[Tubular apocrine adenoma: apropos of a case].

Tubular apocrine adenoma is a rare, benign apocrine tumour of the skin, histologically distinct from other epithelial proliferations, showing apocrine differentiation. A new case of this uncommon lesion is reported herein and a review of the literature concerning this tumour is made.

[Calcified Malherbe epithelioma of an unusual histological type].

A case of calcifying epithelioma (of Malherbe) is reported, which seems to be developing on an epidermal cyst: microscopic examination shows a sudden transformation of the wall of the epidermal cyst into a calcifying epithelioma. This histological feature is very rare: only 2 similar cases have been reported in the literature until now.

[Congenital atrophoderma in multiple plaques. A new clinical form of mastocytosis?].

The authors report the case of an eight-day-old male child with multiple yellowish atrophic plaques, approximately 2 to 10 cm in diameter, on the scalp, neck and abdomen. Darier's sign was negative. No visceral involvement was detected. Histological examination showed numerous histiocytic-like cells with a foamy cytoplasm throughout the dermis. Metachromasy with toluidine blue stain and naphtol AS-D chloracetate esterase were negative. Mastocytes were identified upon ultrastructural examination which demonstrated long interdigitated villi. Unusual features were the absence of typical cytoplasmic granules and the presence of multiple large electron-lucent vacuoles which were assumed to be stored lipids. The authors hypothesized that chronic stimulation of mastocytes in utero may have led to cell degeneration through an immunologic or pharmacologic mechanism. This could explain the negativity of specific stains for mastocytes and the collagen and elastic damage responsible for the atrophy of the lesions.

Action of nitroguanidinated luteinizing hormone on different rat gonadal tissues.

The biological activities of nitroguanidinated derivatives prepared from ovine or porcine luteinizing hormone were investigated using rat Leydig cells and pseudopregnant rat ovaries. In these tissues nitroguanidyl ovine luteinizing hormone (NGoLH) or nitroguanidyl porcine luteinizing hormone (NGpLH) were unable to stimulate adenylate cyclase or steroidogenesis but were able to inhibit the binding of ovine or porcine native LH to their specific receptors. When added to incubations of isolated Leydig cells or pseudopregnant ovary slices, NGoLH as well as NGpLH inhibited the stimulating action of native LH on adenylate cyclase or steroidogenesis. However, these derivatives had no inhibiting action on the stimulation of adenylate cyclase and steroidogenesis induced in the Leydig cells by choleratoxin or on the stimulation of testosterone production induced by 8-bromo-cyclic AMP. Since NGpLH (which does not contain lysine residues or free alpha-amino groups in the beta-subunit) exhibits the same antagonist action as NGoLH, we conclude that the nitroguanidination of the alpha-subunit is sufficient to endow the derivative with antihormone properties.

Action of some luteinizing hormone derivatives in ovaries from pseudopregnant rats: dissimilarities between their activities on this organ and on Leydig cells.

Biological activities of several derivatives of ovine LH obtained by chemical modification of the amino groups were investigated using ovaries from pseudopregnant rats. Binding-inhibition activities and steroidogenic potencies of ethylated, isopropylated and guanidinated LH were in good agreement, whereas adenylate cyclase activities were relatively greater. When compared with previous results on binding-inhibition activities and steroidogenic potencies using isolated rat Leydig cells, the ovaries from pseudopregnant rats appeared to be more discriminating. Ethylated and isopropylated derivatives exhibited lower binding-inhibition activities and steroidogenic potencies in female gonads. This difference was particularly evident in the case of guanidinated LH which exhibited a very low binding-inhibition activity and consequently was unable to act as an inhibitor of the action of LH on the ovaries. Guanidinated porcine LH (in which all the lysine residues of the alpha-subunit were transformed into homoarginine, without modification of the beta-subunit which does not contain lysine) showed similar biological activities to guanidinated ovine LH in the isolated Leydig cells as well as in pseudopregnant ovaries. It can, consequently, act as an inhibitor of LH action on Leydig cells but not on the ovary of the pseudopregnant rat. Thus, the inhibitory properties of this derivative can be ascribed to the modification introduced in the alpha-subunit.

[Clear cell acanthoma. Ultrastructural and immunohistological study].

