[Ultrasound assessment of extranodal abdominal lymphoma involvement: an overview].

The abdominal sonograms of 680 patients with malignant non-Hodgkin's lymphomas (NHL) and Hodgkin's lymphomas (HD) were reviewed. An involvement in the spleen (n = 101), liver (n = 47), gastrointestinal tract (n = 70), kidney (n = 17), and various other organs (n = 19) could be demonstrated on ultrasound. Lesions were confirmed by histology (n = 117), sonographic (n = 141) and clinical follow-up. The different sonographic features of lymphomatous infiltrates in different extranodal sites are described, and possible correlations between lymphoma subtypes and sonographic texture characteristics are discussed. In cases with splenic and liver involvement, lymphoma subtypes had different patterns. A predominantly diffuse or small nodular infiltration was found in NHL of low-grade malignancy, and large nodular lesions were found to be more typical in high-grade NHL. The ultrasound findings basically do not allow a specific diagnosis and even may resemble a variety of malignant and non-malignant diseases other than lymphomas.

Adrenal pheochromocytomas: a broad spectrum of sonographic presentation.

Sonographic findings in 19 patients with proved adrenal pheochromocytomas observed over a 10 year period were evaluated retrospectively. Adrenal tumors were analyzed by number, size, echogenicity, internal echogenicity, and biologic behavior. There were 16 benign and 3 malignant pheochromocytomas, all of which were well marginated or encapsulated and ranged from 1.4 to 11 cm in greatest diameter (mean, 4.8 +/- 2.2 cm). A broad spectrum of sonographic appearances has been noted, including purely solid tumors (68%), complex masses (16%), and cystic lesions (16%). Compared with renal parenchyma as a reference tissue, 10 (77%) of 13 solid pheochromocytomas were isoechoic or hypoechoic, whereas three (23%) were hyperechoic. Six (46%) of the solid tumors were homogeneously echogenic, and seven (54%) were heterogeneous. Ultrasonic discrimination between benign and malignant pheochromocytoma on the basis of acoustic features alone has proved impossible. Abdominal sonography, however, provided evidence of malignancy in all three patients with malignant tumors by disclosing regional or distant metastases. In conclusion, rather than showing a specific uniform ultrasonographic appearance, pheochromocytoma is associated with a broad spectrum of possible sonographic presentations.

[Lymphoma infiltration of the liver: spectrum of ultrasound characteristics in 47 patients].

Between 1980 and 1993, 680 patients with malignant lymphoma were referred for abdominal sonography as a part of initial staging. In 53 patients intrahepatic abnormalities suspected for lymphoma involvement were detected. 6 of these had metastases of lung cancer (n = 4) or microabscesses (n = 2). Sonographic findings of 47 patients with hepatic lymphoma were reviewed. Hepatic lymphoma was confirmed by histologic examination (n = 23) and sonographic/clinical follow-up (n = 24). 12 patients had Hodgkin's disease, 18 high-grade NHL, and 17 low-grade NHL. Lesions were hypoechoic in all cases. Five different sonographic patterns were found: small nodular lesions (n = 28; 60%), large nodular lesions (n = 14; 30%), bulky disease (n = 2; 4%) and diffuse involvement (n = 2; 4%). One patient (2%) had lymphomatous involvement which surrounded the portal vessels. High-grade NHL preferred patterns of large nodular or bulky diseases (9 of 17 patients). Small lesions were found to be more typical in low-grade NHL (13 of 18 patients), and Hodgkin's disease (9 of 12 patients).

Spontaneous splenic rupture in acute myeloid leukemia: sonographic follow-up study.

We report on a 52-year-old patient with acute myeloid leukemia. After induction chemotherapy, routine ultrasound displayed a round focal hyperechoic intrasplenic lesion. Tumor was primarily misdiagnosed for splenic hemangioma. Diagnosis of spontaneous progredient splenic rupture was made by color-coded Doppler sonography, sonographic follow-up examinations and confirmed by splenectomy for final treatment. No trauma was found.

Sustained trilineage response in a patient with ALG-resistant severe aplastic anaemia after treatment with G-CSF, erythropoietin and cyclosporin A: association of recovery with marked elevation of serum alkaline phosphatase.

Aplastic anaemia is characterized by multilineage bone marrow failure resulting in pancytopenia. We have successfully treated a young woman with severe aplastic anaemia (SAA) who was resistant to antilymphocyte globulin (ALG) and corticosteroids, with a combination therapy consisting of erythropoietin, cyclosporin A and granulocyte-colony stimulating factor (G-CSF). The patient received erythropoietin and CSA for a period of 10 months without success before G-CSF treatment was started. After 6 weeks of G-CSF therapy she responded with a sustained trilineage recovery. This suggests that immunosuppression together with haemopoietic growth factors may be an effective treatment in patients with SAA who are ALG resistant and cannot be treated by BMT.

