Is there an isolated arrhythmogenic right atrial myocarditis?

Two cases with drug refractory ectopic atrial tachycardia are described. A map-guided partial resection of the right atrium (RA) was done after preoperative endocardial catheter mapping had shown well-defined areas of fractionated RA potentials. Intraoperatively, there were no aneurysmal formations present as described by other authors. Histopathologic examination of the resected tissue showed atrial myocarditis in both patients. Postoperative right ventricular myocardial biopsies revealed no inflammatory tissue. A minor elevation of antibodies against echoviruses was found in one case. Postoperative electrophysiologic studies were negative. We conclude: focal RA myocarditis without concomitant ventricular myocarditis may represent one cause of drug-resistant ectopic atrial tachycardia. Map-guided surgical intervention may cure the disease.

[Extra-ovarian site of a virilizing lipid cell tumor].

One of the rare tumors causing virilization are the lipid cell tumors which are mostly located in the ovaries. We report here on a 13 year old girl with virilization due to a lipid cell tumor of the broad ligament. The patient showed increased levels of testosterone, androstenedione and 17 alpha-hydroxyprogesterone while normal values of dehydroepiandrosterone, cortisol, estrogens E1 and E2, and gonadotropins were recorded. The endocrinological and histological pictures, as well as the diagnostic aspects will be discussed.

Phorbol ester induces the biosynthesis of glycosylated and nonglycosylated plasminogen activator inhibitor 2 in high excess over urokinase-type plasminogen activator in human U-937 lymphoma cells.

The tumor-promoting phorbol ester PMA induces changes in the histiocytic human lymphoma cell line U-937 akin to cellular differentiation (Ralph, P., N. Williams, M. A. S. Moore, and P. B. Litcofsky, 1982, Cell. Immunol., 71:215-223) and concomitantly stimulates the biosynthesis of plasminogen activator inhibitor 2 (PAI 2) and of urokinase-type plasminogen activator (u-PA). PAI 2 is found in a nonglycosylated intracellular and a glycosylated secreted form. The former appears to be identical to PAI 2 previously purified from placental extracts and large-scale U-937 cell cultures. The sixfold increase of PAI 2 antigen measured 24 h after PMA treatment in cell extracts and conditioned media is accompanied by an equal increase of active PAI 2 mRNA, whereas the 6 to 13-fold increase of u-PA antigen in the same samples is associated with only a 1.5-fold mRNA increase. The increase of PAI 2, but not of u-PA, biosynthesis requires transcription. A 50-fold molar excess of PAI 2 over u-PA is found in both extracts and conditioned media of PMA-treated cells. PAI 2 represents at least 0.3% of total de novo synthesized protein 24 h after induction with PMA. Thus, PAI 2, but not u-PA, is an abundant product of this precursor analogue of the mononuclear phagocyte lineage, and might represent a new marker for monocyte/macrophage differentiation.

Fibrinolysis in pregnancy: a study of plasminogen activator inhibitors.

During pregnancy the plasma concentration of two different inhibitors of plasminogen activators (PAIs) increases. The only one found in the plasma of nonpregnant women (PAI1) is immunologically related to a PAI of endothelial cells; its plasma activity, as deduced from the inhibition of single-chain tissue-type plasminogen activator (t-PA), increased from 3.4 +/- 2.3 U/mL (mean +/- 95% confidence limits) in the plasma of nonpregnant women to 29 +/- 7 U/mL at term, and its antigen level, measured by a radioimmunoassay, increased from 54 +/- 17 ng/mL to 144 +/- 25 ng/mL. In pregnancy plasma a second PAI (PAI 2) related to a PAI found in placenta extracts was observed. Its level, quantified with a radioimmunoassay, increased from below the detection limit (approximately 10 ng/mL) in normal plasma to 260 ng/mL at term. One hour after delivery, PAI 1 activities and antigen decreased sharply, but the PAI 2 antigen levels remained constant. Three days later, the PAI 1 antigen levels had fallen to normal levels, but the PAI 2 antigen levels were still at least eightfold above the nonpregnant values. During pregnancy, the t-PA and prourokinase (u-PA) antigen concentrations increased 50% and 200%, respectively, whereas the plasminogen and alpha 2-antiplasmin levels remained constant. Despite the large variations in the levels of PAs and PAIs, the overall fibrinolytic activity as measured in diluted plasma by a radioiodinated fibrin plate assay did not change significantly. Just after delivery, a great increase in the t-PA antigen levels was observed. Three to five days after delivery most parameters of the fibrinolytic system were normal again. Our results demonstrate that during pregnancy and in the puerperium profound alterations of the fibrinolytic system occur that are characterized by increases in PAs and their inhibitors, but these alterations do not affect the overall fibrinolytic activity.

