Different genotypes of Yersinia enterocolitica 4/O:3 strains widely distributed in butcher shops in the Munich area.

The distribution of Yersinia spp. in butcher shops in the Munich area was studied. The isolates recovered were then characterised with pulsed-field gel electrophoresis (PFGE) to identify possible contamination routes. A total of 298 samples were collected from eight small butcher shops between June and August in 2001. Of these, 113 were surface samples from carcasses, offal and raw pork products, and 185 were environmental surface samples from tools, equipment and processing areas. The samples were studied with direct plating, overnight enrichment in nonselective broth and selective enrichment in two different enrichment broths. The Yersinia isolates recovered were characterised with PFGE using NotI, ApaI and XhoI DNA restriction enzymes. Yersinia was recovered from all eight butcher shops, and pathogenic Yersinia enterocolitica 4/O:3 was present in six shops. The occurrence of this pathogen on raw pork products varied from 8% to 25%. Pathogenic Y. enterocolitica 4/O:3 was isolated from two environmental samples: a worktable and a chain glove. Most (18/24) of the Yersinia-positive samples were found already after direct plating. Forty-nine Yersinia isolates from 24 samples were studied with PFGE. Twelve genotypes (I-XII) were obtained among Y. enterocolitica 4/O:3 when 33 isolates from 16 samples were characterised with NotI, ApaI and XhoI enzymes. The genotypes of Y. enterocolitica 4/O:3 strains differed among butcher shops. In most (5/6) shops, more than one genotype was found, indicating different contamination sources. In conclusion, raw pork products from butcher shops are frequently contaminated with different genotypes of pathogenic Y. enterocolitica 4/O:3, thus serving as an important transmission vehicle from butcher shops to humans.

The German Food Code and Nutrient Data Base (BLS II.2).

This article describes the conception and structure of the German Food Code and Nutrient Data Base (BLS). The data bank contains approx. 12,000 coded foods, menus and menu components in different stages of processing with up to 158 nutritional data for each product. Since comparatively few analytical data on the composition of foods are available, the majority of the data in the BLS are based on nutritional data calculated from recipes. Thus, a standard instrument for an uninterrupted (no missing values) evaluation of consumption surveys is made available.

[Diagnostic strategy in pancreatic tumors].

In 62 patients with proven pancreatic tumours in a joint retrospective study the sequence of diagnostic imaging methods was analysed. Their diagnostic value was evaluated according to their contribution to final diagnosis and a diagnostic strategy was derived. A valuable primary investigation is sonography. It should be complemented, according to the specific question, by ERCP, PTC and CT. The investigation with barium meal is a complementing method for the differentiation of epigastric disorders. Biopsy of the tumour for histologic purposes should be done as early as possible. A first guiding diagnosis was obtained with ultrasound in 50% of the cases, in 53% of them already with the first examination. In 23% of the patients, however, the final diagnosis was established not before surgery or dissection.

The subclavian steal phenomenon: a common vascular disorder with rare neurologic deficits.

Three hundred twenty-four patients with reversed vertebral artery blood flow were reviewed for cerebrovascular events. Neither the presence nor the type of vertebral artery steal (permanent, n = 204; intermediate, n = 120) determined neurologic symptoms, which rather were related to coincidental carotid obstructions or abnormal flow velocity patterns in the basilar artery. Most patients (n = 209; 64%) had no neurologic symptoms. Hemispheric events (n = 99; 31%) occurred most frequently in patients with additional carotid lesions and nonhemispheric events in a few patients (n = 16; 5%) often with bilateral vertebral steal. Intracranial Doppler studies from the vertebral and basilar arteries excluded spontaneous retrograde basilar artery perfusion in any of a subgroup of 50 patients examined in addition. Thus, blood flow reversal in extracranial arteries is a marker for atherosclerotic vascular disease in general, rather than an indicator of a patient's risk to develop cerebrovascular events from hemodynamic insufficiency in the territory affected. Surgical treatment is discouraged for most patients with this benign flow abnormality.

[Ultrasound diagnosis in ileus].

Unspecific clinical symptoms and negative general X-ray findings from the abdominal region have often been characteristic of early ileus stages and have caused delay in prognosis-enhancing surgery. These early stages can be more reliably identified by means of abdominal sonography. Ileus was detected by means of general abdominal sonography from three of 14 of the authors' own patients from whom negative findings had been recorded by X-ray examinations. Their ileus was eliminated by immediate surgical intervention.

Surgery of coarctation of the aorta: a nine-year review of 253 patients.

From 1973 to 1982, 253 patients--164 males and 89 females--underwent an operation for coarctation of the aorta in our clinic. Of the patients 72.3% presented with a circumscribed lesion and 58.5% with associated congenital cardiovascular defects. Resection with end-to-end anastomosis was performed in 138 patients (54.5%). Seventy-four patients (29.3%) had vascular graft prosthesis, 1.2% underwent the Clagett's operation, 9.1%, the indirect isthmoplasty and the rest (5.9%), the subclavian flap plastic. Ventricular fibrillation led to the intraoperative death of 3 infants with associated intracardiac and multiple somatic defects. The operative mortality was high in children under 15 months (13.2%), 1% in all the others taken together and 0% in all cases without concomitant lesions. Paraplegia occurred in only one patients (0.4%). One hundred twenty-eight patients were followed-up over a mean period of 3.6 years. The systolic and diastolic pressures decreased by a mean of 30 mmHg and 15 mmHg respectively. Eighty-seven patients (68%) had normal blood pressure at the time of examination. The rest (41 patients) had persistent postoperative hypertension necessitating medical management. The surgical technique elected did not influence the level and incidence of persistent postoperative hypertension, rather the level of the preoperative systolic right arm-to-leg pressure gradient (SPG) related closely to the incidence of persistent hypertension. Thirty patients (23.4%), among whom were 24 children under 10 years--some of them with preductal hypoplasia--presented with an SPG above 20 mmHg.(ABSTRACT TRUNCATED AT 250 WORDS)

[Blood bactericidal activity in calves].

Methodical aspects relating to determination of serum-bactericidal capacities were studied, with the view to using them to test the bearing of abiotic environmental factors on defence mechanisms against infections. Communicated are methodical experience by which to enhance the methods used to higher accuracy as well as experience regarding biological variance in calf and results obtained from follow-up checks in which antigens were systematically used. Reference is made also to other methodical issues. The method actually used to determine serum-bactericidal capacity was found to be suitable for an assessment of reactivity of infection defence in calf for its good accuracy. Follow-up checks, using antigens, will enable better assessment of the action of abiotic environmental factors on the complex defence system of calf.
