[Description of tinnitus (author's transl)].

Tinnitus is known as a distressing times sequelae of different ear diseases since the ancient. The objective tinnitus by definition is heard by the patient and the investigator. The subjective tinnitus is hearable for the patient only. The character of tinnitus sometimes leads our attention to that place of the passway of hearing where tinnitus is originating. The goal of therapy is to make the tinnitus tolerable at least - if there is no real cure: this means to convert the noncompensated tinnitus to a compensated one.

Electric Stimulation Therapy↗

[Examination in three hearing aids (postaural types) concerning the influence of voltage on the output sound level and on the automatic grain control (author's transl)].

In three modern hearing aids the effect of voltage on the initial sound pressure and the conduct of regulation are examined. The following apparatus were measured: Bosch 66 F AGCI related to the regulation at power entrance, Siemens 274 PP AGCI related to the regulation at power entrance, Microtechnic A 18, related to the regulation at power exit. The examinations show that the hearing aids tested have to operate with that voltage recommended by the producers. The nickel-cadmium accumulators are not efficient. Writing a prescription for an adequate power supply is as important as the prescription of the hearing aid itself.

[Noise exposure of the ear during surgery (author's transl)].

We measured the noise generated by three vacuum pipes with different inner diameters in the external ear canal with a probe microphone. The highest level was generated by the largest pipe when solid material blocked part of the entrance. Then we measured peak levels up to 140 dB-C. Such levels generate only low Leq but due to their high on-off ratios they may be dangerous to the ear if repeatedly applied.

[Some problems concerning electric tests in cases with Bell's palsy (author's transl)].

Some draw backs of Neuronography are discussed: 1) Estimating and correlating response levels is very unreliable. 2) It takes four days for some prognosis. A very fine indicator is the area under the response diagramm, that is the area between curve and Zeroline. To calculate this area we have developed a microprocessor based instrument at the ENT-Clinic Würzburg.

[Occupational hearing loss (author's transl)].

Occupational hearing loss is most likely due to chronic noise exposure. In few cases the "Acoustic accident" is to be considered. Criteria are noise exposure levels in excess of 90 dB, uncomfortable position of head and neck, and hearing loss immediately following the accident. Pure tone audiometry shows a flat hearing loss with prevalence at 1000 to 2000 c/s. Disturbances of the blood flow in the cochlea during noise exposure is discussed as the pathogenic factor.

[Brainstem auditory evoked response (BER) in paedaudiological diagnosis (author's transl)].

Intention of paedaudiology is to detect and to evaluate hearing loss in children. In new born and small infants, in children with behavioral disorders and multiple disabilities, the clinical techniques of audiometry are often not useful. The brainstem audiometry is a reliable alternative and not so cost intensive and time consuming as it is often assumed. Encouraging is this technique in regard of hearing aids: prevention of attaching unnecessary hearing aids, ruling out not appropriate fitted hearing aids and proper habilitation of unilateral hearing loss.

[Use of the temporary osteoplastic epitympanotomy for allogenetic compound middle ear grafts (author's transl)].

The topography and the size of the single middle ear structures and the possibilities of damage are shown, as they face the otosurgeon using the temporary osteoplastic epitympanotomy in connection with middle ear implants. The landmarks and technical aids are named, which make this operation a save and quick approach for the removal of the middle ear pathologies and in the use of allogenetic material.

[The corneal reflex stimulated pneumatically (author's transl)].

We describe an apparatus measuring the cornealreflex stimulated pneumatically. The flow of preassurised air in the range of 0.1-1.5 bar is controlled by an electromagnetic switch. The time the valve is open, is variable. The air flow is directed to the cornea of the eye. The blink reflex is recorded by means of the surface electromyogramm and displayed on the screen of a storage scope. The device allows a clean, reproducable and simple measurement of the cornealreflex.

Automatic recording and evaluation of neuronography.

The introduction in the 60' s of the Nerve Excitability Test (NET) by Laumans and Jongkees for the diagnosis of Bell's palsy stimulated an interest of quantitative testing of the facial nerve. In 1970 Yanagihara reported the Evoced Myography (EMG; i.e. NET is combined with EMG). Starting with electrophysiological basis of the impulse spread of peripheral nerves May, in 1971, extended this concept by including supraliminal stimuli, i.e. Maximal Stimulation Test (MST), for diagnostic purposes. This method eventually developed into what is known as neuronography (Esslen, Huffmann, Ehrenberger, Satoh). It is undoubtedly a refinement in the question of prognosis of Bell's palsy. Continuing the original concept of May we have developed an electronic device with which to carry out automatically the neuronography that demonstrates an evaluation on a numeric display. We use the conventional bipolar negative square wave for a 1 ms stimulation. We stimulate the angle of the mandible, where the ramus marginalis can be readily found in all patients. Neuronography and Evoked Myography provide earlier a more precise prognosis of facial palsy than Nerve Excitability Test (NET). We developed an electronic device for the Maximal Stimulation Test (MST) of May, M., that is automatic. The device counts the spikes of the evoked myogram. A time window is related to the stimulus to exclude stimulus noise and background muscle noise from the counter. Only spikes seen in this window are processed. The number of spikes generated per stimulus is the measurement of the live axons in a stimulated nerve.
