Serum erythropoietin in cross-country skiers.

Serum erythropoietin (Epo) activity, hemoglobin (Hb), and hematocrit (Ht) were determined in 21 cross-country skiers during the training season. The Epo levels were not significantly reduced in the skiers relative to the normal population (P less than 0.01 and P less than 0.001, respectively). In 11 athletes Epo, Ht, urinary gamma-glutamyltransferase, N-acetyl-beta-glucosaminidase, and microalbuminuria were determined before and after a 50-km ski race at 1600 m above sea level. A significant increase of these variables (except for Ht) was found after the competition (P less than 0.001). It is concluded that while the reductions in Hb and Ht, which are typical of several endurance exercises, are not accompanied by a renal hypoxia sufficient to stimulate Epo overproduction, the renal hypoxia reached during the strenuous exercise of the race at altitude may be effective in determining blood increases in Epo.

Reduction of erythrocyte magnesium concentration in heterozygote beta-thalassaemic subjects and in normal subjects submitted to physical stress.

Erythrocyte and serum magnesium (Mg) concentrations have been assayed in a group of sedentary heterozygote beta-thalassaemic subjects (beta-thal), in a group of non-thalassaemic well trained runners before and after a 25 km running race, and in a group of sedentary healthy controls. The mean erythrocyte Mg concentration (EMg) found in beta-thal (2.72 mEq/litre) and in runners, both before and after the race (2.58 mEq/litre before, 3.10 after), was significantly lower than the EMg values from the control group (3.69 mEq/litre). We propose various hypotheses to explain the reductions observed.

Unmodified performance in runners following anabolic steroid administration.

The effect of a low-dose treatment (10 mg/day) of stanozolol on the anaerobic threshold (AT) and on maximal velocity (Vmax) was studied in ten well-trained runners in a 6-week trial experiment. The serum levels of testosterone (T), interstitial cell-stimulating hormone (ICSH), and of follicle-stimulating hormone (FSH) were determined before, during, and after the steroid administration. No improvements of AT and Vmax were found. No improvement of the competition running times was observed during the experimental period. The ICSH and FSH serum concentrations did not change significantly, while a marked decrease of the T serum concentration was observed. It is concluded that a low-dose treatment of stanozolol, sufficient to depress the testicular function, does not improve athletic performance.

Variations of serum testosterone concentrations after physical exercises of different duration.

Serum testosterone concentration was determined before and after physical activities of different duration. The subjects under study were: (1) 7 competitive walkers before and after a 20-km race (average race time 1 h 30 min); (2) 9 middle-distance runners before and after 1-h training; (3) 16 marathon runners before and after a marathon run (average running time 2 h 33 min); (4) 30 ultramarathon runners before and after a 107-km race (average running time 14 h). Serum testosterone increased by 51.8% (NS) in competitive walkers, by 38.2% (P less than 0.05) in middle-distance runners, and by 44.9% (P less than 0.01) in marathon runners; it decreased by 31.9% (P less than 0.001) in the ultramarathon runners. These findings, and similar variations of serum testosterone observed in an ultramarathon runner during a 6-h training session, suggest that serum testosterone increases during physical activities lasting up to 3 h and decreases to or even below the pre-exercise values for longer physical efforts. The possible mechanisms responsible for these differences are discussed.

[Relation between the duration of exertion and levels of serum testosterone].

The serum testosterone levels have been shown to be increased following middle- and long distance competitions (up to 42,195 km). The hormone serum levels have been now studied in 63 runners before and after a 107 km race and in an ultramarathon runner before, during and after a 6 hours race. After the 107 km race the mean hormone levels have been found to be reduced from 9,72 to 7,45 ng/ml for the age class 20-29 years; from 6,57 to 4,69 ng/ml for the group 30-39; from 6,22 to 5,13 ng/ml for the group 40-49 years; from 7,56 to 5,46 ng/ml for the age class 50-59. In the ultramarathon runner the testosterone levels sharply rose at the beginning of the training session (from the 7,20 ng/ml to 11,50 ng/ml after 20' training), and subsequently decrease after 3 hours (9,20 ng/ml) and at the end of the 6 hours training (4,60 ng/ml). Based on these and on previous findings the Authors hypothesize that a prolonged exertion produces an initial stimulation of the hormone producing system, followed by its functional exhaustion.

[Blood testosterone levels before and after races of various duration].

