Abnormal n-nonacosane storage in humans: detection by gas chromatography/mass spectrometry of tissue extracts.

We report a case of n-nonacosane storage disease, which went undiagnosed until the death of a 55-year-old farmer. Clinical, histological, and biochemical features are discussed. n-Nonacosane storage was identified by gas chromatographic-mass spectrometric analysis of different tissue extracts, n-nonacosane concentration reaching 1.2 mg g-1 of lung tissue and 0.32 mg g-1 of liver tissue. It was possible to rule out a work-induced intoxication, and n-nonacosane storage appeared to be accounted for by a lifelong, heavy consumption of unpeeled apples and Brussels sprouts.

Self-expanding endovascular prosthesis: an experimental study.

A new type of endovascular prosthesis was inserted in 28 animals and evaluated for several factors, including thrombogenicity, tendency to migrate, critical implant zones, and incorporation into the vascular wall. The new prosthesis is a woven, multifilament structure of stainless steel alloy; its inherent elastic, self-expanding characteristics hold it against the vessel walls. Forty-seven endoprostheses (3-5 mm in diameter, 15-50 mm long) were percutaneously implanted with either a 6-F introducer sheath, a coaxial 9-F catheter, or a 0.014-inch (0.036-cm) guide wire into the femoropopliteal, coronary, carotid, and renal arteries and iliac veins. Anticoagulant or platelet antiaggregating agents were not used before or after implantation. Angiographic and histologic analyses showed that the prosthesis had a very low thrombogenicity when it was well adapted to the native vessel diameter and that it was incorporated into the vessel wall by a new intima by the 3d week after implantation. No migration occurred, and branch vessel flow was preserved even in those vessels in which ostia were traversed by the prosthesis. This prosthesis has potential for clinical application in the treatment of postangioplasty restenoses, particularly in the coronary arteries.

[Subungual glomus tumors. Apropos of 2 cases].

Glomus tumours are uncommon, though not exceptional, benign growths developed from the constituents of the "neuromyoarterial glomus" initially described by Masson. Two cases of subungual glomus tumours of the hand in middle-aged women are reported. In spite of characteristic symptoms, with paroxysmal episodes of pain triggered off by cold and the most minute traumas, and of the finding in both cases of a violine++ tumour under the nail, the condition was belatedly diagnosed (7 and 27 years respectively after the onset of symptoms). Diagnosis rested not so much on standard radiography, which showed erosion of the distal phalanx in both patients, as on arteriography of the hand which displayed a small vascular pool at the arterial stage. Tumoral excision was successful.

Isolated amyloidosis of the atrioventricular valves. A study of one case, curiously associated with diffuse storage of plant wax paraffin.

Autopsy of a 55-year-old man, whose death was due to coronary failure, revealed: localized amyloidosis limited to the leaflets of the atrioventricular valves and their chordae tendinae, with no apparent relationship to atherosclerosis, and deposits of crystallized vegetable hydrocarbons of dietary origin, mainly in the lungs, lymph nodes and liver. Observations of isolated amyloidosis in healthy valves are exceptional. The association with multi-organ deposition of vegetable wax hydrocarbons raises the question of a possible link between these two entities.

Pulmonary lipogranulomatosis due to excessive consumption of apples.

A 55-year-old man was admitted for treatment of unstable angina. Chest x-ray films showed a micronodular pattern, but there were no respiratory symptoms. Autopsy, carried out following sudden death, revealed pulmonary granulomas surrounding lipid crystals. The same crystals were found elsewhere: lymph nodes, spleen, liver, adrenal glands, and were composed of aliphatic hydrocarbons of vegetal origin, mainly C29H60. Investigation of the case history revealed excessive consumption of apples, to which the hydrocarbon deposition could be attributed, since C29H60 is a natural constituent of the cuticular wax in apple peel. This is a unique example of pulmonary granulomatosis due to storage of vegetal hydrocarbons of dietary origin.

Percutaneous vascular stent: experimental studies and preliminary clinical results in peripheral arterial diseases.

A new pattern of metallic intravascular stent has been experimentally and clinically tested. When the stent is percutaneously placed through a six french introducer, it is held against the vessel wall by its natural elastic properties. In 28 animals, 47 stents of different sizes (from 3 to 5 mm of luminal diameter and from 1.5 to 5 cm of length) have been implanted in the femoro-popliteal (16), the coronary (20), the carotid (3) and the renal arteries (5) and in 3 femoral veins. Angiographic and histological results have shown the stent's low thrombogenicity when it is well fit to the vessel's caliber. Moreover, they have shown its incorporation to the vessel wall by intimalisation as early as the third week after its implantation. The collateral vessels which covered by the stent remain permeable. At the end of this experimentation, 10 implants have been achieved in 8 patients (4 recurrences of iliac stenosis and 6 femoral stenosis). The angiographic results show the perfect tolerance of the organism at 6 months. This good tolerance is revealed by a thin intraluminal border which does not change the artery's diameter. This confirms the experimental results. The permeability is good except for two patients who had an obliteration after one month (one iliac and one femoral obliteration).
