[Community-acquired pneumonia (CAP) with hospital treatment. The value of the clinical picture and complementary exams in predicting its etiology].

We studied 162 patients with community-acquired pneumonia admitted for hospital treatment, in order to determine the utility of clinical and ancillary examinations for predicting etiology and guiding the most appropriate empirical treatment. Acute first appearance of symptoms, purulent expectoration, chest sounds indicating lung condensation, pleuritic chest pain and leukocytosis over 12,500/ml were statistically significant in differentiating typical pneumonias from those with atypical behavior patterns. The last two features were the most relevant according to multivariate analysis. We conclude that careful taking of case histories and basic blood testing continue to be relevant and must not be considered anachronistic for the differential diagnosis of community-acquired pneumonias.

Mesothelial versus endothelial cell seeding: evaluation of cell adherence to a fibroblastic matrix using 111In oxine.

OBJECTIVES: The aim of this study was to compare the behaviour of mesothelial cells (MC) to that of endothelial cells (EC) when seeded onto a PTFE, prosthesis coated with a fibroblastic matrix. DESIGN, MATERIAL AND METHODS: Three study groups were examined: a control group (Control) of PTFE prostheses with a fibroblast matrix (n = 8); Group EC, PTFE prostheses seeded with EC on a fibroblastic matrix (n = 8); and Group MC, PTFE, prostheses seeded with MC on a fibroblastic matrix (n = 8). All cell types were labelled with 111In (100 microCi/ml) 24 h after seeding, when the cells had formed a monolayer on the prosthetic surface. Radioactive levels were measured at 2, 4, 6, and 24 h. RESULTS: Both EC and MC showed optimal adherence. The MC had a better radioactive uptake and retention than the EC. The number of EC and MC cells that remained adherent to the matrix was large enough to ensure complete covering of the prosthetic surface. CONCLUSION: The use of MC is therefore feasible as an optimal alternative for achieving a natural covering on vascular prostheses prepared with a fibroblastic matrix.

Inhibition of the intimal hyperplasia in an arterial autograft model by blockade of the N-terminal of the integrin beta3 subunit by monoclonal antibody P37.

Myointima formation or intimal hyperplasia is a major undesirable problem at the anastomotic ends of narrow bore arterial autografts and in other arterial wall injuries, which often leads to late restenosis and thrombosis and whose pathogenesis is still not understood. Platelets are suspected to intervene at some stages of its development, together with endothelial and muscle cells, the extracellular matrix and, most probably, adhesion receptors. To ascertain whether and at what stage beta3 integrins are involved, a rat arterial autograft model was used, together with monoclonal antibody P37, which is directed to the sequence 101-109 of the beta3 subunit of the human platelet fibrinogen receptor (integrin alphaIIbbeta3) and inhibits platelet aggregation in vitro and acute thrombosis in vivo. Three groups of animals were used: group I underwent an arterial autograft of a 5-mm segment of the right common iliac artery; group II received, intravenously, a single dose (0.8 mg/kg) of P37 at 15 min before the graft implantation; and group III was treated as group II but a similar dose of antibody was additionally given on day 14 after the operation. Animals in each group were sacrificed on days 7, 14, 21, 30 and 50 after the operation, and the grafts were removed for light and electron microscopy observation and further time-dependent morphometric analysis. By day 14, group I autografts already showed intimal hyperplasia and secretory smooth muscle cells, while group II and II autografts presented only some degenerative changes in the medial layer, with no signs of hyperplasia. Intimal hyperplasia was observed on day 21 in group II and on day 30 in group III, although less pronounced than in the corresponding controls. However, by day 50, the three groups had the same thickness of myointima. The immunohistochemical determination of metalloproteases suggests no role for these enzymes in the immunoinhibition of myointima formation. We conclude that P37 inhibits the onset of the intimal hyperplasia in the arterial autografts and that this onset in treated animals seems to be related to the decay of the circulating antibody. Further work is required to decide whether a higher or longer presence of circulating P37 can definitively prevent the development of intimal hyperplasia, as well as to ascertain which cells and which beta3 integrin receptors intervene.

