[Home blood pressure monitoring].

Home blood pressure measurement is a simple, precise and highly reproducible method. Its application is possible in a large majority of patients. The method is valid if performed with devices validated according to international criteria and if an appropriate methodology is applied. It is advantegeously used for diagnostic of high blood pressure, particularly white coat hypertension. It allows for an exact appreciation of treatment efficacy and a long term follow-up of blood pressure control. In a majority of patients it improves their compliance to drug treatment. Its precision allows to decrease the number of patients needed in clinical trials. The main obstacle of larger use of home blood pressure monitoring is the need to give a detailed instructions to patients and generally a laborious evaluation of results. The latter could be simplified by using devices transferring measured results directly to physician's computer. However the cost of such instruments remains high. Finally the lack of financial reimbursement of physicians using home blood pressure measurement limits further its larger use.

Changes in rat melanin-concentrating hormone and dynorphin messenger ribonucleic acids induced by food deprivation.

Melanin-concentrating hormone (MCH) and dynorphin genes are expressed in two discrete neuron populations of the rat lateral hypothalamus. Their roles remain hypothetical in mammals. In order to analyse changes in MCH and dynorphin gene expression, a multiplex competitive semi-quantitative reverse transcription-polymerase chain reaction (RT-PCR) was developed to assay their mRNAs. This technique was used to examine MCH and dynorphin mRNA content in 24-h and 48-h food-deprived rats compared to controls. A two-fold induction of dynorphin mRNA by 24 h, followed by a sharp decrease at 48 h were observed. A moderate increase in MCH mRNA content was noticed by 24 h; 48 h of fasting restored the control levels.

Exploring the expression of the melanin-concentrating hormone messenger RNA in the rat lateral hypothalamus after goldthioglucose injection.

Melanin-concentrating hormone (MCH) is expressed in a large neuronal population of the rat lateral hypothalamus. This area is known to be implicated in the regulation of thirst and hunger and to contain glucose-sensitive cells. In the present study, we investigated the effects of goldthioglucose (GTG), a toxic form of glucose, on the expression of the MCH gene in the rat lateral hypothalamus by immunocytochemistry, in situ hybridization and competitive RT-PCR. We observed that the MCH immunoreactivity and the level of MCH mRNA were not changed after intraperitoneal GTG injection (0.35 mg/g body weight). These results together with previous data suggest that the glucose-sensitive cells of the lateral hypothalamus are different from the MCH neurons and remain to be identified.

[Study of gene expression of melanin-concentrating hormone and of dynorphin in the rat hypothalamus by multiplex competitive RT-PCR].

Melanin-concentrating hormone (MCH) and dynorphin genes are expressed in two discrete neuron populations of the rat lateral hypothalamus. Their roles remain hypothetical in mammals. In order to analyze changes in MCH and dynorphin gene expression, we developed a multiplex competitive semi-quantitative RT-PCR allowing to study simultaneously the variations of their mRNAs. This technique was used to examine MCH and dynorphin mRNAs contents in rats food-deprived for 24 or 48 hours as compared to controls. A 2-fold induction of dynorphin mRNA by 24 hours, followed by a sharp decrease at 48 hours and return to control level were observed. A moderate increase in MCH mRNA content was noticed by 24 hours. A 48 hours fasting restored the control levels.

[Informed consent: what is the question? Correlation between practice and theory].

Informed consent in medical or human experimentation is a concept generally accepted by most doctors. The foundation of informed consent is self-determination and, in the legal setting of France, contractual agreement. The practical application of informed consent in medical specialities such as psychiatrics or intensive care raises difficult problems subject to much debate. Contemporary history would suggest that the strength of informed consent may be highly insufficient to avoid the risk of dictated power dominating-patients. In order to find a solution to these problems, the field of action of informed consent must be widened. We propose here to define informed consent as one of many means to defend patient dignity and human rights.

[Intracytoplasmic injection of spermatozoa (ICSI). Ethical requirements and assessment].

There is no doubt that the clinical application of intracytoplasmic sperm injection (ICSI) has been a major breakthrough in the treatment of extremely severe male-factor infertility. We analyzed the impact of ICSI on medically assisted reproduction practices. The potential of risks ICSI, whether related to abnormalities associated with male infertility, penetration within the ovocyte or the risk of introducing foreign material, should not be neglected. With ICSI, a situation of human experimentation has been created without adequate prior animal experimentation and research has been conducted without the approval of ethics committees as required by the Huriet Law in France. When first introduced, ICSI was not accompanied by an evaluation protocol to follow the biological risks and societal impact. We thus believe that meeting the requirements of the following ethical criteria deserves discussion. ICSI should still be considered as an experimental clinical procedure. Risk should be evaluated with animal models since all forms of spermatozoa can now be injected into mouse oocytes. Complete independence is needed for biological and genetic assessment, pediatric follow-up on children born and societal and health care impact. Scientific personnel as well as patients and the society in general should be continuously informed of the status of the evaluation work. As for any experimental therapy, the process of obtaining informed consent takes on special significance. Careful explanation of the procedure is needed so the patients can be in a position to determine what level of risk is acceptable.

