Overexpression of c-myc and loss of heterozygosity on 2p, 3p, 5q, 17p and 18q in sporadic colorectal carcinoma.

AIM: The aim of the present study is to evaluate the prognostic influence of loss of heterozygosity on 2p, 3p, 5q, 17p and 18q, and c-myc overexpression on surgically treated sporadic colorectal carcinoma. METHODS: Tumor and non-tumor tissue samples from 153 patients were analyzed. Fifty-one percent of patients were male, and mean age in the series was 67 years. Tumors were located in the proximal colon in 37 cases, in the distal bowel in 37, and in the rectum in 79 patients. c-myc overexpression was studied by means of Northern blot analysis, and loss of heterozygosity through microsatellite analysis. RESULTS: c-myc overexpression was detected in 25% of cases, and loss of heterozygosity in at least one of the studied regions in 48%. There was no association between clinical and pathologic features, and genetic alterations. The disease-free interval was significantly shorter for patients with both genetic alterations; the presence of both events was an independent prognostic factor for poor outcome in the multivariate analysis (RR: 4.34, p < 0.0001). CONCLUSIONS: The presence of both loss of heterozygosity and overexpression of the c-myc oncogene separates a subset of colorectal carcinoma patients who have a shorter disease-free interval after curative-intent surgery.

[Lumbago and its impact on primary care].

OBJECTIVE: To analyse the effect of Back Pain on Primary Care. DESIGN: Retrospective descriptive study. SITE. "Miralbueno" Health Centre in Zaragoza. PATIENTS: Those patients seen at our Health Centre for the above reason between January 1 and August 31, 1990. MAIN MEASUREMENTS AND RESULTS: 53 cases were recorded: 1.83% of the total number seen. The largest number of cases were of patients between 30 and 50 years old, with no differences as to sex, 35.8% presented radicular irradiation, 5.7% sensitive involvement, 7.5% motor involvement and 9.4% alteration of the osteotendinous reflexes. The average case lasted 21.3 days, with the cases where there was neurological involvement lasting on average longer. Ten patients needed time off work, which meant the loss of 181 working days. CONCLUSIONS: Cases of back pain, which are a common problem at the Primary Care level, should be the target of organisational efforts to cut waiting times for further tests and treatment, in order to reduce the mental and socio-economic repercussions of this pathology.

[Expediency of hospital admission for acute diarrhea syndrome in a short-stay unit].

We present the results of a 187 cases acute diarrhea study managed in the Short Stay Unit of Clinic Universitary Hospital of Zaragoza during 1987-1988. To that aim, we designed an actuation protocol which included the gathering of several data, the realization of complementary studies and the evolutive pursuit. The coproculture resulted positive in a 54.5% of cases and Salmonella was the microorganism more frequently isolated. Most of the species were sensible to common antibiotics and all of them sensible to norfloxacin. Nearly a third part of patients developed some kind of complication. Therefore we infer that acute diarrhea may be subsidiary of diagnosis and treatment in Short Stay Units.

[Quality control of a computer system for repeat prescriptions].

The results of a quality control of a computer system of repeated prescription (RPS) and its basic features are reported. The selected quality criteria were the following: reduction of the bureaucratic index in the visit, correct fulfillment of the treatment order, health attention circuits of the RPS, and compliance of the patient with the treatment. The record of activities and a random sample of treatment orders (TO) were used as sources of data. Quality indexes near the optimal standard (OS) were achieved: bureaucratic index 12.8% (OS = 15%), fulfillment of the TO 94% (OS = 100%), specified dosages 86% (OS = 100%), prescriptions in out-of-date orders 7% (OS = 0%), use higher than calculated 1% (OS = 0%), and use lower than calculated 36% (OS = 25%). Finally, the corrective measures for the detected quality problems are proposed, and the advantages of the computer RPS are discussed.

[Vaccination coverage in a pediatric population attended by a primary health care center].

It is hereby presented a quality control study of a systematic vaccination program developed in an urban primary care center. Target population were children between 0 and 8 years attended in the center. They were considered as having a correct vaccination coverage those cases in which it was possible to verify documented accomplishment of the local vaccination schedule. Out of the 538 children included in our study, 168 (31%) were founded not to have appropriate vaccination coverage evidenced in their clinical records, and their families were requested for an interview: 126 (75%) attended de appointment. In 93 of them it was founded a recording mistake, and 33 cases were verified as no correctly covered. Vaccination coverage rate was 86%, being inverse relationship between age and coverage rate. They are discussed corrective actions derived from the quality control.
