Plasty of tendon with dura mater. Experimental study.

The authors present an experimental study of 30 rats, in which human dura mater preserved in glycerin was used to repair gaps in the Achilles tendon. The animals were killed 8 weeks after the surgery and evaluated by gross and microscopic examination. The results showed that the dura mater did not cause foreign-body reactions or adhesions and was surrounded by concentric layers of fibroblasts and collagen fibers, suggesting that it could be employed as a substitute for damaged tendons.

Functional anatomy of the portal vein and its main and segmental branches in the adult man.

The AA. study the special disposition of collagen, elastic and muscular fibers in the supraduodenal portion of the portal vein and its main and segmental branches. Forty specimens were removed at necropsy from adult individuals and prepared by various methods: a) pellicle preparations stained by resorcin-fuchsin (Weigert), Azan, azo-carmine B, alum-carmine, or else examined by polarized light without staining, b) dissection under stereomicroscope of specimens treated by the method of Semper or van Gieson, c) histological sections stained by Masson's trichrome, Azan, Weigert's resorcin-fuchsin and Verhöff methods. The results corresponding to the different layers and levels of the vascular system studied are schematically reported in Figures 1A, 1C, 2A, 2E and 4B and are commented upon under a morphofunctional criterion.

Presence of apolipoprotein E epsilon4 allele in cerebral palsy.

The apolipoprotein E gene, which is located on chromosome 19, has three alleles (epsilon2, epsilon3, and epsilon4). Several recent publications associate the presence of the apolipoprotein E epsilon4 allele with the occurrence of neurologic diseases, and consider it a risk factor for the development of central nervous system affections. A group of 40 patients with cerebral palsy was studied and compared to a control group of 40 subjects, and higher occurrence of the allele epsilon4 in the group of subjects with cerebral palsy was observed. A significantly higher risk of developing cerebral palsy was demonstrated among those subjects with the apolipoprotein E epsilon4 allele.

Mycobacterial subcutaneous arteritis.

The authors report three patients with subcutaneous erythematous nodules in different phases of development, unspecific systemic symptoms, positive PPD test, and normal chest X-rays. The histopathological study of the older nodules showed a granulomatous arteritis with a few acid-fast bacilli in the vascular wall. The nodules at an early phase showed an unspecific panniculitis with some acid-fast bacilli in apparently normal cutaneous vessels. These findings suggest that the mycobacterium has a vascular tropism and may cause a primary granulomatous arteritis.

Age related changes in the elastic fiber system of the interfoveolar ligament.

In order to evaluate age related changes of the elastic fiber system in the interfoveolar ligament, we studied the deep inguinal ring from 33 male cadavers aged from stillborn to 76 years. Selective and alternated staining methods for elastic fibers were performed to differentiate oxytalan, elaunin, and mature elastic fibers. We confirmed quantitative changes of the elastic fiber system with aging. There was a significant and progressive reduction of the oxytalan fibers (responsible for tissue resistance) and a significant increment in the mature elastic and elaunin fibers (responsible for tissue elasticity). Furthermore, there were structural changes in the thickness, shortness and curling of these mature elastic fibers. These changes induced loss of the elastic fiber function and loss of the interfoveolar ligament compliance. These factors predispose individuals to the development of indirect inguinal hernias that frequently emerge in adults and aged individuals, especially above the fifth decade.

Study of fracture healing in protein malnutrition.

The present study was designed to assess the effects of nutrition on fracture healing in a controlled animal model. Tibial fractures were studied in 115 Wistar rats divided into four groups: group 1,50 control animals kept on a normal diet; groups 2 and 3, 45 protein-deprived animals, and group 4, 20 animals submitted to protein malnutrition before the fracture and then placed on a normal laboratory diet. The parameters evaluated during the study were: variation in animal weight during the experiment (confirmation of protein malnutrition), fracture healing as determined by bone callus radiology, macroscopic evaluation of bone callus mechanical resistance, and histological fracture examination by common light microscopy after staining with hematoxylin-eosin. The results showed the beneficial effects of protein nutrition support on the healing of lone bone fractures. Callus formation was significantly improved in groups 1 and 4 compared to groups 2 and 3. In groups 1 and 4, callus development was normal, with regular bone tissue formation, whereas in groups 2 and 3 there was a large amount of fibrous-connective tissue with scarce formation of osseous tissue.

