[Prevalence of hepatitis B virus infection in dentists of District I of the province of Buenos Aires].

Two samples were analyzed; first survey carried out by 250 odontologists chosen out of total number of 1,500 dentists belonging to District I Province of Buenos Aires; the other, by 31 students out of the 180 who attended the last course at the School of Odontology in the National University of La Plata. Through the determination of anti-HBc by enzyme-immuno test, a prevalence of infection due to HBV of 9.6% was found in the odontologists' sample. The prevalence is meaningfully higher than of the general population (5.8), therefore it is demonstrated for the first time in our medium that these professionals constitute a high-risk population. The prevalence found by the students (6.4) was not meaningfully different from that of the general population. The prevalence of an i-HBc found in each of the services taken into account was directly proportional to the number of patients daily assisted. The prevention and sterilization measures for HBV were proved to be unsuitable. No association between a certain HLA pattern and the different serological images, was found.

[Hepatitis non-A, non-B: epidemiological significance in acute viral hepatitis and chronic active hepatitis of hepatological consultation].

157 acute viral hepatitis and 60 chronic active ones have been analyzed focusing on NANB etiology. HAV was implicated in 36.3% of the hole acute viral hepatitis sample, HBV in 29.3%, and HNANBV was presumed as etiology in 31.2%, 5 patients (3.2%) had acute infection by HAV, on previous one by HBV, except for Epstein-Barr virus, no other test for viruses were determined (CMV, HSV, etc.). Male/female ratio was 1.4:1, 1.9:1, and 1.4:1 for HAV, HBV and HNANBV acute hepatitis respectively; HAV was the main etiology in the 0-9 age group (72.2%) although it only represents 11.5% of the sample; small occurrence of HAV hepatitis were found in patients over 40 (8.8%); HBV was clearly prevalent in patients over 50 (65.2%); the highest concentration of NANB etiology was found between 20-39 years old, but it was represented in all age-groups. Out of 49 NANB acute hepatitis, 12.2% had related transfusional antecedents, 12.2% belonged to health care worker group, and 4.1% had a close family NANB hepatitis contact; 71.5% had no reported antecedent. Viral source was presumably implicated in 75.0% of chronic active hepatitis, 25.0% attributable to HNANBV. Results seem not feasible to transfer to general population due to the facts that most patients were of specialized consult, and pediatric assistance is unusual to the authors practice.

Comparative activity of ceftizoxime against aminoglycoside-resistant aerobic gram-negative bacilli.

The in vitro activity of ceftizoxime was compared with that of other beta-lactam antibiotics against 331 aminoglycoside (AG)-resistant clinical isolates. Two hundred and six AG-resistant, beta-lactamase producing, R-plasmid harbouring Enterobacteriaceae strains had MICs ranging from 0.0125 to 0.063 mg/l. AG-resistant Escherichia coli (36 strains) and Klebsiella pneumoniae (19) had MIC 90 values of 8 mg/l. Proteus rettgeri and P. vulgaris as well as Morganella morganii, resistant to several AGs, had MICs ranging from 0.5 to 4 mg/ml. Against all six isolates of AG-resistant Salmonella enteritidis the MIC90 was 0.5 mg/l. Twenty-seven strains of Serratia marcescens, most of which were resistant to beta-lactam and AG antibiotics, had MICs ranging from 0.5 to 8 mg/l. The AG-resistant strains of Enterobacteriaceae producing several AG-modifying enzymes (AAC(3); AAC(2'); AAC(6'); APH(3')) showed MICs ranging from 0.6 to 4 mg/l. Against 10 AG-resistant strains of Pseudomonas aeruginosa producing AAC(3), AAC(6') and APH(3') enzymes, the MICs ranged from 16 to 64 mg/l. In conclusion, ceftizoxime was equally or more active than cefotaxime, cefoperazone, ceftazidime and moxalactam against AG-resistant E. coli, Klebsiella, Morganella, Proteus, Serratia, Salmonella and R-plasmid harbouring Enterobacteriaceae. Ceftizoxime was less active than cefotaxime, moxalactam and ceftazidime against P. aeruginosa.

Electrocardiographic alterations in leads V1 to V3 in the diagnosis of right and left ventricular infarction.

As Q wave and ST segment elevation in leads V1 to V3 may be due either to right ventricular infarction (RVI) or to anterior left ventricular infarction (ALVI), 72 autopsy patients with acute myocardial infarction who had had conventional 12-lead ECG records were studied to determine the accuracy of these ECG criteria, both for the diagnosis of RVI (29 patients, group A) and of ALVI (43 patients, group B). The accuracy of three ECG criteria (Q wave, ST segment elevation greater than or equal to 0.05 mV, and ST segment elevation greater than or equal to 0.1 mV) in diagnosing group A and group B patients was determined in each precordial lead (V1, V2, and V3) and the three criteria were found to be significantly more accurate in diagnosing group B than group A patients. In conclusion, although Q wave and ST segment elevation in leads V1, V2, and V3 may be present in some cases of RVI, their accuracy is too low to be considered useful diagnostic criteria in these patients.

