Correlation of age with the status of hepatitis B virus replication and histological changes in chronic type B hepatitis.

The interrelationships of age, hepatitis B virus (HBV)-DNA, hepatitis B e antigen (HBeAg)/antibody (anti-HBe) and hepatic histological changes were examined in a large series of patients with chronic type B hepatitis. A significant inverse relationship between serum levels of HBV-DNA and the age of patients was demonstrated:a high level of HBV-DNA (greater than 500 pg/ml) was found in 100%, 50%, 34%, 10% and 0% of patients in the second, third, fourth, fifth and sixth decade, respectively (P less than 0.001), whereas serum HBV-DNA was negative in 0%, 6%, 8%, 40%, and 50% of cases in the respective decades (P less than 0.001). The correlation of HBV-DNA with age was more pronounced in the HBeAg-negative cases, of whom 81.3% (13/16) of patients below age 40 were positive for HBV-DNA, in contrast to 33.3% (3/9) of those over 40 years (P less than 0.05). In addition, the mean age increased from 25.3 years for HBeAg-positive patients with chronic persistent hepatitis (CPH) and nonspecific reactive histological changes (NSRH), to 32.7 years for HBeAg-positive and 34.9 years for anti-HBe positive patients with chronic active hepatitis and chronic lobular hepatitis (P less than 0.001), and 40.9 years for anti-HBe positive CPH and NSRH (P less than 0.001). These data suggested that age correlated closely with the status of HBV replication and hepatic changes in chronic type B hepatitis.

The activity of 3-hydroxyacyl-CoA epimerase is insufficient to account for the rate of linoleate oxidation in rat heart mitochondria. Evidence for a modified pathway of linoleate degradation.

The activities of cis-delta 3-trans-delta 2-enoyl-CoA isomerase (EC 5.3.3.8), 3-hydroxyacyl-CoA epimerase (EC 5.1.2.3), and 2,4-dienoyl-CoA reductase, all of which have been proposed to function as auxiliary enzymes in the beta-oxidation of polyunsaturated fatty acids, have been determined in mitochondria from rat heart and rat liver. In heart mitochondria the activity of 3-hydroxyacyl-CoA epimerase was lower, whereas that of 2,4-dineoyl-CoA reductase was higher than the observed rate of linoleate degration. This observation suggests that 2,4-dienoyl-CoA reductase and not 3-hydroxyacyl-CoA epimerase functions as an auxiliary enzyme in the metabolism of polyunsaturated fatty acids in heart. A modified pathway of linoleate degradation is presented.

DNA ploidy and biologic aggressiveness of gastric adenocarcinoma in Chinese.

The mortality rate of gastric cancer in the Chinese population has reached a plateau. The main prognostic factor for gastric adenocarcinoma is recognized as tumor stage. Recently, abnormalities in DNA content have been considered as a new prognostic factor. Whether abnormal DNA content can be used as a prognostic tool for Chinese patients with gastric cancer is unknown. To investigate this relation DNA ploidy and prognosis of gastric cancer patients were studied using paraffin-embedded specimens. A group of 104 newly diagnosed and surgically resected gastric cancer specimens obtained from January 1984 to December 1986 were examined for DNA content by flow cytometry. The quality of flow cytometry was acceptable with a mean coefficient of variance of 5.45. The results showed that 38 cases (36.5%) had DNA aneuploidy; 42 cases had metastatic lymph nodes with enough tumor cells, and 31.0% of these cases had DNA aneuploidy. DNA aneuploidy of primary tumors was correlated to lymph node metastasis and patient's age, whereas DNA aneuploidy of metastatic lymph nodes was significantly correlated to the serosal invasion of the gastric wall at the primary site. The important parameters for prognosis were curability of surgical resection, serosal invasion, tumor size, and distant metastasis. DNA aneuploidy of both primary tumors or metastatic lymph nodes appeared to be unrelated to the prognosis of gastric adenocarcinoma in Chinese patients. We therefore propose that DNA aneuploidy of gastric cancer is associated with tumor growth but not biologic aggressiveness.

Treatment of early childhood caries: a review and case report.

Caries in infants and young children long has been recognized as a clinical entity and described variously as nursing caries, nursing bottle syndrome and so on. Although prolonged use of the baby bottle is believed to be associated with increased risk of caries, use of the bottle may not be the most important factor in caries development. The term early childhood caries (ECC) has been suggested because it more closely reflects the multifactorial etiologic process involved in the disease. A case of ECC treated in a general dental practice and managed under local anesthesia is reported.

Esthetic designs of removable partial dentures.

The increased emphasis on physical appearance in contemporary society has increased the demand for esthetic dental restorations. Although the success of implant dentistry has expanded the scope of esthetic fixed prostheses, many patients demand a removable partial denture (RPD) for health, anatomic, psychological, or financial reasons. Fabricating an esthetically pleasing RPD while avoiding the unsightly display associated with conventional clasp assemblies often presents a challenge to dentists. This article examines using lingual clasps, proximal undercuts (also known as rotational path insertion), and acetal resin clasps as simple and effective means of improving RPD esthetics.

A review of sodium fluoride varnish.

Fluoride varnish was incorporated into clinical dentistry to reduce caries; its use in the U.S. has increased progressively since it was approved by the FDA in 1994. This paper reviewed clinical studies and found that fluoride varnish's effectiveness in caries prevention, ease of application, and safety give it an advantage over other types of topical fluoride treatments (such as gels and rinses) or other caries management methods. As a result, it is regarded as one of the superior topical fluoride agents for young children.