Antithrombotic map of the N-terminal domain of the GPIIIa subunit of the human platelet fibrinogen receptor (GPIIb-IIIa) determined in vivo by monoclonal, immunochemical inhibition of acute arterial thrombosis.

The inhibition of the platelet fibrinogen receptor, the glycoprotein IIb-IIIa (GPIIb-IIIa) or integrin alphaIIbbeta3, has recently became an accepted practice in clinical cardiology. The interest lies now in the improvement of the antithrombotic activity and the minimization of the secondary effects of the receptor inhibitors, by their evaluation in vivo in the different dynamic conditions and pathological states under which these inhibitors have to perform. In this paper, we functionally map in vivo the N-terminal domain of the GPIIIa subunit, using the antithrombotic activity of five murine monoclonal antibodies (mabs) (P37, P40, 95-1, P95-2 and P97), all of them inhibitors of platelet aggregation in vitro and directed to this ligand binding domain of the human fibrinogen receptor. Competition experiments have shown that these mabs bind with high affinity (5-7 nM) and compete very strongly among themselves for binding to human resting platelets, except P40, which neither binds nor competes. These antibodies were assayed in a dog model of acute thrombosis in the carotid artery, which were induced 15 min after their intravenous administration (0.8 mg/kg). The antithrombotic activity was quantified by the measurement of the [111In]oxine-labelled platelet deposition at the site of the arterial lesion and was expressed as the percentage of the total circulating platelets. Antibody P37, directed to the GPIIIa 101-109 sequence, decreased the platelet deposition 630-fold with respect to control animals. P95-2, P97 and P95-1 decreased the platelet deposition 160-, 32- and 25-fold, respectively, while P40, directed to the GPIIIa 260-302 sequence, did not show any antithrombotic activity. We conclude that all the mabs directed to the N-terminal domain of GPIIIa, which inhibit platelet aggregation in vitro and whose epitopes are very close to each other and exposed in resting platelets, have high antithrombotic activity in vivo, which varies depending on the actual location of the epitopes in the receptor topography. Among these antibodies, P37, the strongest receptor inhibitor in vivo and whose epitope is most probably the closest to the fibrinogen binding site(s), seems the best candidate for comparative studies in animal models with today's best GPIIb-IIIa inhibitors and for clinical trials in humans in order to arrest or prevent thrombosis, reocclusion and late restenosis.

Adenosine deaminase activity in the aetiological diagnosis of community-acquired pneumonia.

A prospective study was undertaken to assess the usefulness of serum adenosine deaminase (ADA) activity in the aetiological diagnosis of 75 patients (mean age 58 years) with community-acquired pneumonia who required hospitalization. Measurements of ADA were also carried out in 35 healthy subjects (mean age 52 years). The serum ADA activity in patients with typical bacterial pneumonia (TBP) was 21 +/- 7 IU/l and in controls 22 +/- 9 IU/l. In 43 patients with atypical pneumonia (AP), ADA levels (43 +/- 23 IU/l) were significantly higher than in the previously related groups (p < 0.001). Analysis within the group of atypical pneumonia showed significant differences for infections caused by Coxiella burnetii (61 +/- 19 IU/l, p < 0.001), Mycoplasma pneumoniae (44 +/- 26 IU/l, p < 0.001) and Legionella pneumophila (39 +/- 15 IU/l, p < 0.05), as compared with patients with bacterial pneumonia and normal control subjects. We conclude that serum ADA in patients with community-acquired pneumonia requiring hospitalization may provide useful additional diagnostic information on the aetiology of pulmonary infection.

Structural and ultrastructural study of the myocardium after 24-hour preservation in University of Wisconsin solution.

