Dietary calcium intake in postmenopausal Chinese women.

Dietary calcium intake has been shown to vary both between and within populations. The aim of this study was to compare two methods of assessment of dietary calcium in the local population of postmenopausal Chinese women. The intake was measured using 24h dietary recall as well as a food frequency questionnaire. There was a high correlation in the measurement of calcium intake between these methods. The mean daily intake in postmenopausal women not taking calcium supplementation was 397.6 mg assessed using 24 h recall, and 390.9 mg according to the food frequency questionnaire. There was no evidence of a difference in the reproducibility of the results given by the two methods. The dietary intake of calcium in postmenopausal Chinese women is below the recommended daily allowance for adult females in East Asia, and coupled with other factors may predispose to the development of osteoporotic fractures.

Case report: spontaneous pregnancy following thymectomy for myasthenia gravis associated with premature ovarian failure.

We report the case of a 29-year-old woman who was diagnosed to have premature ovarian failure (POF) and myasthenia gravis at the age of 27 years. She was treated by thymectomy and pyridostigmine. Hormone replacement was started after the operation and she subsequently conceived spontaneously. We believe this is the first report of a spontaneous pregnancy after the diagnosis of POF associated with myasthenia gravis.

Transvaginal sonography in the diagnosis and management of cervical incompetence.

Cervical incompetence remains a controversial area in obstetric management. The development of transvaginal sonography has resulted in a clearer image of the internal cervical os, a development which may indirectly have beneficial effects on outcome. However, the rapid development of cervical dilatation in some cases necessitates frequent ultrasound monitoring.

Severe ovarian hyperstimulation syndrome with minimal ovarian enlargement: a case report.

Severe ovarian hyperstimulation syndrome is an uncommon but potentially lethal complication of treatment with in vitro fertilization and other assisted reproductive technologies. We describe such a case which was atypical in that ascites and hydrothorax occurred despite the absence of ovarian enlargement. Whilst the pathophysiology of the syndrome remains unclear, the clinician must remain alert to the possibility of such an unusual presentation of this syndrome.

Transvaginal sonography of the uterine cavity following curettage for early pregnancy failure.

In a study of 50 women presenting with sonographically confirmed early pregnancy failure, the uterine contents were measured in both sagittal and transverse planes before proceeding with evacuation of the retained products of conception. A repeat sonographic study of the empty uterine cavity was performed on the day after curettage and similar measurements were made, the subjects having been divided into four groups according to gestational age. There was no significant difference between the groups in the postoperative measurements of either uterine cavity width or area, nor was the size of the empty uterine cavity influenced by the volume of tissue within the uterus before evacuation. Whilst the measurement of width was similar both before and after evacuation of the uterus in all groups, the sagittal area was significantly smaller postoperatively. There was no evidence of retained tissue at any of the postoperative sonographic examinations, nor did any of the subjects in the study develop subsequent complications. Assuming the dimensions of the uterine cavity following curettage are representative of an empty uterus, this will form the basis of a prospective study to determine whether spontaneous abortion can be safely managed without curettage in selected cases, if the preoperative sonographic measurements fall within a specified range.

The relationship between follicle diameter, fertilization rate, and microscopic embryo quality.

The diameter of preovulatory ovarian follicles was measured at the time of transvaginal US-guided oocyte retrieval, and the oocytes were subsequently examined to assess fertilization rates and the quality of developing embryos. With follicles divided into three groups of increasing diameter, there were no significant differences in the fertilization rates of oocytes recovered from follicles of different size. Embryo quality improved with increasing follicle size, although the differences were not highly significant. Our results demonstrate that an acceptable laboratory outcome can be achieved with oocytes retrieved from smaller sized follicles.

Neonatal outcome and its relationship with maternal age.

The relationship between maternal age and neonatal outcome was examined in 22,689 pregnancies using various determinants of neonatal well-being which included evidence of fetal distress, birth-weight, Apgar scores, the necessity for admission to the neonatal unit and other indicators of neonatal morbidity. Differences in the incidence of congenital malformations and perinatal mortality were also studied. There was a trend towards more frequent fetal heart monitoring, lower birth-weight and a higher rate of neonatal unit admission for infants delivered by younger women. There was also a significant increase in the Caesarean section rate with advancing maternal age. Maternal age had no effect, however, on the incidence of fetal distress, Apgar score, the development of respiratory disease, the need for intubation and ventilation nor on subsequent neonatal central nervous system complications. There was also no association between maternal age and either perinatal mortality or the incidence of congenital malformations. The favourable outcome in teenagers in this study may have been influenced by the extremely low pregnancy rate amongst young adolescents in Hong Kong, but a similar outcome in the mature age women was likely to have reflected the recognition of risk and its appropriate management.

Unilateral gamete intrafallopian transfer: the preferred method?

Pregnancy occurred in 8 of 14 patients with the unilateral transfer of three or four oocytes at GIFT procedures. These results suggest that unilateral GIFT may be at least as successful as the bilateral approach. It would appear that unilateral GIFT rather than in vitro fertilization should be used for patients with one normal fallopian tube, and if the pregnancy rates with unilateral GIFT are substantiated, then this would be preferable to the bilateral approach, as operating time is shorter and there is less risk of trauma to the fallopian tubes at the time of gamete transfer.

Ultrasonic measurement of fetal femoral length in singleton and twin pregnancies.

The fetal femoral length and biparietal diameter were measured by ultrasound on 1657 occasions in 1289 singleton pregnancies between 12 and 41 weeks' gestation. Mean values for fetal femoral length were significantly lower than those demonstrated in previous studies, with the difference becoming greater approaching term. Three hundred twenty-two measurements of fetal femoral length were made on 57 patients with twin pregnancies, with no significant difference noted between the means of these recordings and the means of the singleton pregnancies.

Characteristic fetal heart rate changes in severe rhesus isoimmunization.

A case of pregnancy complicated by severe rhesus isoimmunization is described. The finding of a sequence of fetal heart pattern changes on daily cardiotocography offers a simple method of detecting the severely compromised fetus, which appears to be related to the underlying pathological process, and is more sensitive than other tests of fetal well-being currently used.
