[Transrectal echography in the infertile male. Statistical study].

We evaluated the transrectal echo patterns in 140 infertile men using the following parameters: the transverse and anteroposterior diameters of the prostate, the presence of periprostatic venus plexus, and the transverse diameter of the seminal vesicles. Patients were classified according to data gleaned from semen analyses following the criteria recommended by the WHO. Microbiological analyses using the Meares and Stamey technics, as well as semen analyses, were performed in those patients who presented with leukospermia. These were considered a separate, homogeneous group classified as "inflammatory". The following patient groups comprised the study: azoospermia (18 patients), oligospermia (4), astenozoospermia (43), oligoastenozosermia (48), and inflammatory (27). Statistical analyses of the ultrasound data for the infertile males and those of the control group were performed using the Mann-Whitney U. test and compared. Data were statistically significant for the following parameters: the transverse diameter for the infertile males with azoospermia, the anteroposterior diameter of the prostate for those patients with leukospermia, and the periprostatic plexus for those in the oligoastenozoospermia and inflammatory patient groups.

[Modification of the echographic pattern of carcinoma of the prostate treated with Lh-rh agonists].

We present the results of a study undertaken to assess the echo patterns and prostate size in 13 patients with prostatic carcinoma treated with Lh-rh agonists. The study revealed a gradual reduction of the anteroposterior and transverse diameters. After 12 months, these were 28% and 22% of their respective initial values. Furthermore, the prostatic echo patterns changed significantly in 66% of the patients, and infiltration of the seminal vesicle improved or became stable in 50%. These findings suggest that transrectal ultrasonography may be useful in the follow up of prostatic carcinoma patients receiving hormone therapy.

Antibiotic resistance and serotypes of Streptococcus pneumoniae from patients with community-acquired pneumococcal disease.

From August 1978 to December 1981, 200 Streptococcus pneumoniae strains isolated from adult patients with pneumococcal disease were tested for susceptibility to penicillin G, erythromycin, clindamycin, tetracycline, and chloramphenicol by disk diffusion. Minimal inhibitory concentrations (MICs) were determined by agar dilution and broth dilution. The sources (numbers) of these isolates were blood (111), cerebrospinal fluid (30), sputum (26), pleural fluid (16), and miscellaneous (17). Of the 200 strains, 18 were partially resistant (MIC, 0.1 to 1 micrograms/ml) and 2 were resistant to penicillin. A total of 144 (72%) strains were tetracycline resistant, 87 of which had MICs of greater than or equal to 64 micrograms/ml. Ninety (45%) isolates exhibited various degrees of chloramphenicol resistance, with MICs ranging from 16 to 64 micrograms/ml. Five strains were resistant to erythromycin and clindamycin. Eleven penicillin-resistant strains were also resistant to chloramphenicol and tetracycline. Twenty-one different serotypes were encountered among the 120 typed strains studied. The most prevalent serotypes, in order of frequency, were 3, 1, 5, 19, 8, 6, 9, and 4, representing approximately two-thirds of the total number of isolates serotyped. These findings clearly indicate the need to perform antibiotic susceptibility testing in all cerebrospinal fluid isolates and other clinical significant isolates.

Distinct glycosylation of serum proteins in patients with cancer: brief communication.

Concanavalin A and wheat germ agglutinin, lectins that interact with serum glycoprotein in a manner similar to the antigen--antibody reaction, were used as "antibodies" in a single radial immunodiffusion technique to test a coded serum panel (from the National Cancer Institute, Bethesda, Md., and the Mayo Clinic, Rochester, Minn.) containing a) 99 serum samples from patients with different types of malignant neoplasms of the gastrointestinal tract, prostate gland, and lung, b) 50 samples from patients with benign diseases of the same organs as those affected in the cancer patients, and c) 50 samples from apparently healthy smokers. The resulting precipitation rings were not correlated to serum protein concentration, and the differences (demonstrated by Student's t-test and with a generalization of the one-sided two-sample Kolmogorov-Smirnov statistic for evaluating diagnostic tests) established that serum glycoproteins are glycosylated differently in cancer patients than in people without cancer.

