Many substances in the plant kingdom and in man's diet occur as glycosides. Recent studies have indicated that many glycosides that are not mutagenic in tests such as the Salmonella test become mutagenic upon hydrolysis of the glycosidic linkages. The Salmonella test utilizes a liver homogenate to approximate mammalian metabolism but does not provide a source of the enzymes present in intestinal bacterial flora that hydrolyze the wide variety of glycosides present in nature. We describe a stable cell-free extract of human feces, fecalase, which is shown to contain various glycosidases that allow the in vitro activation of many natural glycosides to mutagens in the Salmonella/liver homogenate test. Many beverages, such as red wine (but apparently not white wine) and tea, contain glycosides of the mutagne quercetin. Red wine, red grape juice, and tea were mutagenic in the test when fecalase was added, and red wine contained considerable direct mutagenic activity in the absence of fecalase. The implications of quercetin mutagenicity and carcinogenicity are discussed.
Twelve female patients undergoing intermittent hemodialysis (HD) and 5 females posttransplantation (PT) were studied. All the HD patients had menstrual disturbances and 5 had galactorrhea. The mean basal LH level was significantly elevated (p less than .05) in patients on HD compared to normal controls, but the mean LH response to luteinizing hormone releasing hormone (LRH) was not significantly different from the control group. Mean basal FSH and the FSH response to LRH was normal. In the PT pateints the LH response to LRH was significantly greater at 120 min when compared to normal females. In the HD group the serum 17B estradiol, progesterone and testosterone levels were significantly lower than in the controls but in the PT group only testosterone levels were significantly lower. These results differ from those previously found in uremic males. Elevated prolactin levels were found in the patients on hemodialysis and correlated well with the presence of galactorrhea. These was no correlation between the elevated prolactin levels and amenorrhea in the patients on hemodialysis but one PT patient with amenorrhea had elevated prolactin levels.

The growing movement toward specialty certification in nursing has resulted in an increased demand for continuing education that supports specific areas of specialty practice and certification status. In examining a number of the specialty practice core curricula documents, it is evident that there is a tendency to organize nursing content using the medical model as a framework. A method by which core curricula for specialty practice are organized and developed from a distinctly nursing perspective is needed. The authors suggest that conceptual frameworks, developed by nurses who are expert in a specific clinical practice area, are a practical method for the development of such curricula.
It is important to examine the role of supervision in facilitating change and promoting the success of care delivery models using UAP in ambulatory care. Good supervision is a role expectation of all professional nurses, and is especially important in ambulatory settings where patient care objectives and patient diversity and complexity may not be well understood by UAP. The ramifications for nurse executives and managers include ensuring that organized opportunities to learn and be supported in supervision are part of the development of every staff nurse in the ambulatory setting. Sufficient time must be allocated in staffing models so that staff nurses can supervise appropriately. Nurse executives and managers must also assist staff nurses in finding rewards and satisfaction in supervision. Research must be designed to provide a database to assist in decisions about the optimal numbers of UAP that can be effectively used in ambulatory models without compromising patient outcomes. Research is also needed regarding the optimal number of UAP that can be effectively supervised by one staff nurse.
