Effect of non-erupted 3rd molars on distal roots and supporting structures of approximal teeth. A radiographic survey of 202 cases.

Root resorption of 2nd molars in proximity to non-erupted 3rd molars has been widely reported. The purpose of this study was to determine the prevalence of root resorption in second molars adjacent to non-erupted third molars. Its association to age and gender of the patient, location and inclination of the non-erupted third molar and to distal bone support of the 2nd molars was analyzed. A radiographic survey of 202 periapical radiographs taken in patients with clinically missing third molars was conducted. 3 examiners independently evaluated the radiographs and only those cases where at least 2 observers agreed were included in this report. Statistical analysis was performed on 186 radiographs. Associations were analyzed with the Pearson chi 2 test. Radiographic evidence of root resorption was found in 45 2nd molars (24.2%) of which 12 (6.5%) showed moderate to complete root resorption. Non-erupted tooth apical position and mesio-inclination of 60 degrees or more relative to the distal root of the second molar were significantly associated with root resorption (p = 0.01368 and p = 0.0194, respectively). Resorption was positively associated with age of patient (p = 0.00606). These results may support early extraction of impacted 3rd molars especially in cases with a mesio-angulation of 60 degrees or more and an apical location in proximity to the distal root of the 2nd molar.

Alveolar ridge preservation following extraction of maxillary anterior teeth. Report on 23 consecutive cases.

The purpose of this article was to clinically evaluate the predictability of a surgical procedure to preserve the alveolar ridge after extracting maxillary anterior teeth with advanced bone loss. This procedure is indicated when a tooth-supported fixed partial denture will rehabilitate the area. Non-resorbable hydroxyapatite crystals are used as graft material and a rotated pediculated split thickness palatal flap covers them. Twenty-three patients who were treated with this procedure were followed for 12 to 24 months. Results were predictable with minimal postoperative ridge deformation. Indications for this procedure are discussed.

Transferring the occlusal and esthetic anatomy of the provisional to the final restoration in full-arch oral rehabilitations.

Provisional restorations are an excellent diagnostic instrument, especially in full-arch oral rehabilitations where no normal occluding contacts remain. Esthetics, phonetics, function, parafunction, and dysfunction after evaluation and acceptance through clinical trial with the provisional restorations should be accurately transferred to the final restorations to ensure the same clinical success. This article presents a sequence of clinical procedures that enables a methodical and controlled transfer of the morphology of the provisional restorations to the final ones in full-arch oral rehabilitations.

Variation of skin surface temperature over the masseter muscles in patients with myofascial pain following occlusal splint treatment.

The skin surface temperature (SST) over the masseter muscles was measured in 19 patients suffering from myofascial pain (MP) and 20 controls who had no history of any craniomandibular disorder. Seven measurements with intervals of 2 weeks were carried out. MP patients received an occlusal stabilization appliance during their second visit as their only treatment. Clinical symptoms, including muscle sensitivity to palpation, jaw movement and general feeling were evaluated at each visit and compared to baseline. The results indicated that SST in the control group remained almost unchanged throughout the study. In the MP group, the mean temperature decreased during the study after initial treatment. Accordingly, there was a probability of 88.5% that the occlusal appliance treatment in the MP group would cause a decrease of SST over the masseter muscle. A significant relationship between clinical improvement and a decrease of SST was found in the MP group. Temperature recordings with certain limitations could be an objective assessory tool in diagnosis and follow-up of patients with myofascial pain.

Transferring provisional restorations to final master casts.

Many principles guide preparation and fitting of provisional restorations. Their importance as a diagnostic tool is considered. Aesthetic, phonetic, functional, parafunctional and dysfunctional factors after evaluation in the provisional restorations have to be transferred to the master cast to incorporate them into the final restoration. A method for transferring morphology of provisional restorations to the master cast through preparation of a wax analogue fixed partial denture is presented. This method is precise, practical, suitable for most rehabilitation cases, and not clinically time-consuming since most stages are carried out in the dental laboratory.