Silver nitrate: antimicrobial activity related to cytotoxicity in cultured human fibroblasts.

The aims of this study were to ascertain whether silver nitrate (AgNO3) concentrations below those used in clinical practice inhibit bacterial growth, and in parallel study the cytotoxic effects on human fibroblasts. The cytoprotective effects of fetal calf serum (FCS) were also evaluated. The cytotoxic effects of eight different silver nitrate concentrations were determined by assessing mitochondrial activity of viable cells capable of cleaving tetrazolium salts. Antimicrobial activity of AgNO3, range: 7-550 x 10(-5)%, was tested against Staphylococcus aureus, Citrobacter freundii, and Pseudomonas aeruginosa. Silver nitrate concentrations exerting antimicrobial effects were: S. aureus, >70 x 10(-5)%; P. aeruginosa, >/=270 x 10(-5)%, and C. freundii, >/=550 x 10(-5)%. With 2% FCS, the lowest AgNO3 concentration studied (7 x 10(-5)%) showed cytotoxic effects (cell survival 71 +/- 19%) at only 2 h of incubation. Under these conditions AgNO3 cytotoxicity was time- and concentration-dependent in all exposure periods. Cytotoxicity was greatly enhanced causing 76% fibroblast growth inhibition at concentrations of 14 x 10(-5)% and contact time of 2 h. The AgNO3 concentration of 7 x 10(-5)% was also cytotoxic with 5% FCS in the media compared with controls, although cell survival was higher than with 2% FCS. The cytoprotective action of FCS was clearly shown at the concentration of 10% at which AgNO3 cytotoxicity of 7 x 10(-5)% to 28 x 10(-5)% was partially or completely inhibited. Our results show that AgNO3 at concentrations 100-700 times more diluted than that normally used in clinical practice retained effective inhibitory activity against some of the above-mentioned microorganisms. However, even these concentrations are cytotoxic for cultured fibroblasts. Thus, silver nitrate concentrations up to 100 times more diluted can be used, since they possess bacterial growth-inhibiting power, are less cytotoxic and therefore favour wound healing.

Spectrographic measures of the speech of young children with cleft lip and cleft palate.

Twenty-two consecutive children with repaired cleft lip and/or palate [isolated cleft lip (CL) 6, isolated cleft palate (CP) 7, unilateral cleft lip and palate (UCLP) 7, and bilateral cleft lip and palate 2] with a mean age of 27 months underwent spectrographic measures of tape-recorded speech (DSP Sona-Graph digital unit). Controls were 22 age- and sex-matched noncleft children. Data analyzed included (1) the Spanish vocalic variables [a, i, u, e, o]: first formant, second formant, duration, and context; (2) obstruent variables [p, t, k]: burst, voice onset time, and duration, and (3) nasal variables [m]: first formant, second formant, and duration. Statistically significant differences were observed between the CL group and the control group in the first formant of [e] and in the increase of the frequency of the [t] burst. Comparison between UCLP and controls showed differences in the second formant of [a], in the first formant of [o], and in the second formant of [o]. These results suggest a small but significant influence of either the cleft lip or its repair on lip rounding for [o] and [u]. In addition, tongue position differences were most likely responsible for the differences seen with [a] and [e]. Spectrographic differences in the current patients did not contribute to meaningful differences in speech sound development. Individualized care (orthodontics, surgery, speech therapy) in children with cleft lip and/or palate attended at specialized craniofacial units contributes to normalization of speech development.

Allergic reactions following skin contact with fish.

