[HMPAO-SPECT during carotid artery compression (Matas test) in the evaluation of cervical tumor surgery with potential carotid involvement].

For planning the surgical approach in patients with head and neck tumours and extensive cervical metastasis the evaluation of possible vascular involvement is necessary. If the carotid artery is suspected to be infiltrated by the tumour masses its resection or plastic substitute becomes sometimes inevitable. For evaluation of the collateral brain perfusion before a possibly necessary carotid resection we examined 10 patients with neck tumours performing the Matas-Test combined with HMPAO Single-Photon-Emission-Computed-Tomography. During ultrasound-controlled manual compression of the involved common carotid artery 99mTc-labelled HMPAO was injected. The SPECT demonstrated the brain perfusion during the carotid compression time period. 6 patients showed a decreased perfusion of the affected hemisphere with - as far as obtained - normal baseline SPECT-results and balloon occlusion tests. 2 of these patients underwent a carotid artery resection, one of the common carotid artery and one of the internal carotid artery. One patient had a significant perfusion deficit in the SPECT but no pathologic findings in angiography and balloon occlusion test. He suffered from hemiparesis postoperatively. The other patient showed preoperatively a minor decrease in perfusion and had no neurological defects postoperatively. Our findings show that the Matas-Test combined with HMPAO-SPECT is a valuable tool for the preoperative evaluation of the collateral brain perfusion. This technique is not invasive and can be performed in every nuclear medicine department with an ECT camera. The HMPAO-SPECT during carotid compression can add useful information or even replace angiography and the balloon occlusion test.

[Regression of cerebral post-transplantation lymphoma under cyclosporin A reduction].

4.5 months after successful kidney transplantation a Non-Hodgkin-lymphoma with polymorph centroblastic appearance of the tonsillar gland developed in a 21 years old male patient during immunosuppressive therapy with cyclosporine A and prednisone parallel to infection with Epstein Barr virus. Focal epileptic seizures occurred and were due to cerebral posttransplantation lymphomas as proven by brain biopsy. Reduction of immunosuppressive therapy led to complete remission as shown by CCT and MRI.

Vascularization of the myocutaneous latissimus dorsi flap. Injection study on the thoracodorsal artery.

This study reports on investigations of the thoracodorsal artery by injection studies. This artery has a long proximal extramuscular course before it enters the muscle. A consistent neurovascular hilum was found at a considerably constant location on the inferior muscle surface, 2-3 cm medial to its lateral edge and about 5 cm distal to the inferior scapular border. A constant branching pattern of the thoracodorsal artery into a lateral and a medial vessel stem was found. A high number of muscle-perforating arteries from a dense network in the cutaneous and subcutaneous layer. The thoracodorsal artery supplies the whole cutaneous area adjacent to the latissimus dorsi muscle plus a streak of about 2 cm at the medial and distal muscle borders. The presented anatomical landmarks are useful for locating the neurovascular hilum, and the intramuscular course of the thoracodorsal artery for exploration of the vessel by Doppler sonography or dissection. The length of the pedicle and the relatively big vessel gauge are good anatomical markers for the free transfer of a latissimus dorsi flap.

Myocutaneous vascular territory of the thoracoacromial artery. A topographical and morphometric study of the arterial vascularization of the pectoralis major myocutaneous flap.

In order to clarify the vascularization of the pectoralis major muscle by the pectoral branch of the thoracoacromial artery, morphometric investigations and selective injection studies of the vessel were performed. A comparison with the anatomical orientation points clinically used for locating the vessel and its course confirms that the origin of the artery is at a constant spot at half the sternoacromial distance; the main vessel axis, however, deviates considerably from the clinically used acromioxiphoid axis in lateral direction. The lumen of the thoracoacromial artery has a range of more than 150% of the smallest vessel. Atherosclerotic alterations are not the reason for this variety but only an additional handicap for the vessel capacity. Two types of muscle-perforating arteries were found, and the cutaneous area of blood supply was defined.

Trigeminal and olfactory synergism in the perception of smell.

In normal subjects olfactory evoked cortical potentials are seen with latencies of 150 m/sec and 350 m/sec. To evaluate the site of potential generators in central nervous pathways olfactory evoked cortical potentials were registered in patients with well-defined lesions of the central nervous system and the maxillary nerve. Absence of the first potential was seen in lesions of the brain-stem and the maxillary nerve. Absence of the second potential was observed in lesions of the frontal cerebral medulla and the temporal cortex. No olfactory potentials were found in circumscribed lesions of the postcentral cortex. Sites of the lesions were controlled by computed tomography. Appearance of the two potentials is correlated to normal perception of smell which is established by combined olfactory and trigeminal nerve function.

[B-mode sonography of the neck. Comparison of results of sonography, computer tomography and pathomorphology].

High-resolution B-scan sonography has so far been hardly utilized in the investigation of neck tumours. The results of B-scan sonography, computed tomography, and anatomic pathology are compared and discussed. Lymph node tumours of the neck are easily and precisely recognized by ultrasonography. The rate of false positive tumours diagnoses is higher in B-scan sonography than by computed tomography. The rate of false negative diagnoses is the same with both methods. The technical development of B-scan sonography will widen its application in E.N.T. surgery.

[Origin of olfactory and rhinosensory evoked cortical potentials in diseases of the central nervous system].

