[Clinical management units in primary care: a question under debate].

OBJECTIVE: To find the views of health professionals on how the terms of the new management agreement with clinical units affect the quality of health delivery. DESIGN: Qualitative study to find professionals' views. Individual opinions of key informers were obtained, secondary sources were analysed and a discussion group rounded off the study. The categories for the analysis were established on the basis of the results obtained. SETTING: Primary care districts in the provinces of Sevilla and Cádiz, in the Autonomous Community of Andalusia. Health districts of differing social-economic and geographical characteristics were chosen. PARTICIPANTS: 9 professionals involved in the new system of management. The selection criteria refer to the kind of professional, the unit or work team and the characteristics of the health district. METHOD: The semi-structured interview with key informers and a discussion group were the basic techniques for gathering information. RESULTS: A view that the clinical management units were designed as a savings strategy was detected. There were differences about primary care teams. CONCLUSIONS: Professionals think there is too much rigidity and adding-up in the measurement of targets and objectives. This creates unfavourable working conditions, which affects the quality of health service delivery.

[Mortality registries as a source for detecting health disorders probably of occupational etiology].

Study aim was to assess Andalucian death certificate records to find potential occupational disease problems. Methodology proposed by Rutstein (1983) has been adapted. We had checked Seville City Council Mortality Registry (1987-89) and Andalucian Mortality Registry (1975-85). In the former 8.1% of diagnoses and in the latter 8.6% of diagnoses have one included in the Sentinel Health Event (occupational) List. Accurate occupation data in the Seville City Council Mortality provides appropRiate information about occupational diseases. Limitations of the occupational data in the Andalucian Mortality Registries make it impossible to find the majority of potential occupational diseases, and occupation data were even worse during the second period of study (1980-85).

[Perinatal mortality in Spain. The need of an epidemiologic approach].

Autonomovi Communities perinatal mortality evolution in Spain from 1975 to 1983 is studied aimed at the assessment of existing inequalities in our country concerning this important public health problem. Although a positive evolution has been reported in all of them, differences in their mortality figures have rosen, showing higher rates those communities ranking lower socioeconomic indicators. Epidemiological analysis is needed so as to planning health activities as well as to study social processes responsible for this problems. Embodiment of social services into existing medical services is proposed, especially in Communities where the "sociodemographic disadvantage" is an essential problem. An optimum action in public health throughout the perinatal period world require a comprehensive analysis of the health situation assessing the problems which produce a greater number of survivors with serious sequalae as a result of a lower mortality.