This study deals with myocardial preservation after 24 hours of continuous perfusion with University of Wisconsin (UW) solution, focusing on the morphological changes produced by preservation and reperfusion, and their possible relationship to the composition of the solution and the immediate hemodynamic findings after orthotopic heart transplantation in dogs. Following preservation, the histological images of the myocardium were normal in almost every case. After revascularization, although 50% of the hearts recovered preoperative functional levels, there were relevant histological changes in all of them. These changes consisted of interstitial edema, damage to at least 50% of the mitochondria, presence of contraction bands, loss of intercellular junction structure and vasoconstriction. According to these results, the association of continuous perfusion and UW solution appears to be effective during the ischemic period of prolonged myocardial preservation, but not during reperfusion. Perhaps the proportion of viable hearts could be increased by preventing vasoconstriction.

Influence of stress on calcification of delipidated bovine pericardial tissue employed in construction of cardiac valves.

Since the development of cardiac bioprostheses, numerous chemical treatments have been assayed to prevent mineralization. The effectiveness of chemical treatments that eliminate lipids from the tissue was tested by combining two models. First, handmade bovine pericardial bioprostheses, subjected to chemical treatment with chloroform/ methanol and glutaraldehyde or treated with glutaraldehyde alone for use as controls, were subjected to mechanical stress in a heart valve, accelerated wear tester (100 x 10(6) consecutive cycles). Then, the bioprostheses were unstitched and tissue samples were taken from the portion subjected to maximal stress (P1) and from that surrounding the sewing ring, which had not been subjected to mechanical stress (P2), for subcutaneous implantation. After 21 and 60 days of implantation, we observed calcification of the samples subjected to mechanical stress, even after delipidating treatment, with no significant differences with respect to the control group. However, the treated samples from the portion not subjected to mechanical stress presented a slighter accumulation of calcium after 60-day implantation (5.60 +/- 3.09 mg Ca2 +/g dry weight of tissue) versus the control group (47.17 +/- 20.4 mg Ca2+/g dry weight of tissue), the difference of which was statistically significant (p < 0.01). At the time of these medium-term studies, marked calcification was observed in tissue subjected to delipidating treatment in the zones that underwent mechanical stress.

Calcification of pericardial tissue pretreated with different amino acids.

Since the development of cardiac prostheses, numerous chemical treatments have been assayed to prevent the process of their mineralization. The effect of chemical treatment with amino acids is assessed in a subcutaneous implantation model in rats. Pericardial tissue from young calves was treated with L-lysine, L-glutamine, L-arginine or L-glutamic acid, each at a concentration of 0.5 M, following treatment with 0.625% glutaraldehyde. Then, the tissue was implanted into young rats for periods of 21 and 60 d, after which the calcium accumulated was quantified by atomic absorption spectroscopy. Values similar to or higher than those found in control samples indicated a lack of effectiveness of these treatments. Only in the 21-d implantation samples treated with L-lysine and L-arginine was less calcium accumulated than in the control tissue. After 60 d of implantation, all groups showed high levels of calcium deposition. The values obtained after 60 d of subcutaneous implantation were 87.5 +/- 52.4 mg Ca2+ per g dry weight of tissue for L-lysine, 108.7 +/- 43.5 mg Ca2+ per g dry weight of tissue for L-glutamine, 130.4 +/- 22.4 mg Ca2+ per g dry weight of tissue for L-glutamic acid, 119.3 +/- 27.6 mg Ca2+ per g dry weight of tissue for L-arginine and 100.0 +/- 38.3 mg Ca2+ per g dry weight of tissue for the control group. Treatment with amino acids does not appear to prevent the calcification of cardiac bioprostheses or of collagen-based biomaterials when assayed in a model of subcutaneous implantation.

Airways reactivity, atopy and bronchoalveolar lavage in male smokers with airflow obstruction.

