[Herbal medicine--taking a position. Effectiveness and usefulness of herbal preparations].

During the discussion on the elimination of certain groups of drugs and disorders from the list of obligatory services to be remunerated by state health insurances, vague, undefined terms such as "controversial" are being applied to phytopharmaceuticals and their fields of application. This may be traced back to an ignorance and global rejection of available scientific documentation. The rational background of modern phytopharmaceuticals are discussed.

Putative mechanisms of cytoprotective effect of certain antacids and sucralfate.

An investigation of cytoprotective activity of certain antacids and inert particles was carried out by treating ethanol-induced gastric mucosal damage in rats in order to clarify possible mechanisms by which aluminum-containing antacids act. Al(OH)3 inhibited gastric mucosal damage in a dose-related and time-dependent manner. Neither aluminum ions themselves nor the particle size of the Al(OH)3 complex were responsible for the observed cytoprotection, since neither AlCl3, chemically inert Al2O3*C, nor sea sand showed protective effects. Hyperosmolality in the gastric lumen was not a deciding factor in inducing cytoprotection. Silicic acid and titanium dioxide, with superficial charge similar to Al(OH)3 proved to be effective in inhibiting gastric hemorrhagic lesions and releasing PGE2, suggesting that the surface charge of Al(OH)3 may be important in its cytoprotective properties. The same may also be valid for sucralfate. Since antacid-induced cytoprotection was only partly reduced by pretreatment with indomethacin, it is likely that additional mechanisms and mediators other than prostaglandins, such as nonprotein sulfhydryls, also are involved in gastric cytoprotection arising from aluminum-containing antacids.

[Stool behavior following the administration of antacids].

The direct correlation between the dosage of magnesium in antacids and the appearance of partly massive diarrheas in ulcus-patients known from literature could be confirmed. The clinically relevant dosage of antacids of 420 mval NK/day, divided in six individual dosages, resulted in diarrheas from the second to fourth day in 32 percent of the test persons when antacid A with a high content of magnesium was administered. There was no hint for a constipating effect of Solugastril gel at these high dosages (= 12 individual doses). Indeed the application of an Al-Ca-antacid may result in a slight fecal impaction, however, at the same time also to an increase of stool frequency, which is similar to that obtained with antacid B having a lower content of magnesium. The antacid can be regarded as neutral concerning stool frequency and consistency in the majority of test persons. From all four tested antacids the triple-combination (Al, Mg, Ca), showed the least important deviations from the preceding norms concerning stool frequency and consistency. The triple-combination shows an absolutely neutral stool behaviour. Serum electrolytes remained in the normal range with the Al-Ca- and the Al-Mg-Ca-antacid; whereas after application of antacid. A with high magnesium content, important increases of magnesium could be observed after seven days already, which were also dealt with in the literature.

[Clinical experiences with a 6% hydroxyethyl starch 40000 solution; metabolic parameters and tolerance].

A 6% hycroxyethyl starch solution with a mean molecular weight of 40 000 and a degree of substitution of 0.5--0.55 was tested under controlled conditions at 79 clinics in West Germany. The following parameters were evaluated: tolerance, circulation effectiveness, influence on serum electrolytes, hemoglobin and hematocrit. A total of 779 patients received either 6% HES 40 000 alone, 6% HES 40 000 and electrolyte solution or 6% HES 40 000 solution and other volume effective plasma substitutes for treatment of hypovolemia. The results show that stable circulation can be achieved in 4 to 6 hours during narcosis as well as in the subsequent phase with a sufficient supply of fluid through 6% HES 40 000 solution. One case of flush and a slight chill were reported as side effects related to the hydroxyethyl starch.

[Effects of a fully balanced synthetic diet on physical efficiency (author's transl)].

Nutrition with Biosorbin MCT and Biosorbin forte had no disadvantageous effect in any single case. There are no hidden risks if controlled by suitable tests to see whether the fluid supply is satisfactorily adjusted in the individual case. Subjective assessment from a felling of thirst is unreliable. Clinically and ergospirometrically there is an increase in capacity in almost all groups after taking the diet. This is all the more clear if the simultaneous reduction in body weight is taken into account. A direct effect of the elementary constituents of the diet is clearly recognizable. As a result of being rapidly absorbed and metabolized there are immediately available as energy suppliers and delay the threatening fall in efficiency. In almost all subjects an improvement in respiratory economy was achieved, the body weight approached the ideal weight and the relative efficiency improved.
