[State of the therapy of hemangiopericytoma].

To informations from literature 700 patients with a hemangiopericytoma can be expected globally until the year 1988. Actually there is no uniform therapeutic procedure for this cancer disease. The own therapeutic strategy is represented, successfully applied to three patients: Actually we consider optimal the combination of surgical extirpation with following radiotherapy of the former tumor bed with a minimum target dose of 45 Gy to guarantee a total remission and a long-time survival.

Skinned fibers of human atrium and ventricle: myosin isoenzymes and contractility.

Different myosin isoenzymes of pig and human atrium and ventricle and rat ventricle were characterized by two approaches: pyrophosphate polyacrylamide gel electrophoresis (PP-PAGE) and analysis of the myosin P light chains by two-dimensional polyacrylamide gel electrophoresis (2D-PAGE). We further investigated the relation between atrial and ventricular myosin isoenzymes of human, pig, and rat, and the maximum (unloaded) shortening velocity (Vmax) and the Ca2+ sensitivity of chemically skinned fibers of the same species. The myosin isoenzymes of both human and pig atrium comigrated in the PP-PAGE with rat V2 isomyosin, whereas the ventricle of human and pig comigrated with rat V3. In both human and pig ventricle, a myosin P light chain polymorphism exists (two phosphorylatable P light chains with the same molecular weight but different isoelectric points). In contrast, we found no P light chain polymorphism in the atrium of human and pig and in the ventricle of rat (one phosphorylatable P light chain only). A correlation exists between Vmax, Ca2+ sensitivity, and atrium- and ventricle-specific myosin isoenzymes of human and pig. Vmax was determined by the slack-test method. Plots of delta l versus delta t of atrial and ventricular skinned fibers were well fitted by a single straight line up to delta l = 15% and delta l = 13%, respectively. Vmax of skinned ventricular fibers was lower than Vmax of skinned atrial fibers in both human and pig. Ca2+ sensitivity of skinned fibers of ventricle, however, was higher than Ca2+ sensitivity of atrial skinned fibers in both human and pig.

[Current clinical aspects of changes in the lung parenchyma caused by the interaction of polychemotherapy and radiotherapy in the treatment of small cell bronchial cancer].

The use of radiotherapy in the treatment of bronchial carcinoma evokes changes of the healthy lung parenchyma which may lead to pneumonitis and fibrosis. At present more than 20 tumour chemotherapeutics with lung-toxical effect are known, which may induce a pneumonitis or a fibrosis. When two or more lung-toxically acting cytostatic drugs are combined, we have to take into consideration interactions with synergistic character on the healthy lung parenchyma. The combination of a lung-toxical cytostatic drug with the radiotherapy or the combination of a polychemotherapy with the radiation may cause pneumonitides and fibrosis with increased mortality rate. In our therapy programmes for the treatment of the small cell bronchial carcinoma the polychemotherapy of the tumours (I. cyclophosphamide, methotrexate, nitrosomethylurea, II. doxorubicin (adriamycin), dacarbacin, vincristine, III. cyclophosphamide, adriamycin, methotrexate) and the radiotherapy were simultaneously used. Hereby a pneumonitis was x-ray-diagnostically ascertained in more than 70% and a fibrosis in 30-80% of the patients treated. In our opinion these high rates of changes of the lung parenchyma have their cause in a complex process: interaction of the tumour chemotherapeutic drugs among one another; interaction of the tumour polychemotherapy with the radiotherapy (particularly in simultaneous application); the bronchial carcinoma is, depending upon anatomical localisation of the tumour and size a risk factor for an infection; the immunosuppressive situation of the patient increases the danger of an infection particularly in the area of the respiratory tract during the therapy. The course of a pneumonitis is decisively determined by a secondary infection.

[Morphological and biochemical changes in the pars obliqua of the vastus medialis muscle in degenerative disorders of the knee joint].

