Urinary unconjugated cortisol in normally menstruating women and during estrogen-gestagen treatment.

Changes in 24-hour excretion of unconjugated urinary cortisol and creatinine clearance were determined in 9 normally menstruating women and in 9 women undergoing estrogen-gestagen treatment on 3 consecutive days. A correlation between the cortisol excretion and the creatinine clearance was found only in the luteal phase of the menstrual cycle. The cortisol excretion increased neither during the menstrual cycle nor during the estrogen-gestagen treatment. A marked intra-individual variation in the excretion of unconjugated urinary cortisol was demonstrated in both groups.

A Gemeprost vaginal suppository for cervical priming prior to termination of first trimester pregnancy.

A randomized, double-blind, placebo-controlled, multicentre study was conducted to assess the efficacy and tolerance of a single 1.0 mg Gemeprost (16, 16-dimethyl-trans-delta 2PGE1 methyl ester) vaginal suppository for dilatation of the cervix uteri prior to vacuum aspiration termination of first trimester pregnancy in nulliparous women. The suppository was inserted three hours before vacuum aspiration. In the six centres involved, the preoperative cervical diameter was significantly greater in women treated with Gemeprost. Further mechanical dilatation was either unnecessary or significantly easier than in placebo-treated women. A significant reduction in operative blood loss was noted in the Gemeprost group.

Urinary free cortisol during pregnancy.

Adrenocortical function was measured during early and late normal pregnancies by determining urinary free cortisol on three consecutive days. A marked intra-individual variation in urinary free cortisol excretion was found. This pattern did not differ in early and late pregnancy. No correlation was demonstrated between the excretion of free cortisol, estrogen levels and creatinine clearances.

Ammonia tolerance test used to differentiate between ascites of cirrhotic and malignant genesis.

Eighteen patients with ascites, 9 with cirrhosis of the liver and 9 with malignant disease, were subjected to the ammonia tolerance test by instillation of ammonia transrectally . Arterial blood samples were taken before and 15 and 30 min after the instillation to determine the plasma ammonia concentrations. The results show a significant difference in the plasma ammonia concentration between the two groups of patients after instillation of ammonia. Since the method is non-invasive, it is recommended as a screening test in the differentiation between ascites of cirrhotic and malignant genesis.
