[HMB 45 positive balloon cells in combined nevi].

The combined naevus is made up of two components, one resembling a melanocytic naevus, the other a blue naevus. Clinically, these naevi do not give any obvious cause for concern. Histological examination shows that the combined naevus consists of a superficial melanocytic naevus and a deep-seated spindle cell blue naevus. There is a rare variant in which the pigmented spindle cells of the "blue" naevus are replaced by large balloon cells varying in melanin content. These combined naevi, because of the large cells with abundant cytoplasm, closely resemble malignant melanoma. As a further aid to diagnosis we used the monoclonal antibody HMB 45. In our study, the vesicular cells in all seven combined naevi examined reacted strongly with HMB 45. It is suggested that HMB 45 is not always helpful in differentiating between melanoma and naevi.

[Endometriosis of a scar].

We report on a case of cutaneous endometriosis having developed in the scar of a cesarean section. The slowly growing cutaneous nodule did not cause any cyclic pain or swelling. Histologic examination showed mainly histiocytic granulation tissue without multinuclear cells and extensive phagocytosis of hemosiderin. Only few tubular glands were seen.

[Effect of mitotic density on the probability of survival of patients with thin (less than or equal to 1.5 mm) melanomas].

Our studies dealt with the prognostic significance of the mitotic count for patients with thin (less than or equal to 1.5 mm) melanoma. We determined the mitotic rate of 50 patients who had died of melanoma thinner than 1.5 mm within 8 years (short-term survivors). The specimens were examined by means of a rectangular visual field; thus we could calculate the mitotic count to the square mm of the tumor surface. This outcome was compared to the mitotic rate of 50 long-term survivors who had not died within 8 years and who corresponded to the first group with regard to sex, tumor thickness, as well as tumor site. In both groups, at least 15% of the melanomas did not show any mitotic figure. With females, the average mitotic values were identical in both groups (1.1 vs. 1.15). In males, however, short-term survivors revealed a significantly (p less than 0.001) higher mitotic count (2.2) than long-term survivors (0.75).

[Malignant melanoma: prognostic evaluation using correlation coefficients].

Many of the clinical and histological variables of malignant melanoma are correlated with each other. This interaction can be examined with simple and partial correlation coefficients. The exophytic growth of the melanoma, the level of invasion, the melanoma type of Clark, and the predominating cell type are correlated with the thickness of the tumor. These variables do not influence the patient's survival in tumors with the same thickness. Sex of the patient and ulceration and diameter of the melanoma are independent variables which are not correlated with the tumor thickness. They influence the patient's survival for tumors of the same thickness. The site of origin of the melanoma is not related to the tumor thickness but to the sex of the patient. The influence on the survival of this variable is controversial.

[Florid oral papillomatosis. A contribution to cytostatic therapy].

Based on the recent clinical experience in treating florid oral papillomatosis (FOP), the only successful therapy appears to be the use of cytostatic agents. The case report of a patient with clinically and histologically confirmed FOP is described to emphasize the advantages of administering Bleomycin intra-arterially (external carotid artery) by means of a Holter catheter. This catheter is used to release pressure in the treatment of internal hydrocephalus and is designed to remain in place for a number of years. The successful treatment of FOP points out the possibility of employing a Holter catheter for administering cytostatic agents in the treatment of malignant neoplasms in the facial area. Cytophotometric examinations of the prickle cells within the FOP mass showed a diploid pattern of DNA distribution similar to that of benigh tumors. However, it is not possible to definitely establish the malignant or benign nature of FOP in this manner.