[Sonographically guided fine-needle puncture of adrenal tumors].

Ultrasound-guided fine-needle aspiration biopsies were performed in 37 patients (23 women, 14 men; mean age 58 [47-81] years) out of 310 patients in whom an adrenal tumour had been diagnosed by ultrasound between 1985 and 1991. In 21 the tumour diameter was 2-4 cm, in 16 over 4 cm. In 36 patients (97%) the material was suitable for histological analysis. There were 15 metastases, 4 primary adrenal carcinomas and 17 adenomas. 20 of 27 with an underlying malignant tumour had a malignant adrenal tumour (one of the aspiration biopsies in these cases was false-negative), while in 7 the space-occupying lesion was found to be benign. There were no false-positive findings. Ultrasound-guided fine-needle biopsy thus proved to be a reliable and informative method in the diagnosis of adrenal tumours. It should be undertaken under appropriate indication to ascertain the benign or malignant nature of a suspected adrenal lesion.

Preclinical and clinical experience with cisplatin and carboplatin and simultaneous radiation in non-small cell lung cancer.

Preclinical experiments with cisplatin and carboplatin showed radiosensitizing capability for these drugs against non-small cell lung cancer (NSCLC) cell lines and phase II clinical investigations were undertaken in patients with stage IIIA/B NSCLC. In the first trial, cisplatin 20 mg/m2 was infused on the first day of every treatment week, followed in about 1 hour by radiotherapy. Radiotherapy was given in single daily 2-Gy doses 5 days a week for 3 weeks, and, after a 2-week interval, for an additional 2 weeks. There were 5 complete responses (CRs) and 15 partial responses (PRs), while 6 patients had no change and 4 progressive disease, for a 67% overall response. Median survival was 14 months; the 2-year survival rate was 20%. To investigate the maximum tolerated dose of carboplatin, escalating doses were given on day 1 of each treatment week; simultaneous radiotherapy was administered as in the cisplatin study. Drug doses began at 100 mg/m2 and were increased by increments of 10 or 20 mg/m2 to a maximum of 200 mg/m2, with 5 patients treated at each dose level. Dose escalation was to stop when 3 of 5 patients developed intolerable World Health Organization (WHO) grade 3 myelosuppression. No grade 3 or 4 leukopenia occurred at carboplatin doses less than or equal to 140 mg/m2. Grade 3 leukopenia was seen in only 1 patient from the groups receiving 150, 160, or 180 mg/m2. Of the 35 patients evaluable thus far, 1 achieved CR and 18 PR, while 10 had no change and 6 progressive disease despite treatment, for a 54% overall response rate. Response rate in patients with stage IIIB disease was 48%.(ABSTRACT TRUNCATED AT 250 WORDS)

Splenic injury: sonographic investigation.

We report on sonographic findings in 30 patients with traumatic (n = 13) or non-traumatic (n = 17) splenic injury. An ultrasound (US)-based injury-severity-score consisting of three grades was devised and retrospectively applied in all patients. Initial sonographic abnormalities included: Intraperitoneal fluid (n = 10), perisplenic fluid (subcapsular hematoma) (n = 13), intrasplenic fluid (n = 10), splenic lacerations (n = 7) and intrasplenic flow phenomena diagnosed via Doppler-Ultra-sound (n = 3). The final diagnoses were splenic injury due to acute blunt traumatic episode (n = 9), chronic traumatic episode (n = 2), infectious mononucleosis (n = 2), splenic infarction (n = 14), and various causes (n = 3). On the basis of both imaging information and clinical factors, splenectomy was performed in 16 out of 30 patients. Five (31%) of these 16 patients were initially treated non-surgically but US findings during follow-up examinations made splenectomy necessary. Our results have shown that US remains a useful technique in identifying and quantifying initial splenic injury as well as monitoring the healing progress. Final treatment choices depend furthermore on clinical and laboratory assessments.

[Percutaneous drainage of liver and splenic abscess].

42 patients with solitary (n = 34) and multiple (n = 8) abscesses of the liver (n = 36) and the spleen (n = 6) were treated with ultrasound guided percutaneous interventions. 38 patients (90%) underwent a total of 97 closed abscess aspirations using needles of 0.9 and 1.3 mm in diameter. In 4 cases (10%) percutaneous catheter drainage was performed. Intravenous antibiotics were used in all cases. Those patients with closed abscess aspiration additionally received local injection of aminoglycosides into the cavity. 40 out of the 42 patients could be treated successfully by percutaneous methods for a cure rate of 95.2%. Percutaneous drainage failure occurred in 2.4%. One patient with multiple liver abscesses and catheter drainage died from myocardial infarction (hospital mortality 2.4%). Complications of ultrasound-guided interventions included two minor bleedings, requiring no therapy, and one pleural empyema (complication rate 7.1%). There were no treatment related lethal complications. These results indicate that abscesses of the liver and the spleen up to 10 cm in diameter can be effectively treated by closed (repetitive) needle aspiration and antibiotic therapy with a relatively low rate of complications. About half of our patients with abscesses of more than 10 cm received percutaneous catheter drainage. On the basis of our experience surgical drainage of liver abscesses and splenectomy in splenic abscesses should be restricted to those cases with percutaneous drainage failure.

