Heterologous expression and enzymatic properties of a selenium-independent glutathione peroxidase from the parasitic nematode Brugia pahangi.

A full-length cDNA from the parasitic nematode Brugia pahangi encoding a secreted homolog of glutathione peroxidase in which the codon for the active site selenocysteine is substituted naturally by a cysteine codon has been expressed in Spodoptera frugiperda (insect) cells via Autographa californica nuclear polyhedrosis virus (baculovirus). The recombinant protein was glycosylated and secreted from the cells in tetrameric form. The purified protein showed glutathione peroxidase activity with a range of organic hydroperoxides, including L-alpha-phosphatidylcholine hydroperoxide, but no significant activity against hydrogen peroxide. Glutathione was the only thiol tested that served as a substrate for the enzyme, which showed no activity with the thioredoxin system (thioredoxin, thioredoxin reductase, and NADPH). No glutathione-conjugating activity was detected against a range of electrophilic compounds that are common substrates for glutathione S-transferases. The apparent (pseudo)m for glutathione was determined as 4.9 mM at a fixed concentration of linolenic acid hydroperoxide (3 microM). The enzyme showed low affinity for hydroperoxide substrates (apparent Km for linolenic acid hydroperoxide and L-alpha-phosphatidylcholine hydroperoxide of 3.8 and 9.7 mM, respectively at a fixed glutathione concentration of 3 mM).

Registration health checks: inverse care in the inner city?

BACKGROUND: Attendance at health checks of patients already registered with a general practitioner is known to be poor, with those in need least likely to attend. Little is known of the efficacy of such checks for newly registered patients. AIM: This study set out to determine the characteristics of attenders and non-attenders at health checks for patients registering with a general practitioner in east London, and the effect of health checks on motivation to change unhealthy lifestyles. METHOD: A questionnaire analysis was carried out of patients aged 16 years and over at registration and after a health check in seven east London training practices. Questionnaires asked about personal background and lifestyle including smoking status, alcohol intake, diet, weight, exercise, cervical smear uptake, and motivation to change unhealthy aspects of lifestyle. RESULTS: Questionnaires were offered to 356 patients registering with the practices, of whom 101 declined or returned inadequate data. Of the remaining 255 patients, 118 (46%) attended a health check with 113 completing a second questionnaire after the check. Non-attenders were significantly more likely than attenders to be of lower social class, unemployed, of African origin and to be heavy smokers. Women who did not attend were significantly less likely than attenders to have had a cervical smear within the last three years. Non-attending mothers were significantly more likely than attending mothers to be single parents. Motivation among attenders to stop smoking and drink less alcohol was increased significantly after the health check. CONCLUSION: Attendance at registration health checks at these practices was poor and non-attenders tended to be more socially deprived than attenders and had relatively unhealthy lifestyles. Although the health checks increased the attenders' motivation to alter smoking and drinking habits, inviting all new patients to a health check would appear to result in poor targeting of health promotion resources and may widen inequalities in health.

Acute colonic pseudo-obstruction: a pharmacological approach.

Acute colonic pseudo-obstruction is a functional disorder that closely mimics mechanical large bowel obstruction, and in which inadvertent laparotomy carries a high mortality. Eleven such patients were treated by pharmacological manipulation of the autonomic innervation to the colon with guanethidine and neostigmine. Eight responded to treatment with passage of flatus and/or stool within 10 min with complete resolution of symptoms. In three patients the treatment failed. Postural hypotension occurred in only one patient and no other serious side-effect was apparent. This pharmacological approach to the management of acute colonic pseudo-obstruction is suggested as an alternative to the other treatment options of colonoscopic decompression or surgery, when conservative management has failed.

Trauma care in a military hospital.

The results of a quinquennial audit of trauma care in the Cambridge Military Hospital using the TRISS method is presented. 113 cases were analysed. The mean age was 32 years and the mean Injury Severity Score 16. Twenty-five deaths occurred of which 12 were potentially preventable. Means of reducing the number of preventable deaths are discussed.

Public knowledge and attitudes to AIDS.

Three independent cross-sectional surveys of public knowledge and attitudes about AIDS were conducted on a representative sample of people aged 15-54 resident in Wales. 1,303 were interviewed in their homes in February 1987, 683 in September 1987 and 676 in March 1988. The results show that most people knew that having sexual intercourse or sharing needles with people with AIDS represented a high risk of catching AIDS. However there appeared to be considerable misunderstanding about the nature of HIV infection such that one in three thought that a man and woman with a single heterosexual partner was at high or moderate risk of catching AIDS. The high level of concern coupled with considerable confusion appears to have contributed to both unnecessary anxiety and prejudice. One in four people thought that kissing or being spat on by a person with AIDS represented a high or moderate risk, and one in six thought that coughing, sneezing and sharing towels, soap, drinking utensils or lavatory seats were routes of transmission. More than 50% of men and 40% of women said that it was their own fault if homosexuals and drug addicts got AIDS and did not feel sorry for them. It is concluded that action to date to limit the spread of AIDS has informed but not yet adequately educated the public. If unwanted anxieties and prejudice are to be diminished, initiatives by government, health services and others must now concentrate on developing understanding about the nature of HIV infection and its spread through more personal education.(ABSTRACT TRUNCATED AT 250 WORDS)

Simple methods for denture identification: a clinical trial.

The persistence of identification marks applied to dentures with two spirit solvent based fibre tip pens and graphite pencil covered with an experimental and a proprietary sealant were studied under service conditions. In general, the marks applied with the pens were rapidly lost, but those covered with the sealants persisted satisfactorily. The experimental sealant performed slightly better than the proprietary product.

Effect of phencyclidine on the metabolism of individual brain proteins.

The effect of phencyclidine on the metabolism of a selected number of rat brain proteins was determined using two-dimensional gel electrophoresis and quantitative fluorography. When rats were injected with phencyclidine, modulation of individual protein metabolism occurred in the pituitary and cortex. That is, a few proteins showed increased and others decreased incorporation of [35S]methionine, whereas total protein metabolism was unaltered. In contrast, in vitro treatment of brain tissue with phencyclidine inhibited incorporation of radiolabel into all proteins by approximately 50%, as shown by quantitative fluorography of individual proteins.

Cutaneous adverse reactions to acyclovir: case reports.

Intravenous, oral, and topical formulations of acyclovir have been successful in treating genital herpes. We report on two patients who developed skin reactions while taking acyclovir, which resolved when treatment was stopped.

AIDS--a health education approach in West Glamorgan.

An education programme on AIDS has been established in West Glamorgan. It was designed to provide an information network amongst health authority staff which would reach as wide an audience as possible. The programme has been organised on a direct contact approach because of the limitations of other methods of disseminating information to large groups of people. The initial success of the programme has led to its extension among other interested groups in West Glamorgan such as the prison population, drug groups, and members of the gay community. The programme is to continue and be updated on a regular basis.