We report a new case of a clear-cell acanthoma with an unusual location (para-anal), which was studied by electronmicroscopic and immuno-histological methods, using monoclonal antibodies. The results of this study showed that: a) the inflammatory cellular infiltrate comprised a small number of T-suppressor/cytotoxic lymphocytes and a small number of Langerhans cells, b) Langerhans cells were present in the affected epidermis in reduced numbers (2.2 p. 100) when compared to normal epidermis (4-6 p. 100), c) there was a disorder of both keratinization and epidermal differentiation as was shown by the abnormal reactivity of the tumor to monoclonal antibodies KL1 and BL7, d) human papilloma viruses do not seem to be involved in the genesis of the tumour, e) tumoral cells did not show (secretory) eccrine differentiation, f) the pemphigus antigen was present in the intercellular space of the affected epidermis.

[Buschke's scleredema and monoclonal dysglobulinemia: apropos of 3 cases].

A monoclonal gammopathy was observed in 3 adult patients with long-term and widespread scleredema (Buschke's disease). There was no evidence of multiple myeloma in any patient. The monoclonal immunoglobulin was in every case of the IgG-kappa type. Bence Jones proteinuria was noted in 1 case (kappa). Deposition of monoclonal IgG-kappa in the skin was not detected by immunofluorescence microscopy. These findings and previous reports of 5 cases suggest that diffuse scleredema may be frequently associated with paraproteinemia, but the role of monoclonal immunoglobulins in the pathogenesis of the disease remains to be clarified.

Treatment of cutaneous T-cell lymphoma with a new aromatic retinoid (Ro 10-9359).

A 77-year-old woman developed a diffuse nodular eruption with histologic, ultrastructural, and biologic evidence of cutaneous T-cell lymphoma (CTCL) limited to the skin. She was treated with a new aromatic retinoid Ro 10-9359 (1 mg/kg/day). After 34 days, the lesions flattened completely and the mononuclear cell infiltrate decreased significantly. No clinical recurrences occurred after a 4-month survey. Discontinuation of the aromatic retinoid led to a relapse with identical clinical and histologic features.

[Skin reactions to piroxicam (three cases) (author's transl)].

Three cases of skin reactions that occurred in patients under oral administration of Piroxicam are reported here: one erythema-multiforme-like reaction and two cases of acute, pruriginous erythemato-papulous dermatitis, one of which with intraepidermal blister formation. No past history of skin disease could be formed; lesions spontaneously improved when the drug was stopped. Only in one patient, receiving Piroxicam and Fenoprofene, an acute nephritis was observed. The role of Piroxicam, a new non-steroid antiinflammatory agent in the pathogenesis of the skin reactions observed is discussed. However, the renal involvement formed in one case is likely due to Fenoprofene, as in previously reported cases.

[Premalignant fibroepithelial tumors of Pinkus and previous irradiation: report of a survey of 20 patients].

The search for previous irradiation was done: in nine of the twenty cases radiotherapy or repeated roentgenograms are found, usually twenty years before the start of fibroepithelial tumour. In one case, there was ingestion of arsenic for psoriasis. But in ten cases, no antecedent of irradiation are found (but the interrogatory was sometimes difficult). So it appears that previous irradiation, if it helps starting of fibroepithelial tumour, is not necessary, and the further evolution is the same, with or without irradiation.

LH receptors in ovine corpora lutea in relation to various physiological states and effects PGF-2 alpha on LH-induced steroidogenesis in vitro.

The LH binding properties (determined using tritiated methylated LH) and the in-vitro steroidogenic activity of CL from ewes in the oestrous cycle or early pregnancy (Day 18) were compared. No significant alteration in the Kd values was observed. However, the number of sites was maximal at Day 10 of the cycle and in early pregnant animals which had not been pregnant for at least 3 months (dry ewes). Non-lactating or suckling ewes had half the numbers of binding sites. The increase of the number of receptor sites was accompanied by a steroidogenic response at lower LH concentration. During incubation or superfusion for 5 h, a refractoriness to LH stimulation appeared after 1 h with high LH concentrations and after 3 h with low concentrations. The opposite effect of the addition of indomethacin or PGF-2 alpha suggests the intervention of PGs in this phenomenon.