[Recurrence in post-splenectomy spherocytosis anemia due to presence of accessory spleens].

A young female patient had a recurrence of severe spherocytic anemia there years after having been submitted to splenectomy. The presence of two accessory spleens with a size of 3.5 cm and 4.0 cm, was demonstrated by technetium scanning and ultrasound images. Surgical removal of these noduli resulted again in prompt recovery. The development of the spleen beings in the fifth week of intrauterine life at the dorsal mesogastrium. The lack of fusion of splenic lobuli is responsible for the occurrence of accessory spleens. A surgeon performing splenectomy for the treatment of hemolytic anemia should always search for possible existences of accessory spleens.

[Effect of electromagnetic fields on osteogenesis: an experimental study on rats].

The authors studied experimentally the electromagnetic pulsing field effects in an experimental model in rats, for evaluation of the velocity of consolidation of tibial and fibular fractures. The animals were followed for a period of three weeks under continuous stimulation and there were done radiological evaluation weekly and histological study at the end of the study. There were no histological, clinical or radiological differences between the group of rats submitted to electromagnetic pulsing fields and the control group.

Protein-losing enteropathy in systemic lupus erythematosus.

We describe a 23-year old Brazilian woman with systemic lupus erythematosus and protein-losing enteropathy. Small intestinal biopsy revealed lymphangiectasia. Protein-losing enteropathy responded to corticosteroid therapy and should be suspected in cases of systemic lupus erythematosus with hypoalbuminemia without proteinuria and liver failure. A review of the English literature is presented with comments on pathogenesis.

Epidermal nuclear immunoglobulin deposition in connective tissue diseases.

Epidermal nuclear deposition of immunoglobulins (in vivo ANA) was observed in 45 out of 252 skin biopsies (17.8%). It occurred in 19% of cases with systemic lupus erythematosus, in 32% of the mixed connective tissue disease, in 22% of the scleroderma, in 20% of the cutaneous vasculitis, in 18% of the polymyositis, in 33% of the Sjogren's syndrome, but it was absent in cases with rheumatoid arthritis. The in vivo ANA showed a significant association with serum antibodies to an extractable nuclear antigen (ENA), with speckled pattern of immunofluorescent antinuclear antibody (FANA) and with antibody to a fraction of ENA sensitive to ribonuclease termed ribonucleoprotein (RNP). Indirect evidence was obtained suggesting that the epidermal nuclear deposition of immunoglobulins is a true in vivo phenomenon: some patients with serum antibodies to ENA do not display in vivo ANA and contrariwise, no difference was detected between diseased and normal skin for the occurrence of in vivo ANA and also no association was observed between this phenomenon with immune deposits at dermoepidermal junction or in subepidermal vessels.

[Progressive systemic sclerosis, B-cell malignant lymphoma of the ileum and thyroid adenocarcinoma].

A patient with progressive systemic sclerosis with rapid evolution to death is presented. The post-morten examination revealed besides the typical changes of the progressive systemic sclerosis in the skin, kidneys, heart and blood vessels a B-cell ileal malignant lymphoma with generalized metastases, and an adenocarcinoma of the thyroid gland. A possible mechanism involved in the association of these diseases might be the action of the oncogenes modifying the cellular hormostasis and the immunological tolerance.

[Percutaneous synovial biopsy in the diagnostic evaluation of the patient with rheumatic disease].

Forty seven percutaneous synovial biopsies from 22 patients with rheumatoid arthritis, four of whom with the juvenile form, 13 with indetermined polyarthritis and 12 with monoarthritis, were evaluated. The histopathological examination confirmed the clinical diagnosis in 76% of cases with rheumatoide arthritis and juvenile rheumatoid arthritis, and it suggested the diagnosis of rheumatoid arthritis in 80% of cases with indetermined polyarthritis. In two cases of monoarthritis it reveled acid-fast bacilli, and a granulomatous process in one. These resuls suggest that the synovial biopsies can be useful for the establishment of diagnosis in patients suffering from indetermined poly or monoarthritis.