Although cigarette smoking plays a major role in the development of chronic obstructive pulmonary disease, risk factors that might predispose susceptible smokers to develop manifest airflow obstruction are poorly defined. The aim of this study was to investigate whether BAL cell profiles, bronchial hyperresponsiveness, and atopy were different in smokers with and without airway obstruction. Fifty-seven current smokers with (n = 22) and without airflow obstruction (n = 35), defined spirometrically, and 8 nonsmoking subjects (controls) were studied (fiberoptic bronchoscopy and bronchoalveolar lavage (BAL), measurement of IgE concentrations and skin prick testing with common allergen extracts, methacholine bronchoprovocation testing). Mean values of forced expiratory volume in 1 s (FEV1) percent predicted were significantly lower in smokers with obstruction than in those without obstruction (53.1 +/- 16.9 vs. 72.5 +/- 16.6, p < 0.001). Smokers with airflow obstruction showed an improvement in FEV1 < 20% after inhalation of two puffs of 0.250 mg terbutaline. Current smokers as opposed to controls had significantly (p < 0.01) higher values of cells/ml in BAL (78 +/- 72.4 x 10(4) vs. 28.7 +/- 14.5 x 10(4)) and macrophages (90.5 +/- 19.4 vs. 84.3 +/- 9.3%) and lower values of lymphocytes (5.4 +/- 15.4 vs. 13.1 +/- 10.7%). Smokers with obstruction to airflow showed significantly (p < 0.05) higher values of cells/ml in BAL (102 +/- 88 x 10(4)) than smokers without airflow obstruction (63.4 +/- 57.5 x 10(4)) with no differences in differential cell counts. The provocation concentration of methacholine producing a 20% reduction in FEV1 (PC20) was also lower in smokers with obstruction than in smokers without obstruction (2.9 +/- 3.5 vs. 6.4 +/- 7.2 mg/ml, p < 0.05). Differences in atopy were not found. In summary, current smokers with airflow obstruction tended to have more cells recovered by BAL and greater airway responsiveness than smokers without obstruction, although atopy-related parameters were not different for both groups.

Electrophysiologic effects of procainamide, mexiletine, and amiodarone on the transplanted heart. Experimental study.

The effects of procainamide, mexiletine, and amiodarone on automaticity, conduction, and refractoriness were studied in a model of heterotopic heart transplantation in dogs that combined an innervated heart (recipient) and a denervated transplanted heart (donor). After the surgical procedure, 500 mg procainamide (n = 13), 200 mg plus 0.1 mg/kg per minute mexiletine (n = 10), or 150 mg amiodarone (n = 10) was administered intravenously. During a baseline period and after drug administration, each heart was assessed for atrioventricular interval; cycle length; sinoatrial conduction time; atrioventricular node anterograde and retrograde block points; atrioventricular node and ventricular antegrade effective refractory periods; PR, QRS, and QT intervals on electrocardiogram; systemic arterial, pulmonary arterial, central venous, and pulmonary capillary wedge pressures; and cardiac output. In recipients, procainamide reduced cardiac output, depressed sinus automaticity, slowed conduction time without affecting the QRS interval, and prolonged the nodal and ventricular refractoriness; in donor hearts, it depressed automaticity and prolonged nodal refractoriness, but did not modify conduction or ventricular refractoriness. Mexiletine only moderately depressed sinus automaticity in recipient hearts; it did not affect the other parameters either in recipient or transplanted hearts, nor did it alter the hemodynamic situation. Amiodarone produced hypotension, reduced cardiac output, and prolonged all the electrophysiologic intervals except the QRS interval in recipient hearts. These changes were even more pronounced in the transplanted hearts and led to extreme sinus bradycardia in four cases. Of these three drugs, mexiletine appears to be the safest should treatment for arrhythmias be necessary in transplant recipients.

Inhibition of the calcification of porcine valve tissue by selective lipid removal.

