Dermal cylindromas (turban tumour) and eccrine spiradenomas in a patient with membranous basal cell adenoma of the parotid gland.

A patient with coexisting dermal cylindromas (Turban tumour), eccrine spiradenomas and a variant of basal cell adenoma of the parotid gland characterized by prominent focal deposition of hyaline (membranous basal cell adenoma of parotid gland) is reported. The microscopic appearance of the cutaneous and parotid gland tumours was very similar. This rare association of analogous tumours of sweat and parotid glands has been described only in 5 previous cases. It is hypothesized that the occurrence of the 3 different tumours found in this individual may be explained by the multipotential reserve cell concept and these tumours perhaps could be considered as different expressions of a single genetic disorder.

Zoon's balanitis treated by circumcision.

Zoon's balanitis is a chronic balanitis affecting parts of the glans penis and prepuce, usually considered refractory to treatment. This article reports the study of seven cases successfully treated by circumcision.

Herpetiform eruption with leukocytoclasis: a distinctive clinico-pathological variety of erythema multiforme.

Six patients are described who experienced a prodromal period suggesting an infective process, followed by a distinctive polymorphous cutaneous eruption. The most striking lesion, present in all of them and giving the eruption a distinctive herpetiform appearance, included closely grouped vesicles resting on an erythematous base. Histopathological examination of these lesions showed features of dermal and epidermal types of erythema multiforme but with prominent leukocytoclasis. We suggest that the clinico-pathological picture of our patients may be considered a distinctive clinico-pathological variety of erythema multiforme.

Acquired zinc deficiency syndrome during total parenteral alimentation. Clinical and histopathological findings.

An acute zinc deficiency state developed in 6 patients on total parenteral alimentation, which allegedly included zinc 37 mumol/l. The actual concentration of this trace element when measured by us, was 3 mumol/l. The clinical picture consisted of an acrodermatitis enteropathica-like syndrome, with subsequent development of distinctive nail lesions, which consisted of white transverse bands in all finger- and toenails. Histopathological examination of several of these cutaneous lesions revealed distinctive changes, which we consider rather specific of zinc deficiency states. They consisted in parakeratosis, cleavage and detachment of the superficial layers of the epidermis, mainly intracellular and also extracellular edema of the epidermal cells, development of microvesicles at different epidermal levels, and severe vacuolar alteration of the dermoepidermal junction. These changes also affected the outer root sheath of the hair follicles.

[Nodular hidradenoma. Clinical comments, histology and therapeutics apropos of 2 cases].

The clinico-pathological and therapeutics features of two cases of nodular hidradenoma are reported. The polymorphic microscopic picture that characterizes this peculiar tumor can be seen in one of our observations. The other case belongs to the variety called "clear cell hidradenoma". The latter of big size and with local adenopathies showed a prompt regression to treatment with bleomycin.

[Becker's nevus associated with a smooth muscle hamartoma].

A new case of Becker's nevus associated with smooth muscle hamartoma is reported. This is the third case reported in the literature. The different associations reported in the Becker's nevus give support to the mesenchymal-epithelial interactions.

[Desmoplastic melanoma].

Conley et al., in 1971, described a special type of melanoma characterized by a superficial melanic lesion at the onset; repeated local relapses as subcutaneous tumorations with an histological picture closely resembling an atypical fibroxantoma or fibrosarcoma. After a review of all the published material the autors presents a personal case with the clinical, histological and evolutive characteristics of this disease. The most interesting findings of the published case are the following: The special stains for the melanocytes (silver stain, Dopa, tyrosinase and cholinesterase) were all negative. There was an intense positivity for the lisosomal enzymes (non specific sterases, and acid phosphatases). The ultrastructural study of the tumoral tissues as well as the cells of cultures showed abundant cells with tumoral aspects, with prominent nucleoli somewhat dilated granular endoplasmic reticulum, myelin-like figures, lipidic vacuoles and abundant lisosomes. No melanosomes or premelanosomes were observed. Beside these tumoral cells abundant typical fibroblastic elements were found. There was a great amount of collagen fibers with periodicity superior to the normal. The conclusion is that the desmoplastic melanoma must be considered as a tumor of mesenchimatous origin intervening in its development multiple local and general factors.

[Epithelioise sarcoma. A myofibroblastoma tumor].

A case of epithelioid sarcoma in a 48 year old man is described by the authors. The ultrastructural study showed the presence of abundant cellular elements showing some features closely similar to those of the so-called myofibroblasts. These were: folds in the nuclear membrane, abundant rough endoplasmic reticulum, thick bundles of microfilaments resembling those of the smooth muscle, junctional complexes and degenerative changes (lipid droplets and autophagic vacuoles). These findings suggest the possibility that the epithelioid sarcoma could be a neoplasia originating from the myofibroblasts.

[Scleromyedema with special hyperkeratosis lesions and calcifications].

An observation of Scleromyxedema is repported by the authors. The remarkable peculiarities of this case are the following: 1. Absence of abnormal paraprotein. 2. Positive direct and indirect immunofluorescence. 3. Slight increase of plasma cells of bone marrow. 4. Presence of calcifications. 5. The hyperkeratotic lesions. Those of the fingertips with histology of porokeratosis. 6. Scarcity of the metachromatic depots of muchopolisacharids in the dermis evident only with the Alcian-PAS stain.
