[The surgical treatment of large incisional hernias by lacing of autogenous full thickness skin. 36 cases (author's transl)].

Strips of skin taken from the excess skin adjoining the scar, prepared carefully and used as a loose continuous suture, represent an elastic material which is easy to use and of great strength. Results studied in a group of 36 patients with a follow-up varying from 12 month to 16 years were excellent in 70% of cases and moderate in 25%. The quality of the results obtained, from both a mechanical standpoint as well as in terms of tolerance, together with the simplicity of the technique, have led us to consider it as a method of choice in the repair of large incisional hernias, in particular recurrent.

[Total expansion of the orbit for micro-orbitism].

Micro-orbitism which follows the very early arrested development of the optic vesicles hinders the fitting of a prosthesis. A total enlargement of the orbit is proposed which restores the normal dimensions and enables facial symmetry to be re-established. The risks and advantages of this procedure are discussed.

Anophthalmos

Orbital expansion for anophthalmia and micro-orbitism.

In severe cases of microphthalmos or anophthalmos, the bony orbital cavity is often too small to allow one to create a satisfactory socket for a prosthesis. An omnidirectional expansion of the orbit, by osteotomies done in a step-like fashion, allows the creation of a larger bony cavity and improves the symmetry of the face. The risks of the craniotomy and of the mobilization of the free bone pieces are discussed.

Anophthalmos

[Treatment of malignant melanomas. Trial of D.T.I.C. by intra-arterial infusion].

D.T.I.C. appears more and more to possess the characteristics of an effective chemical agent in the treatment of naevocarcinoma. Having been able to include it in a mixed chemotherapy programme used in a group of 44 patients, we report here our findings. This is a preliminary study, the results of which will have to be confirmed in time on a larger group of patients over a longer period of observation.