Since the development of cardiac prostheses, numerous chemical treatments have been assayed to prevent the process of their mineralization, causing 60% of the failures. The effect of the extraction of lipids from the tissue employed in porcine valves is assessed in a model of subcutaneous implantation in rats. Tissue from aortic and pulmonary porcine valves was treated with chloroform-methanol and 0.625% glutaraldehyde and was implanted into young rats for periods of 21 and 60 d. The calcium accumulated was then quantified by atomic absorption. The effectiveness of this treatment is demonstrated by the detection of much lower calcium values than in the control group. For aortic valve tissue, the values obtained were 40.5 and 188.1 micrograms Ca2+/mg dry weight of tissue for implantation times of 21 and 60 d, respectively, versus 5.48 and 1.4 micrograms Ca2+/mg dry weight of tissue for the same tissue treated with chloroform-methanol. The values obtained with pulmonary valve tissue were very similar: 72.46 and 108.06 micrograms Ca2+/mg dry weight tissue versus 0.67 and 0.80 micrograms Ca2+/mg dry weight tissue for implantation periods of 21 and 60 d, respectively. Thus, phospholipids may be totally or partially responsible for the calcification of the porcine valve tissue employed in the construction of cardiac bioprostheses.

Prevention of arterial thrombosis by a monoclonal antibody against the 100 to 109 amino acid sequence stretch of the beta-subunit of the human platelet fibrinogen receptor: a comparative study with low dose aspirin.

OBJECTIVES: The aim of this study was to compare, in dogs, the antithrombotic activity of aspirin and the murine monoclonal antibody P37, which inhibits platelet aggregation and fibrinogen binding to activated platelets. BACKGROUND: The antithrombotic activity of P37 has been somewhat predictable, given its in vitro platelet antiaggregating activity and localization at or very near the fibrinogen binding site in the platelet fibrinogen receptor, the glycoprotein IIb/IIIa or integrin alpha IIb-beta 3. METHODS: The monoclonal antibody P37 of the immunogamma-globulin-1 isotype was prepared according to previously described immunization and fusion protocols and screening assays. To compare its antiaggregating capacity with that of aspirin, experimental thrombosis was induced in all dogs by means of direct current applied to the carotid artery. Autologous platelets had previously been labeled with indium-111 oxine. The dogs were assigned to three groups: group I (n = 18) was the control group; group II (n = 12) was treated orally with 5 mg of aspirin/kg body weight per day for 7 days before induction of thrombosis, and group III (n = 10) was treated intravenously with a single dose of P37 (0.8 mg/kg). RESULTS: The indium-111 oxine activity deposited in the thrombi was 12.94 +/- 12.83% (mean +/- SD) in group I, 3.55 +/- 2.99% in group II and 0.03 +/- 0.03% in group III. The differences between groups were always statistically significant (p < 0.05). CONCLUSIONS: We conclude that a single dose (0.8 mg/kg) of P37 in a canine model of arterial thrombosis is approximately 100 times more efficient than the administration of aspirin (5 mg/kg per day) in preventing platelet deposition during thrombus formation.

Nonembolic high-probability perfusion lung scan for pulmonary thromboembolism.

Pulmonary thromboembolism (PTE) is a disease whose diagnosis in the emergency department constitutes a challenge to the clinician. Perfusion lung scans have been important in helping in this context, mainly because of the fact that a normal perfusion lung scan rules out the presence of a clinically significant PTE. Nevertheless, false-positive results are sometimes observed, causing a diagnostic dilemma. An example is a case of bronchogenic carcinoma involving a pulmonary artery or its branches. In such patients, some characteristics of the scintigraphic imaging could guide the diagnosis toward a nonembolic origin of the perfusion defect. A case of small-cell carcinoma of the lung involving the left pulmonary artery whose clinical presentation suggested considering the possibility of PTE is reported. The so-called "stripe sign" is reviewed, and certain clues for the differential diagnosis of a massive and one-sided perfusion defect are discussed.

