[Prevention of bacteremia from central venous catheters in hemodialysis: value of application of a mixture of rifampicin and protamine at the insertion site].

During the last 8.5 years, authors observed only 3 bacteremias out of 249 catheters in place in different sites corresponding to a total of 10,063 days with central venous catheterization in hemodialysis (0.30/1000 catheter days). This good result seems to be correlated mainly with the preventive application, on catheter insertion-site, of a rifampin and protamine sulphate mixture. This protocol appears worthy to be known because, at the present time, whatever the new preventive strategies, infection rates reported in the literature are generally more consequent and it is necessary to consider all the possibilities of decreasing them.

Experimental study of the multipolar vortex instability

The instability of a vortex subjected to a stationary dipolar or tripolar constraint is studied experimentally by using a rotating deformable cylinder on which two or three rollers are applied. As the Reynolds number and the aspect ratio of the cylinder are varied, different modes of instability are observed and their wavelength and frequency are compared to theoretical predictions. Secondary instability and cyclic breakup are also evidenced in the elliptic geometry.

[Mycobacterium xenopi osteoarthritis of the ankle in a patient followed for psoriatic rheumatism].

BACKGROUND: Mycobacterium xenopi is an uncommon cause of osteoarticular sepsis. However, a recent series of M. xenopi spondylodiscitis emphasizes the potential risk after invasive procedures. CASE REPORT: A 33-year-old woman was followed for psoriasis rheumatoid arthritis. She had undergone several exploratory procedures including arthroscopy of the left ankle for invalidating joint disease. In 1999, M. xenopi arthritis and osteomyelitis was diagnosis in this joint. DISCUSSION: Sterilization and maintenance of surgical instruments must abide by rigorously controlled strict protocols. The benefit/risk ratio of invasive procedures in debilitated joints should be more precisely evaluated. Specific and repeated sample cultures should be performed to search for mycobacteria in all bone and joint infections, particularly in case of prior invasive procedures.

[Repeated studies of the prevalence of Staphylococcus aureus in the nasal cavity in hemodialysed patients].

OBJECTIVES: In order to better establish a prevention strategy based on mupirocin, we evaluated nasal carriage of Staphylococcus aureus in haemodialysis patients over a 15 month period. METHODS: Search for Staphylococcus aureus in the nasal cavities was made every 2 months in 92 chronic dialysis patients. These patients were divided into 3 groups according to the nature of the carriage: non-permanent, intermittent or permanent. RESULTS: Among the 80 patients retained for analysis, there were 27.5% with intermittent carriage and 11.25% with permanent carriage. Factors which appeared to protect against carriage were rural residence and home self-dialysis. CONCLUSION: Repeated long-term search for nasal carriage of Staphylococcus aureus has provided reliable data for each patient and gave information on the effects of epidemiological conditions and health care structures.

[Fungal contamination of a dialysis water treatment system].

The authors report fungal contamination of the water delivery system in one of their dialysis centers. They emphasize the insufficiency of the legal recommendations for water analysis, as well as the economical and logistical consequences of this type of contamination. Finally, they suggest disinfection procedures allowing eradication of fungi.

[Detection of Pseudomonas cytochrome oxidase and esterase in blood culture broths].

Search for cytochrome-oxidase was performed directly on blood culture broths without prior isolation of the pathogen on agar, using paper disks impregnated with "oxidase" reagent and the Api Z strip which enables additional detection of esterases. Both these tests are indicative of the presence of Pseudomonas. False-positive results were recorded in only 0.65% of the 167 strains of Enterobacteria studies. The 38 strains of Pseudomonas aeruginosa gave 5.26% false-negative results; 88.6% of Pseudomonas strains were esterase-positive. Simultaneous search for both these enzymes thus allows for identification and antibiotic sensitivity testing before agar isolation.
