Maintenance chemoimmunotherapy of nonlymphoblastic acute leukemias.

A trial of maintenance chemotherapy of nonlymphoblastic acute leukemia led to a comparison of two groups of patients in complete remission. Group 1 (14 patients) received only monthly reinduction chemotherapy. Group 2 (17 patients) received identical chemotherapy together with weekly immunotherapy combining BCG and irradiated leukemic cells. While the duration of the first complete remission was unmodified, the overall survival time and, above all, survival after the first relapse were prolonged in group 2 chemoimmunotherapy. These results were all the more marked when a homogeneous group of patients having received the same induction chemotherapy were considered.

[Adult Mediterranean visceral leishmaniasis: report of 12 cases (author's transl)].

Mediterranean visceral leishmaniasis is not only a children disease. We observed 12 adult cases since 1971. Clinical presentation usually associates anarchic fever and splenomegaly, with anaemia, leucopenia, increased sedimentation rate and hypergammaglobulinemia. We must think to kala-azar, specially after a stay in an endemic country (mediterranean area), ask for parasites on bone-marrow smears and more often immunological diagnosis. Spontaneously fatal, the disease is easily cured by antimony derivatives.

[Solitary secreting plasmocytoma in a femoral localization in a young man (author's transl)].

A pathological fracture in 20-year-old man led to the discovery of a right femoral plasmocytoma. Immunoelectrophoresis revealed the existence of a monoclonal Kappa IgG. After tumour reduction by radiotherapy and chemotherapy, surgical excision was possible with the insertion of a reconstructive prosthesus. Functional recovery was complete. The dysglobulinaemia completely disappeared and, with a two year follow-up, no dissemination of the plasmocytoma has been found.

Experimental trabeculotomy with a monomode argon laser.

The laser trabeculotomy is one of the new techniques using light as a surgical ability in the treatment of some ocular diseases. The trabecula, in fact, plays an important part in the absorption of the aqueous humor and its bad functioning is the reason for chronic glaucoma. Here are some results obtained on in vitro trabecula with YAG-neodymium laser and an argon laser, thus showing that it is necessary to have very good focussing if we want to obtain a drill on the trabecula without having an excessive laser power.

Percutaneous vascular stent: experimental studies and preliminary clinical results in peripheral arterial diseases.

A new pattern of metallic intravascular stent has been experimentally and clinically tested. When the stent is percutaneously placed through a six french introducer, it is held against the vessel wall by its natural elastic properties. In 28 animals, 47 stents of different sizes (from 3 to 5 mm of luminal diameter and from 1.5 to 5 cm of length) have been implanted in the femoro-popliteal (16), the coronary (20), the carotid (3) and the renal arteries (5) and in 3 femoral veins. Angiographic and histological results have shown the stent's low thrombogenicity when it is well fit to the vessel's caliber. Moreover, they have shown its incorporation to the vessel wall by intimalisation as early as the third week after its implantation. The collateral vessels which covered by the stent remain permeable. At the end of this experimentation, 10 implants have been achieved in 8 patients (4 recurrences of iliac stenosis and 6 femoral stenosis). The angiographic results show the perfect tolerance of the organism at 6 months. This good tolerance is revealed by a thin intraluminal border which does not change the artery's diameter. This confirms the experimental results. The permeability is good except for two patients who had an obliteration after one month (one iliac and one femoral obliteration).
