[A new procedure for measuring displacement of the anterior ankle joint in the lateral projection].

Different methods are recommended for exploration of stress-radiographs of the ankle joint in lateral projection. After considering the geometric-mathematical propositions a new schedule is shown, which combines simple handling with tolerable error level and regards individual bone size. An index is derived from measurements of the tibia and the talus according to certain conditions. The limits for normal, borderline and pathological indices were statistically determined by measurement of 50 lateral stress-radiographs of uninjured ankle sprains. In comparison with the common measuring of an posterior tibio-talar distance better results were found with a higher score of discrimination for the new method. Additionally the new procedure allows exploration of radiographs which do not fulfill the demands to an exact lateral projection of the ankle sprain.

[Lymphoepithelial carcinoma of the nasopharynx: the treatment results following radiotherapy].

Data of 17 patients with lymphoepithelial carcinoma of the nasopharynx treated with telecobalt were reviewed. The follow-up period was five to nine years. 16/17 patients had lymph node metastases at the initial presentation. Cervical lymphadenopathy was the only tumor symptom in seven patients. The overall five-year survival was 70% (12/17 patients), the disease-free survival 47% (8/17). 2/10 patients survived seven years. Local recurrence in the nasopharynx was seen in 5/17 patients, recurrence at neck sites in 7/17. 5/7 lymph node recurrences developed outside the radiation field. Half of the recurrences appeared late (three to six years after radiotherapy). In 8/9 patients recurrence preceded the hematogenous spreading of the carcinoma. Therapy of lymphoepithelial carcinoma of the nasopharynx should always include radiation of the primary tumor and bilateral cervical lymph nodes with 60 to 70 Gy.

[Quantitative cardiac computed tomography].

The scope and limitations of quantitative cardiac CT have been evaluated in a series of experimental and clinical studies. The left ventricular muscle mass was estimated by computed tomography in 19 dogs (using volumetric methods, measurements in two axes and planes and reference volume). There was good correlation with anatomical findings. The end-diastolic volume of the left ventricle was estimated in 22 patients with cardiomyopathies; using angiography as a reference, CT led to systematic under-estimation. It is also shown that ECG-triggered magnetic resonance tomography results in improved visualisation and may be expected to improve measurements of cardiac morphology.

[Computed tomography, magnetic resonance tomography and echocardiography in hypertrophic cardiomyopathy].

The value of echo-cardiography, CT and magnetic resonance tomography was evaluated in 10 patients with hypertrophic cardiomyopathy. Echocardiography made the diagnosis in most cases. The diagnostic criteria depended on functional and morphological changes. CT and magnetic resonance can show thickening in the myocardium, either symmetrical or asymmetrical. Magnetic resonance tomography provides excellent demonstration of the myocardium and accurate delineation of the degree of hypertrophy. It is well suited for measuring cardiac wall thickness.

[Computer tomographic determination of left ventricular muscle mass].

Left ventricular muscle mass LVMM) was determined by computed tomography in 19 dogs (33 in vivo and 19 postmortem studies). CT values obtained with 4 models were compared with the actual LVMM at autopsy. Excellent correlation was achieved by volumetric analysis (model A; r = 0.95-0.96). Accurate assessment of LVMM was also obtained using a two-axis method (model B), an area-length method (model C), or a representative cross-sectional volume (model D). There was no significant difference between in vivo and postmortem studies. We conclude that CT can provide reliable estimates of LVMM in vivo. Volumetric analysis is not dependent on mathematical models of the left ventricle. However, this method is very time-consuming. The other CT-models are simple enough to permit clinical application. In comparison with other experimental and clinical methods of LVMM quantitation, CT is superior to chest X-ray, ECG, VCG, and M-mode echocardiography, while similar good results can be obtained with two-dimensional echocardiography as well as conventional and digital angiocardiography.

[The value of computer tomography in the diagnosis of local recurrences of carcinoma of the rectum (author's transl)].

The tendency of rectal carcinomas to recur makes it necessary to reexamine these patients repeatedly. The tissues remaining following resection of the rectum can only be examined directly by computer tomography. The value of this procedure in the diagnosis of local recurrences has so far not been established. The results of computer tomography must therefore be correlated with already established methods, particularly clinical examination and CEA measurements.

Hypothenar hammer syndrome caused by recreational sports activities and muscle anomaly in the wrist.