The blood testosterone levels have been measured in nine middle-distance runners seven walkers and in eighteen marathon runners before and after races of different duration. Most of the athletes (24 out of 34, 70,6% of the population) have shown an increase in testosterone levels soon after physical exercise. The values varied from an average of 5,83 to 7,72 ng/ml in middle distance runners, from 4,70 to 6,70 ng/ml in walkers, and from 5,00 to 6,82 ng/ml in marathon runners. The Authors hypothesize that prolonged physical activity in well-trained athletes may determine an augmented incretion of testosterone.

Decrease of alpha-Hasharon globin in beta-thalassaemia.

In time course experiments performed in subjects carrying haemoglobin Hasharon and beta-thalassaemia the synthesis of alphaA-, alphaHasharon-, beta- and gamma-globin has been determined. The alpha-globin molecules synthesized in excess were found to be removed from the red cell cytoplasm (phenomenon characteristic of beta-thalassaemia, described by Bargellesi et al, 1968a); in addition the alphaHasharon-globin molecules were removed at a faster rate than normal alpha-globin. The preferential removal of the mutant alpha-globin explains the reduced level of haemoglobin Hasharon found in subjects carrying the gene for beta-thalassaemia (Alberti et al, 1975). Reductions below normal levels of other haemoglobin variants occurring in the presence of the counterpart-globin thalassaemia are probably determined through a similar mechanism.

Pulsed wave-Doppler ultrasonographic evaluation of the common carotid artery in the resting horse: physiologic data.

A pulsed wave-Doppler ultrasonographic evaluation of common carotid arterial blood flow was carried out on 63 healthy Italian Saddlebred horses. Vessel diameter and tracing morphology were evaluated and blood flow parameters (systolic, diastolic and mean velocity, acceleration and deceleration of the systolic wave, carotid pulse volume) were calculated and correlated with class variables (sex, age and body weight). On the basis of the presence of an incisure in the ascending branch of the systolic curve, subjects were divided in two groups: one with a bifid systolic curve and the other with a monophasic aspect. Correlations between: 1) diameter of the vessel and body weight and 2) carotid pulse volume and flow velocity (systolic, diastolic and mean) were found. A greater systolic pulse volume was found in male subjects, in subjects with greater body weight and in those which had a monophasic systolic wave.

Introducing quality assurance in radiotherapy.

Introducing a Quality Assurance methodology appears particularly useful in Radiation Oncology due to the complexity of the procedures involved and the heterogeneity of the standards adopted, if any, in the great majority of the Centers. There are two possible ways of evaluating quality in the Health Environment: a formal, Institutional certification, or a voluntary one obtained through a mechanism of peer review. The European Society for Therapeutic Radiology and Oncology (ESTRO) started in 1994 with the publication of a methodological Report intended to be adopted by the individual national Societies, and this paper is an invitation to do it.

Aorticopulmonary septal defect in a German shepherd dog.

A 2-year-old, female German shepherd dog was referred with a 2-month history of dyspnea, ascites, and anorexia. On physical examination, an arrhythmia, tachypnea, and a continuous grade V/VI heart murmur were evidenced. Atrial fibrillation and tall R waves were noted on electrocardiography. Survey thoracic radiography revealed generalized cardiomegaly and enlarged pulmonary vessels. Echocardiography demonstrated severe dilatation of the left atrium and ventricle with reduced fractional shortening. Doppler echocardiography showed continuous and turbulent flow in the ascending aorta between the aorta and the pulmonary artery. Necropsy demonstrated an aorticopulmonary septal defect.

Serum and erythrocyte levels of magnesium in microcytosis: comparison between heterozygous beta-thalassemia and sideropenic anemia.

In thalassemia erythrocyte cation permeability is increased, but the increment in ATPase-dependent cation pumps maintains normal concentrations of Ca++, Na+ and K+. In this study we investigated erythrocyte concentrations of Mg++ in heterozygous beta-thalassemia and in microcytic sideropenic anemia. Twenty-five healthy controls, 40 heterozygous beta-thalassemics and 25 patients with sideropenic anemia were studied. Erythrocyte Mg++ was assayed either by atomic absorption or by standard laboratory methods. Erythrocyte Mg++ was significantly lower in the beta-thalassemia group than in the other two groups (p less than 0.001). Serum magnesium was significantly lower in sideropenic anemia patients than in beta-thalassemics and in controls (p less than 0.01), whereas these latter two groups showed similar values. Our results suggest that the increment in ATPase-dependent cation pumps is not sufficient to maintain normal erythrocyte Mg++ concentrations in heterozygous beta-thalassemia. In sideropenic anemia cation permeability is not increased, therefore erythrocyte Mg++ is normal. Low serum Mg++ levels in sideropenic anemia could be explained by a primary Mg++ deficit associated with sideropenia.