Electrophysiological effects of atracurium and vecuronium on normal and denervated hearts.

Atracurium and vecuronium offer some advantages with respect to other nondepolarizing muscle relaxants. However, their administration has been associated with episodes of bradycardia and asystole, the mechanism of which has not yet been clarified. In an effort to analyze the effects of these drugs on automaticity, refractoriness, and conduction, 30 dogs underwent heterotopic heart transplantation to establish a model in which a normal heart (recipient) and a denervated heart (donor) could coexist. The electrophysiological studies included the determination of cycle length (CL), sinoatrial conduction time (SACT), antegrade and retrograde atrio-ventricular (A-V) node and ventricular block points (ABP and RBP), as well as effective antegrade and retrograde A-V node (ARPAVN and RRPAVN) and ventricular refractory periods (VRPs) in both hearts. These parameters were measured in a basal state and after injection of pancuronium (0.5 mg/kg) in 10 animals, atracurium (0.5 mg/kg) in another 10 dogs, and vecuronium (0.12 mg/kg) in the remaining animals. The results showed that pancuronium produced a tachycardia, significantly facilitated A-V conduction in the donor and the recipient hearts, and lowered the A-V node refractoriness in the donor organ, while atracurium and vecuronium induced no changes from the baseline values, either in normal or denervated hearts. Based on the results of this study, the episodes of sustained bradycardia reported in association with atracurium and vecuronium are attributed to the absence of antagonism of vagal stimuli provoked by pancuronium, and/or by surgical or anesthetic manipulations.

Effect of fentanyl on cardiac automaticity and conduction: direct or mediated action?

The effects of fentanyl at a dose of 100 micrograms.kg-1 on cardiac automaticity, refractoriness and conduction have been studied in a model of heterotopic heart transplantation which combines a denervated heart (donor) and a non-denervated heart (recipient) in each of 20 dogs employed. Once the surgical procedure had been completed, cycle length, sinoatrial conduction time, antegrade and retrograde A-V node block points, cycle length at which 1:1 conduction ceases to exist and the antegrade effective refractory period of the A-V node and ventricles were measured. These parameters were determined in hearts both in the basal situation and 10 min after fentanyl injection. In the recipient organs, fentanyl produced statistically significant prolongation of cycle length, sinoatrial conduction time, antegrade block point and antegrade effective refractory periods of the A-V node and ventricles, with respect to the basal situation (P less than 0.01 for all these parameters); in 20% of cases, a complete A-V block was produced. In the donor hearts (denervated), prolongations of cycle length (P less than 0.01) and the antegrade block point (P less than 0.05), as compared with the basal situation, were recorded, but no other modifications were detected. To test whether these effects were due to the action of fentanyl, 10 animals were subsequently injected with naloxone, an agent which reverted the reported changes, although in the recipient organs, there persisted a slight prolongation of the cycle length with respect to basal measurements (P less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

Late sudden cardiac death in the follow-up of patients having a heart valve prosthesis.

Eight hundred thirty-one patients with Björk-Shiley prostheses (341 aortic, 345 mitral, and 145 double) had a mean follow-up time of 68.2 months per patient. Of these, 24 (16% of all deaths) died suddenly: six of 42 with aortic valve replacement (14.3%), 12 of 56 with mitral valve replacement (21.4%), and six of 36 with double valve replacement (16.6%). This correlated with evidence of premature ventricular complexes detected in multiple routine 12-lead follow-up electrocardiograms (p less than 0.001 for simple aortic or mitral valve replacements and p less than 0.01 for combined aortic and mitral valve replacements). Premature ventricular complexes were significantly more frequent among those who died suddenly than among survivors and those who died of other causes (p less than 0.001 in both cases); there were no significant differences between the latter two groups. The actuarial risk of sudden death was significantly greater among those patients who had premature ventricular complexes than among those who did not have this disorder (p = 0.0002). We conclude that the presence of premature ventricular complexes, as an independent variable, was correlated with the risk of sudden death.

