Lymphocyte subsets in schizophrenic disorders. Relationship with clinical, neuromorphological and treatment variables.

Peripheral blood lymphocyte subsets and serum immunoglobulin levels were assessed in 42 patients and 37 healthy controls. 24 patients were free from neuroleptic medication and 15 had never been treated with neuroleptics. 31 patients had a diagnosis of schizophrenia (DSM-III criteria) and 11 a diagnosis of a disorder of the schizophrenic spectrum. As compared to healthy subjects, the drug naive schizophrenic patient group showed an increase of T suppressor lymphocytes, while the drug-treated schizophrenic group showed an increase of T helper lymphocytes. The drug-treated schizophrenic group differed from the drug naive one relative to a decrease of T suppressor lymphocytes. As compared to healthy subjects, the drug naive spectrum disorder patients showed an increase of absolute number of lymphocytes, while the drug treated spectrum group showed an increase of B lymphocytes. These findings did not correlate with any clinical or neuromorphological variables taken into account.

[Family expressed emotion. From research to therapeutic intervention].

The Expressed Emotion (EE) index is today one of the most important tools to evaluate the effect of family environment on the course of schizophrenia. The A. critically examine many experimental researches, performed on the EE index during the past 15 years, reporting the results of the first Italian replication study. The psychoeducational family interventions (originated by the EE research) are then considered, with particular attention to the treatment conceived by the team of the Association for the Research on Schizophrenia (A.R.S.). In the final section of the paper, the A. propose an extension of the EE research to families with chronic organic pathologies, reporting some preliminary results.

[Emotional expression: from research to psychosocial treatment].

Over a four year period, the Schizophrenia Research Association (A.R.S. - Associazione Ricerche sulla Schizofrenia) has drawn up a multi-dimensional plan of action for the families of schizophrenic subjects. The basic points of this plan revolve around advice to the family and group therapy for family members, split into "informative" and "relationship orientation" sessions. Conducting an assessment of Expressed Emotion on family members admitted to these groups, it was observed that their emotional make-up is very different from that of family members of schizophrenics selected on the basis of casual criteria. Therefore, we are proposing a strategy which takes into account the emotional make-up of the family member in dictating therapists' action in terms of each family member.

[Schizophrenia and family-expressed emotions. Study of an Italian population].

Expressed Emotion (EE) is a method of assessing the emotional make-up of a family, widely used in English-speaking countries, with particular reference to schizophrenia. This research programme arose out of a trial application of EE to a group of Italian families. Data are given here relative to a sample of 19 families with a schizophrenic member (according to DSM III criteria). The results confirm the hypothesised correlation between family EE and recidivist symptomatology of schizophrenic subjects: "high family EE" is associated with the most serious and habitual schizophrenic cases. The necessary adaptations which had to be made in order to apply EE assessment methods to Italian families are also discussed.

[Valpromide (Depamide) in the treatment of acute psychotic states. Open clinical study].

In an open clinical study on the effect of Depamide in acute psychosis, 59 schizophrenic and 23 manic hospitalized patients were tested. They were divided in two groups: in group A, Depamide was administered only after an initial period of treatment with haloperidol or chlorpromazine; in group B instead, Depamide was added from the beginning of the neuroleptic treatment. The results show that the association of Depamide with neuroleptics leads to a reduction of agitation and of the hospitalization period, whereas it ameliorates the adaptation of the patients to the therapeutic milieu.

Study of serial lymphocyte subsets in multiple sclerosis: their possible role in the evaluation of disease progression.

We followed-up 30 patients with definite multiple sclerosis (MS) for one year recording monthly: clinical evolution, neurological examination and T-lymphocyte subsets in peripheral blood recognized by monoclonal antibodies. Although clinical relapses could not be strictly associated with lymphocyte abnormalities in all cases, the parallel clinical-immunological survey permitted to correlate significantly the type of clinical course (chronic-progressive, relapsing, stable) with a correspondent immunological pattern (steadily high, transiently high, steadily normal CD4/CD8 ratios respectively). Our results suggest a possible role of serial lymphocyte subsets analysis in defining evolution of MS.

Multimodality evoked potentials and disease progression in multiple sclerosis monitoring: 18 month follow-up of 36 patients.

Serial visual, brainstem auditory and somatosensory evoked potentials have been recorded in 36 MS patients. The follow-up lasted 18 months. A correlation among neurophysiological modifications and clinical status of the tested pathways was inconstantly found. At the end of the study responses at the three modalities worsened in a higher percentage of progressive patients with respect to stable ones (in 41% instead of 11% of cases). Final latencies were more delayed in progressive that in stable cases.

Combination therapy with haloperidol and orphenadrine in schizophrenia. A clinical and pharmacokinetic study.

The use of anti-cholinergic drugs for counteracting neuroleptic induced extrapyramidal side-effects (EPS) is controversial. A possible increase in anticholinergic symptoms and a pharmacokinetic interaction are reported in literature. In order to determine these points, 22 schizophrenic patients were studied. Haloperidol (HL) was administered for 3 weeks and Orphenadrine (ORD) only in the second week of treatment. ORD reduced significantly EPS and its withdrawal led to a marked deterioration of EPS and psychopathological picture. There was an increase in HL plasma levels after starting ORD, and ORD plasma levels were positively related to EPS amelioration. Finally, prolactin seemed to be higher in those patients with EPS, particularly after ORD withdrawal.
