Histopathology of experimental Aspergillus fumigatus keratitis.

Histopathological studies in rabbit's eyes, 7 and 14 days after intracorneal inoculation with 1 X 10(5) Aspergillus fumigatus conidia have been performed. Similar lesions were found in both periods with fungal hyphae in the anterior third of corneal stroma, round cell infiltration from the sclero-corneal edge and in the anterior chamber and, neovascularization. No lesions were found in the Descemet's membrane. Gomori silver-methenamine stain with hematoxiline-eosine counter-stain was found to be the most reliable stain to detect fungal presence in corneal stroma, and Masson's trichromic stain in the study of pathological changes in ocular elements.

Single dose treatment of vaginal candidosis: randomised comparison of amorolfine (50 mg and 100 mg) and clotrimazole (500 mg) in patients with vulvovaginal candidosis.

A double blind randomised comparative study of single dose treatment with amorolfine vaginal tablets (50 mg and 100 mg) and clotrimazole 500 mg monodose vaginal tablets (open labelled) was undertaken in patients with vaginal candidosis. Vaginitis was demonstrated by both a positive culture and positive findings on microscopic examination of a vaginal smear as well as by the presence of clinical symptoms. 118 patients seen over a 6 month period were randomly allocated to receive one 50 mg vaginal tablet of amorolfine (regimen A, 40 patients), a 100 mg vaginal tablet of amorolfine (regimen B, 38 patients) or a 500 mg tablet of clotrimazole (regimen C, 40 patients). At the assessment one week after the end of therapy the proportion of cured patients was 90% in group A, 94.7% in group B and 92.5% in group C. 4 patients (10%) in group A, 2 (5.2%) in group B and 3 (7.5%) in group C did not respond to the treatment. There was a significant association between Candida glabrata and treatment failure (P less than 0.001) and C. glabrata and carrier state (P less than 0.01). At the assessment 4 weeks after the end of therapy the proportion of cured patients was 80% in group A, 84.2% in group B and 67.5% in group C with a relapse rate of 10% (group A), 10.5% (group B) and 25% (group C). C. glabrata was significantly associated with non-effective overall treatment (P less than 0.05). The relapse rate was significantly associated with positive culture results one week post therapy (P less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

Keratomycosis due to Scedosporium apiospermum.

Although no overt injury occurred, a corneal abscess without hypopyon developed in a normal, 25-year-old female, living in a suburban area of Madrid, following a period of intensive garden work. Direct examination of the corneal scrapings on one occasion showed septate, branched hyphal elements. When scrapings were cultured on Sabouraud glucose and brain heart infusion agars a pure culture of Scedosporium apiospermum was repeatedly isolated. The patient responded successfully to treatment with miconazole ointment, although a residual immune ring of the cornea remained and final visual acuity was 6/10.

In vitro interaction between ofloxacin and cefotaxime against gram-positive and gram-negative bacteria involved in serious infections.

The checkerboard method was used to determine the antimicrobial activity of the combination cefotaxime/ofloxacin against 217 bacterial isolates involved in serious infections. Synergy or partial synergy was observed against 19 of 34 (55.8%) Staphylococcus aureus methicillin-susceptible isolates, 4 of 47 (8.4%) Streptococcus pneumoniae isolates, 28 of 34 (82.2%) Escherichia coli isolates and 70 of 102 (68.5%) Pseudomonas aeruginosa isolates. Antagonism was not observed with any of the isolates examined.

Meropenem permeation through the outer membrane of Pseudomonas aeruginosa can involve pathways other than the OprD porin channel.

The outer membrane protein (OMP) OprD is the major channel through which carbapenems permeate the outer membrane of Pseudomonas aeruginosa. In this study, we analyzed the OMP profiles of several P. aeruginosa clinical isolates showing diminished susceptibility to imipenem while remaining susceptible to meropenem. All these isolates lacked OprD or showed a reduced expression of this porin. Susceptibility to meropenem was thus independent of the level of OprD expression, indicating that the antimicrobial could be taken up via an alternative route. The level of expression of OprC (70 kD) was also unrelated to meropenem susceptibility. Nevertheless, OMPs OprF and OprE were expressed by all isolates, suggesting that in the absence of OprD, these porins might be involved in the permeation of meropenem.