Energy/protein ratio in natural-ingredient diets for weanling rats. II. Energy and protein retention and efficiency of retention.

A 3 x 3 factorial experiment was carried out, in which natural ingredient diets formulated at 3 digestible energy levels (DE) (100, 90 and 80% of NRC, 1978 recommendations) and at 3 energy: protein ratios (EPR) (63, 71 and 79 kJ DE/g crude protein (CP)) were fed to 8 weanling male Wistar rats/group. Half of these animals were sacrificed at the end of the 6-week experimental period and their body composition analyzed. Retentions of protein and energy were assessed by difference from a similar group of 8 rats killed and analyzed at the beginning of the experiment. No interaction was found between the 2 factors studied. Main results showed a decrease in intake and retention of both energy and protein, without affecting the efficiency of retention when energy level of diets was reduced by 20%. An increase in the EPR of the diets led to a decrease in protein intake and to an increase in efficiency of protein retention, without affecting the amount retained. No changes in body composition, nor in composition of live weight gains were found as a result of treatments. It is concluded that a 10% reduction in DE level of the diet is of no consequence, and that an EPR of 79 kJ/g leads to more efficient use of the dietary protein without detrimental effects over any other nutritional indicator.

Ultrasonography in the detection of intra-abdominal retained surgical sponges.

Six cases of confirmed retained surgical sponges are reviewed. In five patients a clean, clear-cut acoustic shadow could be identified in relation to a palpable mass. In four of these patients the sponges were identified preoperatively by their ultrasonographic appearance. Abdominal radiographs were usually obtained afterwards, to confirm the diagnostic impression or to rule out bowel gas or calcifications. Since this ultrasonographic finding has not been described previously, the authors call attention to it in order to prevent misdiagnosis.

Effects of salicylate on insulin and glucagon secretion by the isolated and perfused rat pancreas.

The effects of sodium salicylate, a prostaglandin synthesis inhibitor, on glucose-induced secretion of insulin and glucagon by the isolated perfused rat pancreas have been studied. Sodium salicylate inhibited both basal (2.8 mM glucose) and stimulated (16.7 mM glucose) insulin release in a dose dependent manner (1, 5 and 10 mM). This inhibition is not interpretable in terms of a simple inhibition of cyclooxygenase by sodium salicylate. Basal glucagon release was not changed by 1 mM sodium salicylate but the latter partially blocked its inhibition by 16.7 mM glucose. Higher doses of sodium salicylate (5 and 10 mM) inhibited basal glucagon secretion without affecting its response to 16.7 mM glucose. These findings suggest a predominant stimulatory action of endogenous prostaglandins on glucagon release.

Energy/protein ratio in natural ingredient diets for weanling rats.

The effect of 3 levels of Digestible Energy (DE; 100, 90 and 80% of NRC Recommendations) and 3 different Energy/Protein ratios (EPR; 63, 71 and 79 kJ DE/g Crude Protein [CP]) were studied in a 3 X 3 factorial design using 8 weanling rats per group and diets formulated with natural ingredients. A 20% reduction in DE level decreased significantly energy and protein intake and daily weight gain, and increased the feed intake/body weight gain ratio without affecting protein or energy conversion ratios. Wider values of EPR brought about a decrease in protein intake and CP/body weight gain ratio, with no effect on feed and energy intakes, conversion ratios, nor weight gain. At the protein levels used, energy but not protein concentration of diet, determined growth rate; with the natural ingredient diets used, up to 10% reduction in DE level may be compensated for; wider EPR values led to more efficient utilization of dietary protein.

[C-reactive protein. Value in diagnosis of infectious complications of hydrocephalus children with shunt (author's transl)].

The C-reactive protein, leukocytes count and erythrocyte sedimentation rate, were determined in 32 hydrocephalus children treated with shunt, on the 1st day, 5th day and 10th day postintervention. C-reactive protein was positive on the 5th day postintervention in the 66.6% of the patients with infection and the 100% of these patients on the 10th day. In three patients C-reactive protein was the first sign of infection. In 33.3% of the patients with infectious complications, C-reactive protein positive was earlier than leukocytes and erythrocyte sedimentation rate abnormalities. From these findings, authors suggest C-reactive protein as valuable test in the postoperative control of the hydrocephalus children and should alert to the possibility of a infectious complication.