Focal involvement of malignant lymphoma in the liver.

In this study the sonographic findings of 30 patients with hepatic lymphoma have been reviewed. Lesions have been confirmed by autopsy (n = 1), laparatomy (n = 1), sonographically guided biopsy (n = 15), and during long-term follow-up (n = 13). Ten patients had Hodgkin's disease (HD) and 20 non-Hodgkin lymphomas (NHL). Focal lesions, less than 3 cm in diameter, were seen in 20 patients. The remaining ten cases presented large-nodule transformation of the parenchymal texture. All lesions were predominantly hypoechoic with a halo sign in two cases. No diffuse involvement of the liver was observed, and no correlation between histological lymphoma subtyping and patterns of hepatic infiltration was found.

Sonographic investigation in the diagnosis of intrasplenic fluid collections.

The authors report about the diagnosis of 55 patients for whom cyst-like lesion sites or predominantly anechoic lesions were diagnosed by means of sonography of the spleen. Sonographic analysis included interventional sonography with fluid aspiration in accordance with diagnostic and therapeutic guidelines (n = 19), catheter drainage (n = 3), B-mode pulsed Doppler-Ultrasound, and short-term follow-up examinations. The final diagnoses were splenic cysts (n = 25), splenic ruptures (n = 10), splenic infarctions (n = 12), splenic abscesses (n = 7) and splenic metastasis (n = 1). Because of the ultrasound findings in diagnosis and during the follow-up observations, 15 patients underwent splenectomy. Special attention is drawn to the clinical relevance of these procedures for diagnosis of splenic liquid masses.

Sonographic findings of splenic metastases.

Intrasplenic metastases are an uncommon manifestation of malignant disease. Between 1980 and 1990, focal splenic lesions were diagnosed through sonography in 178 patients at our hospital. In 13 of these cases (7.3%), the final diagnosis was intrasplenic metastasis. A variety of echo patterns was observed, with hypoechoic (n = 11) lesions being typical. The clinical significance of these findings is discussed.

Prognostic value of response to chemotherapy using ultrasound in lung cancer with metastatic liver involvement.

Between January 1981 and December 1989, a total of 205 patients with lung cancer were treated in different multicenter trials in our hospital. In 130 cases, small cell lung cancer (SCLC), and in 75 cases, non-small cell lung cancer (NSCLC) was diagnosed. Abdominal sonography for pretreatment staging revealed liver metastasis in 59 patients (42 SCLC, 17 NSCLC), sonographic patterns of liver metastasis were described and the prognostic significance of evaluation of response according to abdominal ultrasound after the first two cycles of treatment was investigated. For both histological types of lung cancer, the hypoechoic or isoechoic (80%), small noduled (97%) liver metastasis with multifocal spread (86%) was found to be typical. Sonographic follow-up examinations for liver metastasis were performed in 46 patients (35 SCLC, 11 NSCLC), and 134 chemotherapy cycles. Response to chemotherapy according to abdominal ultrasound was seen in only 3 out of 11 patients with NSCLC (27%) and 17 out of 35 with SCLC (49%). Best response was achieved after the first two cycles with only 6 patients (6 SCLC, 0 NSCLC) exhibiting complete liver metastasis remission. All other patients exhibited on ultrasound a progressive or non-responsive illness. In summary, sonography is reliable with respect to the diagnosis and follow-up examination of liver metastases in lung cancer. A lack of metastasis regression subsequent to the first two cycles of therapy is associated with poor prognosis and continued treatment cannot be justified in such patients.

Multiple myeloma of the serosa coat.

We report on a 47-year-old male patient with IgA-kappa plasmacytoma, who 12 months subsequent to diagnosis developed marked ascites. On light and electron microscopy morphologically identical plasma cells with bizarre intracytoplasmatic material were found in the bone marrow, pleural exudate, and ascites fluid. This kind of extramedullary spread is extremely rare and usually resists therapy. The mean survival rate for the 9 patients with malignant plasmacellular ascites whose cases have been documented so far was 2 months. After progression under VCMP regimens our patient has been successfully treated over a period of more than 4 years with i.v. VP16 monotherapy with additional intermittent local administration of methotrexate and systemic poly-chemotherapy according to the VAD scheme.