[Carcinoma in mature cystic teratoma of the ovary: pathology, therapeutic and prognostic problems].

Carcinoma develops in about 1-2% of ovarian mature cystic teratomas, usually in postmenopausal women, squamous cell carcinoma being the most common tumor type. This study is based on a review of 300 published and 5 new cases. The carcinomatous tissue tends to invade adjacent organs, whereas lymph node metastasis has rarely been diagnosed. A five-year survival rate of about 15% testifies to the poor prognosis. The treatment of tumors confined to the ovary (stage Ia, Ib; FIGO) usually consists of bilateral oophorectomy and total hysterectomy. Exceptionally, in young women of childbearing age, treatment may be limited to unilateral oophorectomy. A thorough surgical, cytologic and bioptic examination of the abdominal cavity should be done to confirm an early tumor stage. In more advanced stages (FIGO Ic to IV), complete eradication of the malignant tissue offers the only chance of cure. The efficacy of chemo- and radiotherapy has not been studied systematically. We suggest that carcinoma in mature cystic teratoma be staged and treated according to the guidelines established for common epithelial neoplasms of the ovary.

Value of oral provocation tests to aspirin and food additives in the routine investigation of asthma and chronic urticaria.

Nonsteroidal anti-inflammatory drugs and certain food or drug additives are known to induce acute bronchospasms, angioneurotic edema, and urticaria in susceptible patients. Thirty-four patients (17 with asthma and 17 with urticaria), whose case history suggested such intolerance, were challenged orally with increasing doses of seven compounds: acetylsalicylic acid, glafenine, sodium benzoate, sulfur dioxide, potassium sorbate, sodium glutamate, and tartrazine. Among 162 oral provocation tests, 38 were positive (20% decrease in peak flow rate or appearance of acute urticaria/angioneurotic edema). Twenty-four of the 34 patients (nine with asthma and 15 with urticaria) were intolerant to at least one compound. However, no serious reaction was observed. In 20 of these 24 patients (six with asthma and 14 with urticaria), a diet free of additives and nonsteroidal anti-inflammatory drugs resulted, within 5 days, in a marked improvement of symptoms, which persisted 8 to 14 mo after starting the diet. Age, prevalence of IgE-mediated allergy, and nasal polyposis were similar in patients with or without reactions of intolerance. Under the conditions used, oral provocation tests proved to be feasible, safe, and useful in many patients not helped by existing methods.

[Granulosa cell and thecacell tumors in 25 cases (author's transl)].

Granulosa cell tumors are derived from specific ovarion stroma and have the ability to produce hormones. In the 25 cases of granulosa cell tumors no concommitant endometrial carcinoma was found. Because of the varying degree of malignancy tumors which contain granulosa cells must be differentiated from pure theca cell tumors. Granulosa cell tumors are frequently malignant. Theca cell tumors are benign. The treatment of granulosa cell tumors is fundamentally the same as that of epithelial cancers of the ovary. The treatment of theca cell tumors is conservative.

[The adenocarcinoma of the cervix: study of 81 cases (author's transl)].

The present study deals with 81 primary adenocarcinomas of the uterine cervix diagnosed and treated in the departement of gynecology and obstetrics of the University of Zürich from 1950 through 1977. Clinical and morphological data such as patients age, character and duration of symptoms, clinical and anatomical stage, histological differentiation of the tumor and therapy were recorded and analysed in relation to patients survival. Of all these factors the stage of the tumor at time of diagnosis proved to have the highest prognostic significance.

[A Sertoli-Leydig cell tumor as an incidental finding in a 72 year old patient].

In a 72-year old patient a total hysterectomy and bilateral salpingo-oophorectomy were performed because of a proliferating serous cystadenofibroma of the right ovary. A small but typical Sertoli-Leydig cell tumor was incidentaly found in the left ovary. The clinical and histological features of this unusual case are briefly presented.
