Hiatal hernia--manometric and symptomatic assessment of failed surgical management in 60 patients.

The recurrent symptoms in 60 patients who had undergone surgical management of hiatal hernia with reflux were analysed with the help of manometry. Despite reduction of the hernia in 43 patients, the primary and important defect of sphincter incompetence was the commonest residual abnormality. Oesophageal spasm associated with reflux or occurring as a primary condition was another cause of recurrent symptoms. The addition of vagotomy and pyloroplasty to the hiatal hernia repair procedure in several patients did not prevent reflux. The use of valvuloplasty techniques such as the Hill posterior gastropexy, the Belsey Mark IV operation or the Nissen fundoplication should be encouraged when the primary defect is incompetence of the lower oesophageal sphincter.

Gastric ulcer after the Nissen fundoplication: a complication of alkaline reflux.

Four of a series of 40 patients who underwent the Nissen fundoplication to correct reflux oesophagitis developed gastric ulceration 24, 22, 15 and 13 months respectively after the procedure. The gastric ulceration is attributed to bile reflux, and the original oesophagitis to "alkaline" reflux. When severe oesophagitis results from "alkaline" reflux, a Roux-en-Y bile diversion operation with vagotomy and fundoplication should be considered.

Lower oesophageal sphincter resting pressures in achalasia and the response of the sphincter to swallowing and drugs.

Previous reports on the profile of lower oesophageal sphincter (LOS) pressures and swallow responses in achalasia have been conflicting. Both normal and high resting pressures have been reported. Many reports have noted a failure of relaxation of the LOS in response to swallowing. Manometric studies were performed on 17 untreated patients with achalasia, of whom 76% were found to have a resting peak end-inspiratory pressure significantly greater than normal (P = 0,001). Nine patients showed relaxation of the LOS in response to swallowing but this relaxation was usually inadequate and of brief duration. Contractions were premature in 16 patients. One patient showed a manometric pattern closely simulating a Mobitz type 1 atrioventricular block in response to repeated swallows. A common pathophysiological process is postulated. In one patient an injection of secretin reduced the high resting LOS pressure. This supports previous evidence that a hypersensitive sphincter in achalasia is due to a hypersensitivity to gastrin. Hyoscine-N-butylbromide (Buscopan) caused a significant reduction in LOS pressure in all patients in whom it was used.

The volumetric measurement of gastric emptying and gastric secretion by a radioisotope method.

Most methods of measurements of gastric emptying rely on the serial estimation of intragastric volume and do not separately account for the volume of fluid which has been added to the meal by gastric secretion, duodenal reflux, or swallowed saliva. The volume emptied is therefore underestimated. A method of measuring gastric emptying using [125I]RIHSA and the Volémetron is presented. The volume of fluid added to the meal is taken into consideration in this method, giving a more accurate reflection of gastric emptying. Using this method in the dog, emptying was found to be linear rather than exponential.

Haemangiopericytoma of the thigh.

Haemangiopericytoma is a very vascular tumour which is slow growing and often painless. It is important to realise the malignant potential of these tumours and to treat them by wide surgical excision. Angiography was helpful in this case because it gave warning of probable malignancy.