Investigation of the effects of a variable lateral guidance incline on the pantronic registration of mandibular border movement: Part II.

This study was designed to test the hypothesis that changes in lateral guidance inclination could affect the condylar registrations in the recordings of an electronic pantography. In seven healthy subjects, pantronic registrations of immediate side shift, progressive side shift, and orbiting and protrusive condylar paths were recorded with lateral anterior guidance inclinations of 0, 20, 30, 45, and 60 degrees from the same pin height. Analysis of variance with repeated measures showed a significant decrease in the immediate side shift (p 0.016) and the progressive side shift (p 0.051) as the lateral guidance angle increased and no significant effect on the orbiting registrations. These observations, together with those from other studies, suggest that the concept of fabricating the working-side tooth guidance exactly according to instrument condylar settings (which themselves may be subject to a variable recording lateral anterior guidance) appears untenable and demands further study.

A comparative study of three therapeutic modalities in a temporomandibular disorder (TMD) population.

A comparative study of three treatment modalities, pharmacologic, occlusal appliance, and their combined use, was conducted to test their therapeutic efficacy on 61 temporomandibular disorder (TMD) patients. Alprazolam (Xanax) was used for the pharmacologic treatment; the occlusal appliance therapy consisted of a flat maxillary stabilization splint. Of the 61 patients, 19 received Alprazolam, 30 received occlusal appliance therapy, and 12 received combined therapy. Subjects were examined at two-week intervals for two months. Only 42 patients attended all follow-up visits. Eight parameters were studied: severity of pain, periodicity of pain, self-evaluated stress, muscle sensitivity to palpation, joint sensitivity to palpation, joint noises, limitation of opening, and limitation of lateral movement. No significant difference was found between the treatment modalities for most of the parameters. All three proved to be effective. Alprazolam increased the restricted mandibular movement, was least effective on joint sensitivity to palpation, and had no effect on joint noises. The combined treatment approach not only failed to prove superior to the other treatments, but showed less improvement in some parameters, possibly due to the small sample.

A comparative study of the stereognathic ability between patients with myofascial pain dysfunction syndrome and a control group.

This study compared the stereognathic ability of two groups of subjects. One group comprised 35 myofascial pain dysfunction syndrome (MPDS) subjects and the other, 30 control non-MPDS subjects. Thirteen different acrylic resin shapes were used for oral and manual identification. No statistically significant difference was found between the two groups in the accuracy of identification, time for identification and their ratio. The results indicate no difference in stereognathic ability between the MPDS and the control group.

Custom overdenture retainer.

A simple technique for fabricating stud overdenture retainers in the form of custom attachment castings with teflon retention discs has been described. This system allows for selective placement of the retention sphere on the root surface and control of retention through the size of the retention sphere. Using transfer location projections, exact transfer of the castings to the working model is possible. When necessary, the teflon discs may easily be replaced at the chairside.

Functional disturbances of the masticatory system in an elderly population group.

The purpose of this study was to determine the prevalence of craniomandibular disorders in a geriatric population in Israel, and to study morphometric and functional parameters as well. One hundred ten elderly subjects (61-90 years old) were interviewed and examined clinically for the following parameters: general health history, dental comfort and masticatory performance, anthropometric measurements, functional performance of the stomatognathic system, dental status, static and dynamic occlusion, and signs and symptoms of craniomandibular disorders. The results indicate that (1) loss of hearing was the most common debilitating functional disturbance reported, with rheumatoid or rheumatoid-like chronic diseases also common among this age group; (2) chewing performance was compromised and tongue thrust was common; (3) range of mandibular movement was decreased; (4) prevalence of signs and symptoms of craniomandibular disorders in the elderly were compatible with or higher than those in younger subjects, but the older individuals were not disturbed by the problem enough to seek help; (5) there was no association between impaired general health and the prevalence of craniomandibular disorders; and (6) interocclusal distance was large and negatively related to the lower third of the face and positively related to the presence of full dentures.
