Long-term results of cardiac valve replacement with the Delrin-disc model of the Björk-Shiley valve prosthesis.

One hundred two Björk-Shiley valve prostheses with the Delrin-disc occluder were implanted in 83 patients between January, 1971, and July, 1972. Fifty-eight were in the mitral position, 42 in the aortic, and 2 in the tricuspid. Complete follow-up until 1981 was obtained in 93% of the patients (mean follow-up, 66.8 months). Hospital mortality was 18% and late mortality, 19%. Survival according to actuarial methods was 84.8% at 5 years and 78.1% at 9 years after operation. Thromboembolism was detected in 8.8% of patients but caused no deaths. The incidence was 1.2% and 1.5% per year in those patients treated with Coumadin and antiplatelet agents, respectively. Reoperation was necessary in 13% of the patients. Most survivors (72%) are in New York Heart Association Functional Class I, despite a preoperative status of Class III or IV in 57% of the patients. Hospital mortality may be due to poorer understanding of patient management and less refined techniques of myocardial protection. Long-term survival with this prosthesis is similar to that in more recent studies, and rates of thromboembolism and malfunction compare favorably with other prosthetic valves.

Cardiac tumors. Clinical spectrum and prognosis of lesions other than classical benign myxoma in 20 patients.

Between 1961 and 1983, 20 patients underwent operation at the Texas Heart Institute for primary tumors other than classical benign myxoma. Total follow-up was 70.7 patient years (mean 3.5 years). There were 10 adults and 10 children under 12 years of age. There were two operative deaths and four late deaths. All of the pediatric patients had benign lesions (five fibromas, five rhabdomyomas), and only one patient in this group died (during the operation). All operative survivors are alive between 0.9 and 18 years postoperatively, although in three cases excision was incomplete. Of the 10 adult patients, five had benign lesions; all were completely excised. In the other five adult patients, unresectable malignant tumors were found, and all four operative survivors died of metastatic disease within 8 months. Benign cardiac tumors in childhood have an excellent prognosis when completely excised and appear to have a good short-term prognosis even when excision is incomplete. Although prognosis for benign tumors in adults is good, malignant tumors are associated with very poor survival.

Dysfunction of the Björk-Shiley prosthesis. Report of 32 cases.

We present 32 cases of dysfunction of the Björk-Shiley valvular prosthesis, representing 3.01% of the 1,063 BS prostheses implanted at this clinic. The complications observed were thrombosis of the prosthesis, suture dehiscence with or without endocarditis and disc-suture interference. In selected, favourable cases, we prefer thrombectomy with generous washing of the prosthesis and rotation of the disc, without removal of the disc from the prosthesis. Furthermore, when the dehiscence does not exceed 1/3 of the circumference of the ring, we have achieved very good results with single sutures supported on Teflon patches. Subsequent failures of this procedure were not of a technique nature, but rather due to the poor condition of the patients. In situations of clinical emergency, the diagnosis of valvular dysfunction may suffice as indication of surgery, further diagnostic measures being postponed in view of the data of the physical exploration.

The mechanical properties of human dura mater and the effects of storage media.

Data are presented on the tensile strength and ultimate extensibility of human dura mater. Experiments were performed, prior to, and after, storage in glycerol. The results indicate that storage in glycerol has a small effect on the measured mechanical properties, but this was not statistically significant. In addition, storage in saline after glycerol preservation for up to thirty days was found to have no effect on the mechanical properties of dura mater, as long as the tissue had previously been sterilised and sterility was maintained. However, the considerable variability found in the mechanical properties of human dura prevented this being established beyond doubt. The implications of this variability for tissue selection is discussed.

[Nickel content of parenteral nutrition solutions].

Determination of the nickel content in 49 commercial solutions used in parenteral nutrition, by means of atomic absorption spectroscopy in a graphite kiln. Important differences were found between the different components (less than 5 to 23.000 ppb). The largest concentrations of Ni were found in sodium chloride, phosphate salts, 10% calcium chloride and 2M potassium chloride, whereas the aminoacid, glucose and lipid solutions contained the smallest concentrations. The amount of Nickel in the preparations varied depending on the supplier and different salts of the compound itself. The Nickel intake in 2 TPN solutions with high and low concentrations of Ni (827 and 270 micrograms respectively) was also analyzed, and this intake was higher than the estimated amount absorbed in the diet. In the case of the TPN with a high Nickel content, this was higher than the level detected as being the cause of sensitivity reactions in patients sensitive to Nickel.

[Thiamine stability in parenteral nutrition].

In the present work we have studied the stability of thiamine by HPLC in parenteral nutritions provided with or without lipids. As source of nitrogen we have used Freamine 8.5%. Thiamine came from the multi-vitamin complex Pancebrin. The results of this study show that this vitamin is not stable for more than 24 h, even if parenteral nutritions are kept at 4 degrees C, except for solutions that do not contain lipids and are kept at 4 degrees C. Light does not influence the degradation rate of thiamine.

A new surgical technique for aneurysmal right atrium.

A new surgical technique for reduction in size of aneurysmal right atrium (ARA) is described. The extracorporeal perfusion is based on extrapleural single stage caval return and profound hypothermia, with circulatory arrest. This method enabled us to diminish the cavity dimensions in the presence of a giant friable ARA, by excising the thinned portion of the free atrial wall and double-layer closure of the remaining edges (close to the AV groove) to the crista terminalis.
