Adriamycin/cis-platinum/cyclophosphamide combination chemotherapy for advanced carcinoma of the parotid gland.
Five patients were treated with a combination of Adriamycin/cis-platinum/cyclophosphamide for far-advanced and/or recurrent cancer of the salivary gland. Adriamycin, 40 mg/m2, and cis-platinum, 50 mg/m2, were given on day 1; cyclophosphamide, 200 mg/m2 daily for four days, was given by mouth on the third to sixty day of each monthly therapy course. Two patients achieved a complete remission, each lasting five months, and the three others had partial remissions from one to seven months (median six months) in duration. Therapy was well tolerated. Severe nausea and vomiting occurred on the first day of therapy but was self-limited. There was only mild to moderate leukopenia, no significant thrombocytopenia or elevations of serum creatinine, and no evidence of dose-limiting peripheral neuropathy. Adriamycin/cis-platinum/cyclophosphamide chemotherapy appears to be effective in the treatment of advanced parotid gland cancers.
Sleep-related breathing disorders.
Disorders of breathing related to sleep are relatively newly recognized and less than fully understood. This review presents the terminology used to describe them, and describes the physiology of sleep and the control of ventilation, the pathophysiology of breathing disorders during sleep, their various clinical manifestations, current diagnostic techniques, and the treatment modalities available at present. Among the diagnostic approaches discussed are airway fluoroscopy during sleep, pneumography, and polysomnography. Approaches to medical and surgical management of these disorders are reviewed. Speculation regarding the underestimation of the prevalence of these disorders, the male predominance, and their relationship to snoring, coronary artery disease, and hypertension, which also show male predominance, are presented. Also suggested is a relationship of sleep apnea, obesity, and mental retardation in childhood-onset or congenital disorders such as Down's syndrome and Prader-Willi syndrome, and in other endocrine dysfunction diseases.
The effects of cricoid cartilage injury and antibiotics in cricothyroidotomy.
Until recent years cricothyroidotomy has been condemned as an elective surgical procedure because of reports of a high incidence of subglottic stenosis. In this study of cricothyroidotomy the effects of trauma, denudation of the mucous membrane of the anterior half of the cricoid cartilage, and the prophylactic administration of antibiotics (ampicillin) on the development of subglotttic airway narrowing were evaluated in 40 mongrel dogs. Two of the dogs developed minimal anterior narrowing of the airway. None of the dogs developed clinically important stenosis.

Digital subtraction angiography: applications in otolaryngology--head and neck surgery.
Digital electronic technology has made its most notable contribution to radiology in the form of the CT scanners which have so greatly benefited medical practice. Experience with intravenous angiography using a digital fluoroscopy unit developed at the University of Arizona gives promise of a further major benefit in the area of angiography. The majority of examinations are conducted on an outpatient basis with a risk level comparable to that of an intravenous pyelogram. The authors review their experience with this unit, including it's diagnostic and therapeutic applications to congenital and acquired vascular diseases, neoplasms, and vascular trauma of the head and neck.

A case presentation is discussed together with the embryology, differential diagnosis, and surgical approach to the problem.
Sleep apnea syndrome associated with a neck mass.
Sleep apnea is an entity that is becoming more commonly diagnosed as the signs and symptoms are becoming better understood. In this report, the first known case of sleep apnea secondary to a lipoma of the submandibular area of the neck is presented. A brief discussion of the types of sleep apnea, along with the most common signs and symptoms of the entity, are reported. The diagnostic workup, operative findings, and postoperative results in this patient are discussed.

Complications of carotid artery replacement in head and neck neoplasms.
Carotid artery involvement by neoplastic disease has grave prognostic implications. However, with the advent of postoperative radiation therapy and chemotherapy, carotid artery sacrifice and replacement is more widely performed. In this report the authors discuss their experience with preoperative and intraoperative evaluation of cerebral blood flow and complications of carotid artery replacement.
