Health program for VIP's.

The question about special health program for VIP's has been discussed several times. A definition of VIP is a key person either in top management or a person with unique competence in an organization. In small companies you find 5-10 VIP's, in bigger companies up to 50. Our customers indicated a positive interest in a new type of program. They were not satisfied with just a standard screening. They wanted more from screening than just dealing with symptoms of not wellbeing. Most of them had earlier joined different personal development programs. They find those programs useful with well-motivated reasons for change in life patterns. Therefore as a top management consultant, but also certified to work with lifestyle, stress and axiological profile, I was at that time training top management both as individuals and as a team. It was therefore easy for me to combine a personal development program with lifestyle-health screening. In our new program for VIP's we therefore work with both individuals and teams and with both health and personal development.

Blood pressure response in relation to blood lactate during exercise in patients with essential hypertension.

Thirty-four untreated patients with essential hypertension WHO I-II, 17 patients under treatment with beta-blockade, and 32 healthy controls with a wide range of exercise capacity were studied at loadless pedaling (W0) and at the work load eliciting a blood lactate concentration corresponding to 4 mmol X l-1 (onset of blood lactate accumulation or OBLA). At W0 the treated patients had approximately the same systolic blood pressure (SBP) as the healthy controls, while the untreated patients had a higher SBP than the controls. At WOBLA the SBP was higher in the untreated patients than in both the treated patients and the healthy controls. On an individual basis it was found in the healthy controls and the untreated patients that the SBP at WOBLA showed a slight and insignificant increase versus SBP at W0, whereas the difference in SBP between WOBLA and W0 ("exercise SBP increase") demonstrated an inverse relationship (P less than 0.001-0,01) versus W0. It was suggested that for both healthy controls and untreated patients the exercise SBP depended on a "basal" level as depicted by W0 and a further SBP increase due to the exercise load. The difference between the two groups was that the patients with untreated hypertension displayed their relationships at higher pressure levels indicating the presence of a hyperkinetic drive. The treated patients appeared to have a lower increase in SBP between W0 and WOBLA than any other group, suggesting an unfavorable effect of the beta-blockade.

Angina pectoris and blood lactate concentration during graded exercise.

Nineteen patients with angina pectoris referred for preoperative evaluation were studied by psychophysical scaling of the chest pain according to Borg and blood lactate determination at rest and during a graded exercise test. The exercise tests consisted of stepwise increments of 10 W every min. Anginal pain, systolic blood pressure, heart rate (HR), and blood lactate concentration were determined at the end of each work load. The exercise test was interrupted at a rated anginal pain level (RAP) of 5-6/9 ("strong"), and this load was referred to as Wmax. The patients were divided into three groups: A with a Wmax less than or equal to 74 W, B 75-124 W, and C greater than or equal to 125 W. At submaximal loads HR was almost identical at the same absolute work load in all three groups. The heart rate at Wmax varied, however, and averaged 98 (A), 118 (B), and 136 (C), respectively. Corresponding peak values for RAP were 3.8, 4.5, and 4.8. Blood lactate at rest ranged from 1.0-1.6 mmol X 1-1 and increased curvilinearly with intensity, which was also the case for RAP. RAP was almost linearly related to blood lactate in all three groups suggesting a causal relationship between these two parameters.