The Björk-Shiley valve prosthesis. Analysis of long-term evolution.

The long-term follow-up of 831 patients who underwent valve replacement with Björk-Shiley Delrin and standard Pyrolyte prostheses (341 aortic, 345 mitral, and 145 mitroaortic) between 1971 and 1980 is reviewed. The follow-up concluded in 1985. Cumulative follow-up amounted to 4724 years, with a mean of 5.68 years per patient. Data on survival were obtained in 754 patients (complete follow-up in 90.8% of cases). Perivalvular leak was the most frequent complication in aortic valve replacement, whether isolated or combined, with values significantly higher than those registered in isolated mitral replacement (p < 0.001 in both cases). No correlation was found between this complication and valve calcification, but it was statistically correlated with the size 19 model (p < 0.05). Prosthetic stenosis was more common in mitral than in aortic replacement (p < 0.001), and of the former, size 23 was that most often affected (p < 0.001). The earliest case of mitral pannus was diagnosed 20 months postsurgery, and from 45 months on this pathology was the cause of every case of stenosis. The risk of thromboembolism was similar in aortic, mitral, and double prostheses, while it was the single most frequent complication in isolated mitral replacement. Prosthetic thrombosis was triggered in all cases in which it occurred by discontinuance of anticoagulant therapy. Anticoagulant-induced hemorrhages were more frequent in double replacement than in mitral (p < 0.05) and aortic valve replacement. Endocarditis was the complication that produced the highest mortality rate in all groups; the frequency of this infection was higher among patients with double prostheses when compared with either aortic replacement (p < 0.05) or mitral replacement (p < 0.001). The risk of suffering endocarditis was correlated with the existence of active preoperative infection in patients with mitral prostheses and double prostheses (p < 0.001 in both cases). Overall morbidity was higher in the double replacement group with respect to the mitral group (p < 0.01). The rate of mortality was also higher among the double valve replacement patients when compared with both the aortic (p = 0.0002) and mitral (p = 0.006) groups.

Electrophysiology of heterotopic heart transplant: experimental study in dogs.

Electrophysiological properties were studied in a heterotopic heart transplant model developed in 44 dogs (Group I). Cycle length was 359.5 +/- 55.2 ms in the recipient heart and 500.9 +/- 77.9 ms in the donor heart (P less than 0.001). Sinoatrial conduction time was 38.6 +/- 13.6 ms in the recipient heart and 38.6 +/- 10.4 ms in that of the donor (not significant). The Wenckebach point was 175.4 +/- 31.1 ms in the recipient organ and 214.3 +/- 42.6 ms in the donor heart (P less than 0.001). The retrograde block point was 271.6 +/- 48.0 ms in the recipient heart and 353.6 +/- 47.3 ms in that of the donor (P less than 0.01). The effective antegrade refractory period was 133.4 +/- 28.7 ms in the recipient heart and 167.3 +/- 46.3 ms in the donor organ (P less than 0.001). An epicardial atrial cartography was performed at 44 preset points in both hearts, revealing a normal activation sequence and delays of 36-98 ms (mean 58 ms) in the recipient heart and from 39 to 59 ms (mean 50 ms) in the donor heart (not significant). In another 11 dogs (Group II), the same electrophysiological study was carried out under basal conditions and after pharmacological blockade of the autonomic nervous system with atropine (0.04 mg (kg body weight)-1) and propranolol (0.2 mg (kg body weight)-1). No significant differences were found in any of the parameters studied upon comparison of Group II animals in basal conditions with Group I recipients, and Group II dogs after blockade with Group I donors. These results demonstrate that the differences in automatism, conduction and refractoriness between the donor and recipient hearts are not attributable to differences in the haemodynamic situation or in the anaesthetic technique, but to denervation.
