic inflammation and changes in extracellular tissue components we measured interleukin-5 (IL-5), eotaxin, eosinophil cationic protein (ECP), leukotrienes (LTC4/D4/E4), transforming growth factor-beta 1 (TGF-beta 1), fibronectin, hyaluronic acid, and albumin in nasal tissue homogenates of 31 subjects. Nasal polyp samples (n = 16) were obtained during routine endonasal sinus surgery, whereas control non-polyp samples (n = 15) from subjects with (6) and without (9) allergic rhinitis were obtained from the inferior turbinate during septum surgery. In the group of polyp patients 11 received no treatment, whereas 5 were treated with oral glucocorticoids (GCS) within 4 weeks before surgery. IL-5 was measurable in 8 of 11 untreated NP, whereas IL-5 could not be detected in all 15 controls nor in 4 of 5 oral corticoid-treated polyps. The comparison between the untreated polyp group and controls showed significantly higher concentrations of IL-5, eotaxin, ECP, and albumin in polyp supernatants, whereas TGF-beta 1 was significantly lower. In the oral GCS-treated group, ECP and albumin were significantly reduced compared to untreated nasal polyps. The same tendency, but not reaching significance, was seen for eotaxin and fibronectin, while no difference was found for LTC4/D4/E4 and hyaluronic acid between the groups. Our observations suggest a deposition of albumin (and possibly other plasma proteins) and extracellular matrix proteins, which may be regulated by the subepithelial eosinophilic inflammation, as a possible pathogenic principle of polyp formation and growth. IL-5 and eotaxin are found to be key factors for eosinophilic accumulation and activation in NP. Oral corticoid treatment may lead to the shrinkage of NP by downregulation of the eosinophilic inflammation and reduction of the extravasation and deposition of albumin in NP.

Gut mucosal T cell lines from ankylosing spondylitis patients are enriched with alphaEbeta7 integrin.

OBJECTIVE: An intriguing link between gut and synovial inflammation exists in patients with spondyloarthropathy (SpA), illustrated by the high frequency of microscopically inflammatory gut lesions observed in these patients. We hypothesise that aberrant homing of mucosal T cells might play a role in the induction/perpetuation of arthritis in SpA. Here, we analyse the expression of the homing molecules alpha4beta7 and alphaEbeta7 on mucosal T cells from patients with ankylosing spondylitis (AS) and controls, in view of the critical role of these receptors in the homing of mucosal lymphocytes. METHODS: Colonic biopsy specimens were obtained from patients with AS (n = 23) and controls (n = 30). Biopsy specimens were immunostained, treated for extraction of intraepithelial lymphocytes (IEL) and lamina propria lymphocytes (LPL) or cultured in the presence of IL-2. The expression of the beta7 integrins was investigated. RESULTS: In situ no differences were observed in alphaEbeta7 and alpha4beta7 integrin expression in isolated IEL and LPL, whether determined by flow cytometry or by immunohistochemical staining. In gut mucosal T cell lines, alphaEbeta7 expression was significantly higher in the mucosa of patients with AS compared with controls. Alpha4beta7 was highly expressed on T cells in both groups studied. Mucosal T cells either expressed only the alpha4beta7 integrin or co-expressed the alpha4beta7 and alphaEbeta7 integrins. Almost none of them expressed only the alphaEbeta7 integrin. CONCLUSION: In gut mucosal T cell lines from patients with AS an increased expression of alphaEbeta7 was observed.

Metastatic follicular dendritic cell sarcoma of the stomach: a case report and review of the literature.

Follicular dendritic cell (FDC) sarcomas are rare tumours, typically seen in lymph nodes. However, in about one third of the reported cases, a FDC sarcoma presents as an extranodal mass. Involvement of the gastrointestinal tract is extremely rare, and only 3 cases have been described to date. We report on a 40-year-old female patient with a follicular dendritic cell sarcoma located in the stomach and the presence of a metastasis in the liver at the time of diagnosis. Severe asthenia, nausea, back pain and loss of weight were the presenting symptoms. A CT scan of the abdomen and an upper gastrointestinal endoscopy revealed a tumour mass in the stomach. The diagnosis of a FDC sarcoma was made on histological and immunohistochemical findings. We report the second case of a FDC sarcoma presenting in the stomach. Due to its rarity, a FDC sarcoma seldom enters the differential diagnosis of spindle cells neoplasms of the gastrointestinal tract. Complete surgical resection is the treatment of choice for FDC sarcoma.

Ileal mucosal mononuclear cells in patients with seronegative spondylarthropathy.

In order to study local immunoregulatory mechanisms in patients with seronegative spondylarthropathy and histological evidence of asymptomatic intestinal inflammation, we determined mononuclear cells in distal ileal mucosa with an indirect immunoperoxidase technique using a panel of monoclonal antibodies. The number of intraepithelial lymphocytes in inflamed mucosa was not significantly increased. They carried mainly the suppressor-cytotoxic CD8 marker (greater than 80%), less frequently the helper T cell marker CD4 and lacked the pan T epitope CD3 in greater than 50%. There were no intraepithelial B cells or natural killer cells. The lamina propria lymphocytes were T and B cells, the former being more numerous. Helper T cells were more frequent than suppressor-cytotoxic T cells. There were no alterations in the proportion of T cells in inflamed mucosa. Natural killer cells were scarce. They occurred in small numbers in a minority of cases with gut inflammation and spondylarthropathy and rarely in inflammatory bowel disease. It is concluded that although there is an increase of lamina propria monocytes, there is no immunoregulatory imbalance in the ileal mucosa of patients with seronegative spondylarthropathy.

Subclinical involvement of the gut in undifferentiated spondylarthropathies.

In order to evaluate the frequency of subclinical gut involvement in the seronegative spondylarthropathies, ileocolonoscopy with biopsies of the colon, ileocecal valve and ileum were performed on 211 patients with ankylosing spondylitis (AS), reactive arthritis (ReA) and undifferentiated spondylarthropathies. Inflammatory gut lesions were detected in a large number of these patients. It was concluded that the group of undifferentiated spondylarthropathies could be split into fairly equal subgroups: one subgroup suffering from subclinical inflammatory bowel disease associated with peripheral joint symptoms, a second subgroup presenting a form of enterogenic ReA, and a third subgroup in which no relation with the gut could be demonstrated. Ankylosing spondylitis, which has to be considered as a form of undifferentiated seronegative spondylarthropathy, could be subdivided into the same subgroups. These findings confirm the existence of subclinical gut involvement in patients with seronegative spondylarthropathies.

Pigmented villonodular synovitis of the hip--association with osteochondromatosis.

Pigmented villonodular synovitis, diffuse or nodular in nature, is most commonly seen in the knee but occasionally involves the hip. The authors reviewed the medical records of seven patients with a pathological diagnosis of unilateral pigmented villonodular synovitis of the hip. Characteristic findings were proliferation of the synovial lining cells and subsynovial invasion of fibroblast or polyhedral mesenchymal cells in a nodular pattern. Multiple initiating factors have been advanced to account for the occurrence of this monoarticular chronic arthritis, but the etiology and pathogenesis still remain obscure. On three occasions the authors found an association with free floating osteochondromas and well-delineated foci of metaplastic cartilage under the synovial lining cells. Two benign lesions may occur simultaneously and are possibly related entities involving the synovial membranes as a reaction to a common etiologic factor. Although the histopathology of pigmented villonodular synovitis of the hip and the knee is identical, localization in the former joint has its typical clinical, radiological, differential diagnostic and therapeutic aspects.
