[Epicondylitis humeri lateralis, operative technique and results (author's transl)].

Epicondylitis humeri lateralis is caused by excessive strain on the extensors of the forearm. Chronic inflammation of the epicondylus humeri lateralis and irritation of sensitive fibres of the radial nerve result. The pain can be eliminated by Hohmann's operation on the epicondylus humeri lateralis, with complete separation of the forearm extensor roots. Additional denervation of the epicondylus humeri lateralis (A. Wilhelm's method) eliminates the pain sensations due to the disease by interrupting the sensitive nerve fibres of the N. radialis. These two operations are complementary and a combination of both increases the success rate to 90%.

[Replacement of the distal radius joint surface with silicone rubber in animal experiments on monkeys].

According to appearance in high percentage the recent techniques of prosthetic implants are not sufficient in cases of destruction of wrist or arthrosis. Therefore in most cases even intensive physical training cannot give adequate function by full pain relief. With the development of a silicon-caoutchout-prosthesis to replace only cartilage surface without fixation with cement we get a new way, to implant without extensive-resection of bone. The prosthetic material ist very similar to cartilage in hardness and elasticity. Examinations were made on 8 baboons with the result that there was no restriction in function even at full load 6 weeks postoperatonem. Radiological examinations in an observation period of 9 months did not show any significant change as to destruction in the surrounding of prosthetic stem or the articulating surfaces of cartilage and prosthesis.

[The supinator syndrome. Diagnosis - therapy - results].

The compression of the r. profundus of n. radialis by m. superior is a rare nerve compression syndrome of the upper extremity. In many cases its diagnosis is very difficult. 14 patients with a emg-manifest supinator syndrome were examined after operation. The examinations were performed as to etiology, symptoms and postoperative status depending on pre-operative duration of trouble.

[Solitary cyst of the distal radius].

One case of an aneurysmal bone cyst in the distal end of the radius of a 10 year old boy is described. These bone cysts probably develop from local hemodynamic disturbance and should be treated by evacuation and cancellous bone packing. Their propensity for recurrence is confirmed in the literature.

[2-stage flexor tendon transplantation: follow-up study results].

It is shown in 246 flexor tendon grafts in the so-called no man's land that autologous grafting in a single surgical procedure preserving the tendon sheath yields the best results, whereas good results can only be achieved by two operations after more extensive injuries of the sheath with or without accompanying injuries. For estimation of the results we developed our own scheme, because it seemed difficult to make reliable judgments when the rather divergent published criteria were applied.

[Fatigue fractures of the femoral neck (author's transl)].

By means of one case problems of diagnosis and therapy of fatigue fractures of the fermoral neck are demonstrated and discussed by comparison to recent reports in literature. In case of uncertain diagnosis we recommend surgical revision of the fractured zone with take off of bone tissue for histological examination, especially in order to exclude a malignant bone process.

[Thermometric investigations about the efficacy of beta-escin to reduce postoperative edema (author's transl)].

Beta-escin is highly effective in reducing postoperative edemas and swellings. It was tried to demonstrate the effect of beta-escin using the thermometry. The temperature of soft tissues and bones can be measured with the help of the thermometry, in this way the circulation of the skin can be determined. In 65 test persons so-called normal thermograms were established, using the infraredthermometer KT 41, which makes registrations without touching the skin. In 53 patients the temperature of the skin was checked after surgery of the hand. 27 patients received beta-escin (Reparil); 26 patients without medication served as controls. In every case the temperature of the operated hand was compared to the other hand. Among the patients, who received escin, the temperature of the operated hand rose for two days following surgery; afterwards it fell quickly. Among the controls the skin-temperature rose up to the 4. day after surgery and decreased slowly. These results demonstrate clearly that after using beta-escin there is a quick reduction of postoperative inflammation and edema.

[Vascular causes of median and ulnar nerve compression syndromes].

From a group of more than 200 patients with median and ulnar nerve compression syndrome three cases of interest are demonstrated in detail. In these cases vascular complications led to acurate events accompanied by grave symptoms. In order to prevent irreversible damage surgical treatment of such cases is indicated as soon as possible.

[Compression of the ulnar nerve at the wrist (author's transl)].

Compressions of the ulnar nerve at the wrist in or beyond the canal de Guyon are comparative rare. Those originating from compression in the sulcus ulnaris at the elbow are much more common. The clinical symptoms are typical: Weakness of the small muscles of the hand, loss of sensibility and pain. The diagnosis can be made on the clinical picture. It has to be confirmed by electromyography. Surgery should be performed as early as possible to avoid permanent damage to the nerve. Any delay can cause irreversible loss of function of the ulnar nerve. As causes of the compression of the ulnar nerve tumours, inflammation of the sourrounding tissue or trauma have been described. In this paper we report about compression of the ulnar nerve in the canal de Guyon due to a thrombosed aneurysme of the ulnar artery. This condition is quite rare. It is characterized through sudden onset of pain in the hand. Immediate surgery with decompression of the nerve, as we did in our case, will result in complete recovery.

[The prevention and treatment of post-operative and post-traumatic oedema. Chemical laboratory tests on the renal tolerance of beta-aescin (reparil) (author's transl)].

Renal tolerance of the preparation beta-aescin (Reparil) was tested in cases of oedema following fresh soft tissue crush injuries of the extremities and surgical and reconstructive operations on the hand. Forty in-patients with healthy kidneys were treated with 10 mg beta-aescin intravenously twice daily for six days after operations on the hand and after accident surgery. During this period the following tests were carried out every day and two days after cessation of treatment: urea content in urine and serum, creatinine in serum, creatinine clearance, albumin in urine, urine osmolarity and blood and urine sugar. All values obtained were within the normal range. No impairment of total renal function was observed. Taking the stated dosage instructions and contra-indications into consideration, beta-aescin can thus be administered without hesitation.

[The tendon interposition plastic with partly necrosised scaphoid pseudarthrosis (author's transl)].

We have seen in 4 cases of 46 operated scaphoid pseudarthrosis aseptic bone necrosis of a fragment. According to the tendon interposition plastic of Froimson for the substitute arthrose of the saddlepoint of the thumb. We exstirpated the bionecrotic fragment of the scaphoid, splitted the tendon of the flexor carpi radialis muscle, removed a part of the tendon from the belly of the muscle, rolled it in and fixed it in the excavated cavity of the bone. After an observation time of 6 months to 1.5 years all 4 patients have not developed complications.