Characterization of an Arabidopsis thaliana gene that defines a new class of putative plant receptor kinases with an extracellular lectin-like domain.

We have characterized an Arabidopsis receptor-like serine/threonine kinase gene, Ath.lecRK1 (Arabidopsis thaliana lectin-receptor kinase), defining a new and putatively important class of plant receptor kinases. Structural features of the predicted polypeptide include an amino-terminal membrane-targeting signal sequence, a legume lectin-like extracellular domain, a single membrane-spanning domain, and a characteristic serine/threonine protein kinase domain. A recombinant protein containing the kinase domain can be autophosphorylated on a serine residue. Ath.lecRK1 is a member of a gene family of at least two closely related genes. Northern blot analysis indicates that the Ath.lecRK1 gene is weakly expressed in a variety of organs and is regulated in Arabidopsis cell suspension cultures according to the growth phase of cells. The role this new class of plant receptor kinase could play is discussed with regard to the transduction of oligosaccharide and plant hormone signals.

Determination of tryptophan and its kynurenine pathway metabolites in human serum by high-performance liquid chromatography with simultaneous ultraviolet and fluorimetric detection.

An isocratic reversed-phase high-performance liquid chromatographic method for the simultaneous determination of tryptophan and four metabolites of the kynurenine pathway (kynurenine, 3-hydroxykynurenine, kynurenic acid and 3-hydroxyanthranilic acid) in human serum is described. This new method, which uses both isocratic elution and two on-line connected programmable ultraviolet and spectrofluorimetric detectors, allows the determination of these metabolites, in the physiological ranges, with satisfying specificity and sensitivity within 30 min.

Further progress towards a catalogue of all Arabidopsis genes: analysis of a set of 5000 non-redundant ESTs.

Nearly 7000 Arabidopsis thaliana-expressed sequence tags (ESTs) from 10 cDNA libraries have been sequenced, of which almost 5000 non-redundant tags have been submitted to the EMBL data bank. The quality of the cDNA libraries used is analysed. Similarity searches in international protein data banks have allowed the detection of significant similarities to a wide range of proteins from many organisms. Alignment with ESTs from the rice systematic sequencing project has allowed the detection of amino acid motifs which are conserved between the two organisms, thus identifying tags to genes encoding highly conserved proteins. These genes are candidates for a common framework in genome mapping projects in different plants.

Rapid estimation of Escherichia coli in live marine bivalve shellfish using automated conductance measurement.

Conductance measurement for quantitative estimations of Escherichia coli in live bivalve shellfish was evaluated as an alternative to the conventional most probable number (MPN) method used in France. The sensitivity of the two techniques was comparable. A single regression line (r = -0.968, P < 10(-6)) between log10 MPN and detection time (DT) was used to estimate E. coli concentrations for all shellfish examined. Estimation accuracy was +/- 0.92 log unit. Repeatability was better for DT than the log10 of MPN estimations (average coefficients of variation 2.7 and 9.3%, respectively). The conductance signal was attributable to E. coli in 96% of cases, and only 0.7% of E. coli cultures failed to exhibit a signal within 20 h. The conductance method reduces analysis handling time and is much easier to use than the MPN method. Moreover, results can be obtained within 5-9 h compared to 3 d for the MPN method.

[Ethical problems with the decision of refusal to a request for intra-conjugal in vitro fertilization].

What is our knowledge of the couples who suffer refusal of access to IVF when information is available for only 52% of the pregnancies obtained by IVF? With a retrospective study on IVF refusal datas in a centre where they are available, and an investigation by questionnaire in twelve other centers, the authors tried to evaluate the incidence of these refusal. IVF refusal concern 6 to 30% of the couples addressed, varying on a plurifactorial preselection; but a register including refusal is available in only four of the twelve centers. The main reason for refusal is maternal age. Only three of the twelve centers inquired have a real multidisciplinary staff for access decision. Moreover, if the twelve centers knows the future for the obtained pregnancies, it's true sometimes for only seven centers when IVF is unsuccessful, and never for ten centers when refusal of IVF. Assessment of IVF is incomplete, and improvement is needed based on: a better diffusion of the actual knowledge and its applications; a vast application of "ethical committee procedure" when decision for access is taken; and a global approach of IVF, considering refusal as well as success.
