[Mibelli's porokeratosis after bone marrow transplantation].

A 24-year old male patient developed, on both legs, lesions typical of Mibelli's porokeratosis 22 months after bone marrow transplantation, under treatment with cyclosporin A. He denied any family history. Mibelli's porokeratosis seldom develops after an immunosuppressive treatment, and to our knowledge it has exceptionally been described after bone marrow transplantation. A possible complication of Mibelli's porokeratosis is the development of Bowen's disease, basal or squamous cell carcinomas. Immunosuppressive treatment might facilitate the degeneration. For this reason, these subjects should be periodically and carefully examined.

Counting the nucleolar organizer region-associated proteins is a prognostic clue of malignant melanoma.

BACKGROUND AND DESIGN: The nucleolar organizer regions (NORs) are chromosomal loops of DNA to which acidic proteins are associated that are seen by silver staining as black dots within the nucleoli (hereafter, these silver-staining NORs will be referred to as AgNORs). As their size and number reflect cell and nuclear activity, their counting in paraffin sections is regarded as a useful tool for diagnosing and prognosing malignant tumors. We counted AgNORs in 98 patients with stage I melanoma, followed up to an average of 73 months, to verify whether the number of AgNORs is of prognostic value. RESULTS: The number of AgNORs averaged 2.792 +/- 0.901 in the 64 patients without metastases and 4.889 +/- 1.403 in the 34 with metastases. In patients with counts higher than 3.62, there was an 82% probability of metastases developing. CONCLUSION: The technique is fast, simple, and reproducible with easily available reagents and standard light microscopy. In our population, AgNOR counts constituted a more accurate prognostic indicator than Clark's level and Breslow's thickness. Confirmation in a new population is needed.

Primary malignant melanoma of male urethra.

A case of primary malignant melanoma of the male urethra is reported. Treatment included partial urethrectomy, bilateral inguinal and iliac lymphadenectomy, and post-surgical systemic chemotherapy. After thirty-six months, the patient is alive with cutaneous, pelvic lymph node, and gastric metastases.

Impedance cardiography for repeated determination of stroke volume in elderly hypertensives: correlation with pulsed Doppler echocardiography.

In this double-blind, crossover study the authors have validated stroke volume determination by impedance cardiography against the pulsed Doppler echocardiographic method in elderly hypertensives. They found a good correlation between the stroke volume values obtained by the two methods over a range of values from 30 to 130 mL. The coefficient of linear regression was about .95 at each visit. The mean of the differences was -0.73 mL with a standard deviation of 8.46. Given that individual differences are normally distributed, the values corresponding to 2 standard deviations of the mean define a range covering 95% of the observed differences. From the distribution of the data around the mean plot it appears that, in comparison with pulsed Doppler, impedance cardiography tends to slightly underestimate stroke volumes of greater than 90 mL and to overestimate values of less than 50 mL. The results of this study indicate that impedance cardiography may represent a reliable alternative to pulsed Doppler echocardiography for the noninvasive estimation of cardiac output at rest in elderly patients.

[Immunohistochemistry and in situ hybridization techniques in the study of 75 patients with HPV-induced lesions of the genitalia].

Seventy-five patients with anogenital warts were biopsied in the Center for Sexually Transmitted Disease of Galliera Hospital (Genoa). Bioptic specimens were processed for histologic examination, immunohistochemistry using polyclonal antibodies against HPV capsid common antigens and in situ hybridization using HPV 6/11, 16/18, 31/35/51 DNA probes. The aim of the study was to detect patients positive for HPV 16/18, 31/35/51 DNA probes. These HPV types have a higher oncogenic potential and patients with lesions harbouring them are considered to be at risk for progression to malignancy. These patients should be followed up carefully and periodically for early detection of new lesions and neoplastic transformation.

Age-related antihypertensive and haemodynamic effects of verapamil SR: clinical results and effects on atrial natriuretic peptide.

The antihypertensive effect of the calcium-antagonist verapamil was investigated in two groups of patients, adult hypertensives (AH, less than 65 years of age) and elderly hypertensives (EH, greater than 65 years of age), who were treated with 240 mg p.o. sustained-release (SR) verapamil for 4 months. Arterial blood pressure was significantly reduced in both groups: the responders' rate was 65% in the AH group and 82% in the EG group. The heart rate was slightly but not significantly reduced. An improvement in cardiac haemodynamics was observed [cardiac index (CI), from 3.00 +/- 0.51 to 3.25 +/- 0.83 ml min-1 m-2 in AH and from 2.35 +/- 1.08 to 3.04 +/- 0.86 ml min-1 m-2 in EG]. We also evaluated the plasma concentrations of atrial natriuretic peptide (ANP) before and after treatment; ANP levels increased significantly only in the EH group. No serious side effects occurred. In conclusion, verapamil SR provided effective and well-tolerated antihypertensive treatment in both adult and elderly patients.

[Hyperkeratosis lenticularis perstans. A clinical case and review of the literature].

A case of a woman of 55-years-old with verrucous lesions localized on the dorsa of her feet and hands is reported. The light microecopy reveals marked hyperkeratosis with some degree of parakeratosis, epidermal atrophy and light lympho-histiocytic infiltrate in the papillary dermis. A diagnosis of hyperkeratosis lenticularis perstans is made on the basis of the physical examination and histologic findings. A brief review of the literature on the genetic, ultrastructural characteristics and pathogenesis of the disease is made.

A double-blind comparative study of ketanserin with atenolol in essential hypertension.

Sixty patients, with mild to moderate essential hypertension, were considered for a double-blind trial comparing the effects of ketanserin and atenolol. After 2 weeks of placebo, a group of 30 patients was given ketanserin 20 mg twice daily for 15 days and 40 mg twice daily for the subsequent 45 days, while the second group (30 patients) was given atenolol for 60 days at the dose of 100 mg once daily. Blood pressure and pulse rate in the supine and standing positions were evaluated every 15 days. After the beginning of treatment with ketanserin, there was a gradual but highly significant decrease of the diastolic and systolic blood pressure values. No important side-effects or significant alterations in the biochemical parameters considered were observed during treatment with ketanserin.

Topical disodium cromoglycate in the management of pyoderma gangrenosum.

A 52-year-old woman who had had ulcerative colitis for more than ten years experienced a third relapse of pyoderma gangrenosum of her lower extremities. She was treated with topical disodium cromoglycate. The dosage of salicylazosulfapyridine and steroids she was taking, and by which the ulcerative colitis, but not the pyoderma gangrenosum could be controlled, was not increased. Because pyoderma gangrenosum is often very difficult to treat and may require an aggressive approach using drugs that can cause serious side effects, we believe that a topical therapy is worth trying.