Using 34 muscle specimens from the pars obliqua of the vastus medialis muscle of patients with a clinically manifest patellar chondropathy and of those suffering from a gonarthrosis in various positions (cut biopsies under operation), we depicted the slow-twitch (ST) and the fast-twitch (FT) fibres histochemically and ascertained morphometrically the fibre distribution, the areas of the FT- and the ST-fibres as well as the relative fibre volume of the FT-fibres. Furthermore, we determined biochemically the total activity of the enzymes PGK and of the MDH, according to Bergmeyer (1970). By means of the electrophoresis, we determined the isoenzyme pattern of the LDH from simultaneously cut material (Agar-Agarose technique). The mathematical date processing was made by means of the cluster- und discriminance analysis. The results show that the "rule member" (pars obliqua m. vast. med.)--in order to maintain the orthograde gliding motion of the patella in the course of the development of degenerative joint illness--experiences extreme structural and biochemical changes caused by modifications within the neuromuscular motor control pattern. This becomes evident by a FT-hypertrophy at the beginning of the intraarticular disorders and by the increasing development of the "neurogeneous tissue syndrome" with partly fascicular FT-atrophy in cases of progressive degenerative joint processes.

Bioluminescent and fluorometric techniques for determinations of 19 metabolites of ADP/ATP-dependent transformations in energy metabolism in 200 (or 400) mg muscle.

A procedure is described for the determination of the following 19 metabolites in 200 mg gastrocnemius muscle from guinea pig: glucose 1-phosphate and 6-phosphate, fructose 6-phosphate, fructose 1,6-bisphosphate, glyceraldehyde 3-phosphate, dihydroxyacetone phosphate, 3-phospho- and 2-phosphoglyceric acid, phosphoenolpyruvate, pyruvate, glycerol, glycerol 3-phosphate, AMP, ADP, ATP, inorganic phosphate, creatine, creatine phosphate. Bioluminometric and fluorometric techniques for mono- and dinucleotide determinations were used. In the case of fluorometric measurement for NADH, 400 mg tissue were necessary. The coefficient of variation for assays on the same sample was 0.04 for bioluminometric techniques and 0.10 for fluorometric techniques.

Mechanism of the ATP effect in the DNA repair synthesis of gamma-irradiated Escherichia coli cells.

Gamma-irradiation of Escherichia coli cells made permeable to deoxynucleoside triphosphates (dNTP) by toluene induces a repair-type DNA synthesis. As previous studies have shown ATP stimulates this DNA synthesis; we studied the mechanism of the ATP effect by analyzing the kinetics of nucleotide incorporation at various dNTP concentrations. The V values of the DNA repair synthesis rise with increasing dose (0-50 Gy); nonirradiated cells showed a negligible nucleotide incorporation. The apparent Michaelis constant KM for dNTP in the assay was 83-143 microM and the value was much higher than for a DNA polymerase reaction in vitro. ATP stimulated the DNA synthesis with concomitant decrease of KM yet unchanged V values. Similar results were obtained with a rec BC strain. We propose that the ATP effect is due to a greater affinity of dNTPs to the DNA polymerase, possibly by a stabilisation of the structural integrity of the complex DNA with repair enzymes. Activation of exonucleases by ATP could be excluded. Addition of NAD to the reaction mixture inhibits the DNA synthesis possibly by activation of ligase which closes the nicks in the DNA strand.

[Gastrointestinal fiberoptic endoscopy in geriatric patients--experience, results and indications (author's transl)].

From the viewpoint of a district hospital in which fiberoptic endoscopy of the upper digestive tract has been carried out since 1970, experiences and results to more than 1900 endoscopic examinations of the upper gastro-intestinal tract are communicated with forty per cent of the examined patients being older than 40 years. According to our experiences we see, when observing the indications and contra-indications, no age limit for such a method of examination. This endoscopic examination presents in our view a valuable enrichment of the diagnostic and therapeutic possibilities of medical care of elderly citizens.

[The retothel sarcoma of the stomach].

At the instance of a female patient with a retothelial sarcoma (immunoblastoma) of the stomach who after 2 1/2 years post operationem (local tumour excision) does not give a reference to an unfavourable course the author deals with the rarity of this gastric tumour. A deánite attitude is adopted to clinic and problem of the preoperative diagnostics of this rare gastric tumor as well as to the prognosis. By radiological and fiber-gastroscopic investigation of the stomach cannot always preoperatively made the diagnosis corresponding to the histologic structure of the tumour. Therefore, in this case the surgical intervention is aspired to. Postoperatively in patients with rare or malignant tumours of the stomach control examinations are regularly to be recommended. Altogether the prognosis of retothelial sarcomas of the stomach are not more unfavourable than those of the gastric carcinomas.