Drug Resistance, Microbial↗

Susceptibility of Pseudomonas aeruginosa isolates to ceftazidime is unrelated to the expression of the outer membrane protein OprC.

Previously, it has been postulated that the porin OprC facilitates the diffusion of ceftazidime through the outer membrane of Pseudomonas aeruginosa. To further investigate this claim, the outer membrane protein (OMP) profiles of 22 ceftazidime-susceptible clinical isolates were analyzed. No correlation was found between MIC values and the level of expression of OprC. Further, OprC was either undetectable or expressed in reduced amounts in 12 isolates. In contrast, OprF and OprE were present in all isolates studied. This study suggests that OprC is dispensable for the permeation of ceftazidime through the outer membrane of P. aeruginosa.

Augmented repair of Achilles tendon ruptures.

We performed a series of repairs of Achilles tendon ruptures in athletes using suture augmented with a polyethylene terephthalate mesh synthetic graft. No postoperative immobilization was used. The 29 patients were aged 25 to 54 years and all practiced sports a minimum of 4 hours weekly. At the followup, ranging from 6 months to 5 years, ankle mobility was normal in all but one patient, who had lost 15 degrees of extension. All the patients were able to do a series of 10 single-limb heel raises without any impairment when compared with the uninjured side. Average ankle flexion strength was 96% that of the uninjured side (range, 87% to 110%). There were no reruptures, although in two instances problems with scarring occurred. All patients but one were able to resume their sports activities at the same level of intensity as they had before their injuries. These results testify to the advantage of not using postoperative immobilization and beginning functional rehabilitation immediately in patients with such lesions.

[HIV infection in an autonomous Spanish community (La Rioja). Features, trends and conclusions applicable to other regions].

BACKGROUND: The epidemiologic characteristics of HIV infection in La Rioja (Spain) were studied and the future tendencies of the same analyzed. METHODS: The data from official Health sources were collected and analyzed. RESULTS: In La Rioja, the 5th region in Spain regarding to AIDS incidence, the duplication time of the number of cases continues to shorten. The number in December 1991 was 81. The heterosexual route of infection was 5% of the cases of AIDS up to December 1990 and in 1991 it was 15%, while, with regards to the total number of seropositive subjects, the number rose from 0.3% in December 1990 to 10% in 1991. One thousand one hundred fifty-one seropositive patients were detected in December 1991 representing seroprevalence of 0.34% which reaches 1.14% in the age range between 18-32 years. Mean hospital stay was 23.6 days with 3,032 hospital stays calculated for 1991 representing 8.3 occupied beds/day. There are presently 92 patients undergoing antiviral treatment. Between 43-69 new cases of AIDS have been calculated for 1992. CONCLUSIONS: 1) Medical care should be provided to a greater number of seropositive people to avoid early deterioration. 2) The increase in heterosexual infection is alarming. 3) The need for rehabilitation centers for drug abusers and social alternatives to prostitution is clear. 4) Organized campaigns pointing out and returning the dignity and value of life and therewith the role of human sexuality are needed. 5) Alternative means to conventional hospitalization is required.

[Activity of erythromycin and clarithromicin against isolates of Streptococcus pneumoniae with decreased sensitivity to penicillin obtained from healty carriers. Spanish Collaborative Group].

BACKGROUND: Empirical treatment of pneumococcal upper and lower respiratory infections must be chosen on the basis of the susceptibility patterns of nasopharyngeal colonizing strains isolated from healthy carriers. METHODS: The susceptibility to erythromycin and clarithromycin was investigated by a conventional microdilution method among 103 pneumococci isolates recovered from healthy children (n = 63) and adults (n = 40) exhibiting decreased susceptibility to penicillin (MIC > or = 0.12 mg/l). RESULTS: 63% of penicillin -resistant pediatric isolates were susceptible to erythromycin and 78.9% were susceptible to clarithromycin . Among isolates with diminished susceptibility to penicillin , 77.7% were susceptible to erythromycin and 86.3% to clarithromycin . 90% of adult isolates were susceptible to erythromycin and to clarithromycin . Overall, clarithromycin exhibited a better activity than erythromycin . CONCLUSIONS: Clarithromycin , and to a lesser extent erythromycin , are good alternatives to penicillin in the empirical treatment of respiratory tract infections caused by pneumococci with diminished susceptibility to penicillin .
