Calcitonin and reflex sympathetic dystrophy syndrome.

Reflex sympathetic dystrophy syndrome is a difficult condition to treat. Many modalities have been proposed, all of them being clinically effective but whose efficacy is often difficult to assess, and has not been properly compared. A regimen of physical therapy (pressure therapy, antalgic electrotherapy and exercise therapy) with or without calcitonin was investigated in 24 patients randomly assigned to 2 groups, one given physical therapy alone (Group I) and the other physical therapy plus salmon calcitonin 100 MRC units daily for 3 weeks (Group II). Efficacy assessment was based on clinical (pain, oedema and movement in the affected parts), biochemical (blood and urinary phosphorus and calcium levels, plasma 25-OH-D, plasma parathyroid hormone (PTH), creatinin, alkaline phosphatases and urinary hydroxyproline) and scintigraphic parameters, as well as on the patient's ability to resume working. There was significant improvement in pain in the Group II patients after one week of treatment. As a result the authors advocate the use of calcitonin in addition to physical therapy in reflex sympathetic dystrophy syndrome - and even of calcitonin alone where physical therapy is not possible.

Synovial fluid lactate levels in septic and non-septic arthritides.

Lactate concentration was studied in 383 synovial fluid specimens from patients with various arthritides. The highest concentrations of lactate occurred in non-gonococcal septic synovial fluids. High values were recorded in seropositive rheumatoid arthritis and crystal-induced arthritides, medium values in synovial fluids from seronegative rheumatoid arthritis, seronegative spondylarthritides, gonococcal arthritis and haemarthrosis, and the lowest values in aspirates from osteoarthrotic joints. There was a positive correlation between synovial pH and lactic acid concentration. These data suggest that determination of lactate in synovial fluid can be valuable in the rapid exclusion of septic arthritis. Its value for differentiating between other inflammatory arthritides is discussed.

A controlled study of the exocrine pancreatic function in Sjögren's syndrome and rheumatoid arthritis.

The exocrine pancreatic function has been estimated using the N-benzoyl-L-Tyrosyl para-amino-benzoic acid test (NBT PABA test) and by measuring the trypsinemia by radio-immunoassay (RIA trypsinemia) in nonselected cases of Sjögren's syndrome (SS) and seropositive rheumatoid arthritis (RA) in comparison with normal controls. The NBT PABA test was pathological in 37,5% of SS and 35% of RA patients but in none of the controls. The RIA trypsinemia was found to be high in 6,2% of SS and 45% of RA patients. These findings suggest that exocrine pancreatic function is impaired in some SS and RA cases. However, this exocrine pancreatic defect was clinically silent in all patients.
