Self-feeding performance in nursing home residents.

In a study of cognitively impaired nursing home residents, excess disability was found in the specific mealtime task of drinking liquids and among those eating a puréed diet. Nursing home staff tended to rely on spoonfeeding, a process in which the resident is a passive recipient of care rather than an active participant in it, as an intervention among residents who were partially able to feed themselves. Feeding techniques other than spoonfeeding--including verbal and nonverbal prompts, and physical guiding--can support residents' participation in feeding even when independence is no longer possible.

Effect of routine weekly cervical examinations at term on premature rupture of the membranes: a randomized controlled trial.

OBJECTIVE: The purpose of this study was to determine whether routine antepartum cervical examinations at term are associated with premature rupture of membranes (PROM). METHODS: This was a randomized controlled trial conducted at a health maintenance organization in metropolitan Denver. The subjects were 604 term gravidas randomized to a no examination or examination group. Exclusions included preterm labor, third-trimester bleeding, cerclage, multiple pregnancy, history of PROM (rupture before the onset of labor), and planned induction or cesarean. In the no examination group, routine examinations (without clinical indication) were not performed. In the examination group, weekly examinations were performed from 37 weeks until delivery. RESULTS: No statistically significant difference in PROM or prolonged PROM (more than 6 hours) was observed between those unexposed and those exposed to routine cervical examinations at term. In addition, there were no differences between the groups in other relevant outcomes including cesarean delivery, induction, augmentation, chorioaminionitis, or neonatal infectious morbidity. CONCLUSION: In our population, there is no association between routine weekly antepartum cervical examinations at term and PROM or other study end points.

Reminiscence--when the past eases the present.

1. Fostering reminiscence in a group is an appropriate nursing intervention for a variety of nursing diagnoses, or simply to enhance the general quality of life of elderly clients. Reminiscence groups have been used by nurses in a variety of settings with a variety of types of clients, and have been associated with psychological and social benefits. 2. If participation is voluntary and leadership style is nonjudgmental and supportive, a reminiscence group should have a positive impact on elderly participants. 3. Sharing memories in a group situation may not come naturally to elderly clients. By giving thought in advance to the group's purpose, structure and rules, leadership style, and group process, the leader can enhance the group experience for both herself and the elderly participants.