A 34-year-old man with digital ischemia is reported. Angiography revealed thromboembolic occlusions of the proper digital arteries of the index, middle, and ring fingers and a tortuous ulnar artery in Guyon's canal. Though hypothenar hammer syndrome was suspected, there was no relevant occupational history. Magnetic resonance imaging and magnetic resonance angiography demonstrated an anomalous muscular sling around the ulnar artery immediately adjacent to the hook of the hamate. The ulnar artery showed mural thrombi in its tortuous segment. These findings were confirmed during operative exploration. After thrombectomy and embolectomy the involved segment of the ulnar artery was replaced by an autologous vein graft. Postoperatively there was complete resolution of the symptoms. Only during convalescence did it become clear that the patient was a passionate golfer.

Percutaneous transluminal angioplasty for occlusion of the subclavian artery: short- and long-term results.

Percutaneous transluminal angioplasty was performed in 8 symptomatic patients with proximal occlusion of the left subclavian artery. Technical and short-term clinical success was achieved in 7 cases. Nonoccluding embolization to the distal subclavian artery and stenosis of the brachial artery after a combined femoral/brachial approach occurred as complications in 2 patients. Three patients are asymptomatic with a patent subclavian artery 25, 28, and 37 months after angioplasty. Reobstructions in 4 patients occurring after 8, 12, and 16 months were retreated by angioplasty (3 patients) and stent implantation (1 patient with a second restenosis) with good technical and clinical success. Long-term patency was less than 50%, but successful retreatment is feasible. Therefore, we consider percutaneous transluminal angioplasty a reasonable therapeutic option in patients who are not surgical candidates.

Portal vein erosion and acute abdominal hemorrhage as a complication of acute pancreatitis.

The case of a 35-year-old male with portal vein erosion and acute life-threatening abdominal hemorrhage as a complication of acute pancreatitis is reported. Erosion of arterial walls is the most common complication of pancreatitis and pancreatic pseudocysts, and erosion of the portal vein leading to a fistula between the pancreas and the portal vein is very rare. Diagnosis was made by multislice spiral computed tomography, and nonsurgical treatment was elected, leading to complete recoverage.

[Selective intraarterial axillar artery thrombolysis in a neonate: angiographic and thermographic serial time course].

Arterial thrombotic vascular occlusions in the neonatal period are rare. Intraarterial thrombolytic therapy in infants and children is unchanged rarely used but can be very effective. Thrombolytic therapy in adults is well established and a summary of clinical and pharmacologic aspects exists of different thrombolytic agents. Recombinant-Tissue-Plasminogen-Activator (rTPA) is increasingly used in thrombolytic therapy in infants and children in the last years. We report thrombolysis of axillar arterial thrombosis with rTPA in a premature infant. Additional thermographic images were performed, which were very helpful in the clinical monitoring of the newborn.

Diagnosing and staging of pancreatic carcinoma-what is necessary?

The aim of the present prospective observational study was to diagnose and stage pancreatic carcinoma with a minimum of diagnostic procedures. Our experiences in 307 patients with a histologically confirmed pancreatic carcinoma show that for diagnosing pancreatic carcinoma sonography and computed tomography are sufficient in 95% of the cases. The combination of both has a sensitivity equal to that of endoscopic retrograde cholangiopancreatography (ERCP; 96.8 vs. 98.7%; n.s., chi2 test). ERCP is only indicated in cases with negative sonography and computed tomography, and suspicion of pancreatic cancer. For tumor staging, the routine performance of angiography cannot be recommended in view of the fact that although it provides greater sensitivity for the evaluation of an infiltration of the portal vein (80% for angiography vs. 22% for sonography or computed tomography), it is associated with a lower positive predictive value (56.4 vs. 68 and 72%) which results in a lower accuracy. Despite recent advantages in diagnostic technology, less than 50% of unresectable tumors were identified preoperatively at a 10% false-positive rate. The major reason for unresectability is infiltration into the mesenteric axis, which cannot be identified laparoscopically. Laparoscopy or percutaneous biopsy is recommended only in the presence of a tumor with suspicion of distant metastasis detected by radiological imaging and requiring histological confirmation. In conclusion, sonography and computed tomography as the only diagnostic images are sufficient for diagnosing and staging of pancreatic carcinoma in more than 95% of the patients. Only a small number of patients needs further diagnostic procedures.