This is the first report to be published on olfactory evoked potentials in patients with well-defined lesions of the central nervous system and the trigeminal nerve. Absence of olfactory evoked potentials is seen in post-central and parietotemporal lesions. The first peak of the so-called olfactory evoked twin potential is absent in lesions of the basal nuclei and sectioning of the trigeminal or ophthalmic nerve, whereas there is no second peak in subcortico-frontal and cortico-temporal lesions. Tumours of the corpus callosum and sectioning of the maxillary and mandibular nerves do not disturb the olfactory evoked potentials. The anatomically different localisation and the functional synergism of the olfactory and trigeminal systems in the perception of odours and the processing of olfactory evoked potentials are pointed out.

[Computed tomographic volumetry of the pneumatic system of the ear. Comparison of normal subjects and patients with ear diseases].

The volume of the pneumatised portion of the ear was determined by computed tomography in 235 patients. Amongst the 470 ears examined, 320 had no disease in the ear, nose or throat. Normal pneumatisation of the ear can be quantified by CT and strict correlation has been demonstrated between the air content and age from ten to 17 years. Chronic inflammatory processes of the middle ear occurring during childhood interfere with pneumatisation on one or both sides. The most severe failure of pneumatisation occurs with congenital abnormalities of the ear, most cases never showing significant pneumatisation. Chronic infection of the tonsils or adenoids causes much less interference with pneumatisation.

[Immunopathologic reactions in chronic sialadenitis of the submandibular gland (so-called Küttner tumor)].

The aim of this study was to investigate immunological aspects of the inflammatory reaction in the submandibular gland during chronic sialadenitis. Specimens of 54 patients registered at the Institute of Pathology at the University of Hamburg were stained according to the indirect immunoperoxidase method. The distribution patterns of Ig-secreting plasma cells (IgA, IgG, IgM) and of the enzymes lactoferrin, lysozyme, and secretory component were analysed. The formation of lymph follicles was also examined. The results were as follows: Stage 1 of the chronic sialadenitis starts as a simple inflammation with an increase of IgA-secreting plasma cells around the ducts. During stage 2 the unspecific, humoral part of the immune system is stimulated. The production of lactoferrin and lysozyme is enhanced. The titer of IgA rises due to activation of the local, specific immune system. The simultaneous rise of the IgG titer is probably caused by production at different sites. During the course of stage 3--4, the cells involved in the local, unspecific immune system are destroyed. The number of IgG secreting plasma cells also decreases. The formation of lymph follicles and the infiltration of the gland by lymphocytes is interpreted as a transition to the cellular immune response. Stage 4 of the Küttner tumor shows similarities to the autoimmune disease of Sjögren's syndrome.

[Radiation therapy of the nasopharyngeal carcinoma: results of treatment and criteria of optimization (author's transl)].

From 1968 till 1977, out of 84 patients with a malignoma of the nasopharynx 44 underwent sole radiation therapy with curative doses because of histologically established nasopharyngeal carcinoma. The 3-year survival rates for all the carcinomas amount to 63.2%, 5-year survival to 46.5%. Prognoses differ according to the histological type of carcinoma, but therapeutic results are also influenced by osseous involvement or by spread to the lymph nodes. Local recurrences appeared in 20% at the base of the skull, at the pharyngeal roof or in the region of the posterior nasal apertures. Following analysis of the patient collective a critical examination of imaginable and of practically used irradiation schedules is performed. Of the possible arrangements for radiation therapy those with readily reproducible parallel opposed field adjustments using ultrahard photon beams are to be preferred.

[Scanning electron microscopy studies on epithel surface of the plica vocalis in male rats (author's transl)].

The epithelial surface of the Plica vocalis in adult male rat is studied by scanning electron microscopy. It's structure is compared with that of the plica and of the recessus ventricularis. The transition of the epithelium in the plica vocalis to the epithelium of the joining parts of the larynx is documented. Special attention is paid to the occurrence of microridges at the apical poles of epithelial cells in the larynx. Up to now microridge cells have been said to be characteristic for the plica vocalis.

[Scanning electron microscopy studies concerning the epiglottis epithelia of rats (author's transl)].

The epithelial surface of the epiglottis in adult male and female albino rats is studied by scanning electron microscopy. Prominent differences exist between the surface morphology as observed in the males and that observed in the females sacrificed in the estrus phase of the ovarial cycle. The differences are most pronounced in respect to the amount of secretory vesicles, microvilli- and microridge-cells and in respect to the observable number of desquamating cells. The existence and the importance of the microridge-cells are discussed. Special attention is also paid to the results indicating that at least some parts of the extrapulmonary airway epithelia act as possible target organs for sex hormones. Finally the importance of the observed differences is discussed for the correct interpretation of pathomorphological alterations in the extrapulmonary airway epithelia that have been observed under experimental conditions and during infections.

Microridge cells in the larynx of the male white rat. Investigations by reflection scanning electron microscopy.

The laryngeal epithelium of male white rats is studied by reflection scanning electron microscopy (SEM). In addition to ciliated cells, microvilli cells, brush cells and goblet cells that are characteristic for normal respiratory epithelium the microridge or labyrinth cell can be seen in particular regions of the larynx. The apical surface of a typical labyrinth cell is characterized by a system of narrow standing microridges of about 0.05--0.15 micronm in diameter and interconnecting microridges with a diameter of about 0.01 micronm. The microridge system of a labyrinth cell originates from the fusion densely standing microvilli. Between microridge cells and microvilli cells all transition forms can be observed. The preferable localiziation of the microridge cell in the larynx and its possible function is discussed.