We evaluated the clinical characteristics found in 29 children who showed allergic symptoms after direct and/or indirect contact with fish, from 197 children diagnosed with IgE-mediated fish hypersensitivity. Allergic reactions to fish through ingestion began in most patients (79%) within the first 24 months of life. All the patients showed cutaneous symptoms, either alone or associated with other clinical manifestations. Whiff and hake were the species more frequently implicated in eliciting clinical manifestations upon ingestion. After diagnosis, all patients were placed on a strict fish avoidance diet. During this period of avoidance, 29 patients reported allergic reactions (mean age 5 years, 6 months; SD 4 years, 2 months) after incidental skin contact with fish. Clinical manifestations after touching fish were only cutaneous in 28 of the 29 studied patients. One showed local urticaria with wheezing. The majority reported two or more episodes. Incidental skin contact with fish could play an important role in accidental encounters with fish particles in children on a fish avoidance diet for fish IgE-mediated hypersensitivity. Such exposures could result in immunologic immediate contact reactions and in some cases could lead to systemic reactions (contact urticaria syndrome).

[Long-term results of an eradication regime against Staphylococcus aureus in patients treated with peritoneal dialysis].

Catheter exit site infection (ESI) remains a common complication in peritoneal dialysis patients. All the efforts for controlling ESI have been focused on the preventive eradication of Staphylococcus aureus (SA) colonization, because this microorganisms has been shown to be implicated in most of ESI. The main aims of the present study was to analyse the long-term results from an eradicative regimen of SA colonization, and to compare them with those obtained from a historical control group. From january 1993 to december 1999, 60 unselected patients on PD underwent an exhaustive protocol of SA eradication. Every 30-45 days, cultures from nares and exit site were obtained in each patient. SA colonization in nares or exit site was treated with mupirocin, though the colonization of other microorganisms was not treated prophylactically. Thirty patients from the same unit who were followed between 1989-1992 served as historical control group. The rate of peritonitis and ESI, as well as the epidemiological data from the cultures were also analysed. The peritonitis and ESI rates were significantly less in the study group than those in the control group (0.398 +/- 0.553 vs 0.899 +/- 0.970 ep./pat/year, p = 0.002; and 0.102 +/- 0.235 vs 0.340 +/- 0.553 ep./pat/year, p = 0.004). The ESI rate caused by SA was also significantly less in the study group (0.018 +/- 0.096 vs 0.300 +/- 0.53 ep./pat/year, p = 0.0001), though there was a statistically nonsignificant increase in the ESI rate caused by gram negative microorganisms in the study group (0.066 +/- 0.194 vs 0.040 +/- 0.219 ep./pat./year). The percent of patients free of ESI was larger in the study group (80% vs 63%, p = 0.01), though the percent of patients with more than one ESI was the same in both groups (10%). Nasal and exit-site SA colonization occurred in 52% and 32% of the study patients. The rate of catheter loss was less in the study group, though it did not reach statistical significance (0.043 +/- 0.154 vs 0.178 +/- 0.443 losses/pat./year). In conclusion, the eradication of SA colonization is an efficacious measure for the control of ESI. However, further efforts should be carried out in order to control the emergence of gram negative microorganisms, and to discover which factors make a small proportion of PD patients to be more prone to develop ESI of whatever origin.

[Directed chemotherapy in superficial carcinoma of the bladder conditioned by the knowledge of recurrence factors].

A retrospective study of 56 patients (23 primary and 36 relapsing) with superficial vesical carcinoma and treated with transurethral resection and directed chemoprophylaxis was done. Directed chemoprophylaxis is define as a specific type of adjuvant therapy where the drug is determined by a previous study with multivariant analysis of the relapse risk factors of a group of 385 cases, where the chemotherapeutic agent was also a variable. The drug is specified by the data introduced in a microprocessor program. Such chemoprophylactic approach has achieved a marked reduction in the relapse rate and increased the disease-free interval from 8 to 3 and from 70 to 83.5 respectively, with regard to the free drug choice approach.

[Disseminated renal adenocarcinoma. Developmental study].

A study was undertaken on 57 patients diagnosed as having disseminated renal adenocarcinoma. The interval between the onset of symptomatology and diagnosis ranged from 1 to 48 months. The tumor frequently metastasized to lungs and bone. To evaluate survival, this patient population was divided into two groups: those who underwent nephrectomy, 17 patients (9 patients also underwent lymphadenectomy), and those who were put on drug therapy alone. No significant difference was observed with respect to survival for both patient groups.
