Serum digoxin concentration in a neonate with renal and hepatic impairment.

This report demonstrates the questionable usefulness of monitoring digoxin concentrations using the available immunoassays in some clinical situations. The patient reported here showed three characteristics (being a neonate with hepatic and renal impairment) that are associated with the presence of digoxin-like immunoreactive substances. The patient's serum digoxin concentration continued to rise to a peak of 43.7 nmol/L (34.1 ng/mL), 19 days after administration of the drug was discontinued.

Chemotherapy for hepatoblastoma in children.

Hepatoblastoma is an uncommon but highly malignant liver tumour found in childhood. It is associated with high mortality with overall survival of only 35%. Two cases of hepatoblastoma are described. Both had surgical excision of tumour with microscopic residual tumour at the resected margins. They were given adjuvant chemotherapy. Both patients are living well, disease-free and off treatment six years and two years since diagnosis.

Kawasaki disease--the Singapore experience.

50 children with Kawasaki disease were seen between September 1983 to March 1988. Their ages ranged from 3 months to 10 years with a mean age of 25 months. Male to Female ratio was 2.3 to 1. The diagnosis of Kawasaki disease was made between the fourth to fifteenth day of illness. Marked thrombocytosis, raised erythrocyte sedimentation rate, leucocytosis, mild anemia and sterile pyuria were common features. All patients had a normal ECG and chest X-ray. 2D echocardiogram was done in the sub-acute phase of the illness in every patient. Sixteen patients (32%) had coronary artery dilatation. Thirteen of these had serial 2D echocardiograms done over a period of more than one year. Twelve had echocardiographic resolution of the coronary lesion within 18 months of follow up. Patients with coronary artery dilatation were significantly younger. All had full clinical recovery with aspirin therapy. Kawasaki disease afflicts mostly young children and can present as a diagnostic problem. Coronary artery involvement is common, and except for age, it is not predictable by any clinical or laboratory parameter. Serial echocardiographic examinations are necessary in the management of these patients.

Pattern of glomerulonephritis in Singapore children--a renal biopsy perspective.

This study was aimed at determining the pattern of glomerulonephritis (GN) in Singapore children from a histopathological perspective. Fifty-seven consecutive children, aged between 10 weeks to 16 years, who underwent a renal biopsy at the Departments of Paediatrics, National University of Singapore and Singapore General Hospital over an 8 year period were studied. The main indications for biopsy were nephrotic syndrome (67%), recurrent gross haematuria (16%), nephritic syndrome (7%), and renal failure (10%). Primary GN occurred in 81%, while secondary GN was seen in 19%, the most common being lupus nephritis. Of the primary GN, minor abnormalities was the most common (22%), followed by focal global sclerosis (20%), focal segmental glomerulosclerosis (17%), diffuse mesangial proliferative GN (11%), focal mesangial proliferative GN (9%), membranous GN (7%), diffuse endocapillary GN (4%), diffuse sclerosing GN (4%), diffuse mesangial sclerosis (4%), and diffuse crescentic GN (2%). Immunofluorescent examination was performed in 50 children. IgA nephropathy was diagnosed in 17% of the patients with primary GN. Of the children with primary nephrotic syndrome due to minimal change disease or focal global sclerosis, about half had IgM mesangial deposits. Of 47 patients who were followed up, 9 developed chronic renal failure, of which 7 reached end-stage disease (4 have died, while 3 are on chronic dialysis). Three other patients died of other complications. The histopathological findings influenced the therapeutic decision in 49% of our patients. In summary, the pattern of GN in our cohort of patients tended to reflect more severe glomerular lesions, mainly due to our criteria of selection for renal biopsy.

Efficacy of verapamil in the conversion of supraventricular tachycardia in Singapore children.

The efficacy of verapamil in the conversion of 47 episodes of supraventricular tachycardia in 22 children was evaluated. The age of the patients ranged from 15 days to 10 years. Tachycardia was the main mode of presentation. Ten out of 22 children had viral infections. Two patients developed mild cardiac failure. Six patients had underlying cardiac abnormalities. Forty-four out of 47 episodes of supraventricular tachycardia were converted to sinus rhythm by a single dose of verapamil (0.11 +/- 0.08 mg/kg). No significant side-effects were observed. Intravenous verapamil is an effective and safe drug for the conversion of supraventricular tachycardia in children.

HLA-DR antigens in Chinese children with insulin-dependent diabetes mellitus.

We studied the distribution of HLA-DR and MT1, MT2, MT3 genotypes in 23 Chinese children with insulin-dependent diabetes mellitus (IDDM). The frequency of DR3 was 48% in IDDM children compared to 14% of normal controls (corrected p = 0.0098, RR = 5.70). The frequency of DR4 was not increased. These differences when compared with the results of Western populations may contribute to the relative rarity of IDDM among Chinese children.

HLA-system in Chinese children with insulin-dependent diabetes mellitus: a strong association with DR3.

We studied the distribution of HLA-A, B, and DR and MT1, MT2, MT3 genotypes in all 20 Chinese children with insulin-dependent diabetes mellitus (IDDM) attending the four government pediatric units in Singapore. We found an increase in HLA BW22 but the corrected probability value was not statistically significant. AW33 and B17 were observed in 50% and 55% of IDDM children, respectively, compared with 11% and 13% of normal controls, respectively. The values for AW33 were as follows: corrected P = 0.00094 and relative risk (RR) = 8.17; for B17 they were corrected P = 0.001 and RR = 7.55. In addition, the frequency of DR3 was 50% in IDDM children compared with 14% of normal controls (corrected P = 0.0019, RR = 6.20). AW33, B17, and DR3 are in linkage disequilibrium in our normal Chinese population. All ten patients who were positive for DR3 also had B17. The frequency of DR4 was not increased, and there were no protection IDDM related antigens found. These differences compared with the results in Western populations may contribute to the relative rarity of IDDM among Chinese children.

Testicular disease in Singapore children with acute leukemia.

A study was made on testicular leukaemia developing in children treated for leukaemia in the Department of Paediatrics, Singapore General Hospital over the past 6 years. Of the 44 cases of acute leukaemia treated, 24 were males, and 6 developed testicular leukaemia, all occurring in patients with acute lymphoblastic leukaemia. Half the cases developed testicular relapse while on chemotherapy with the other half developing it after all chemotherapy was stopped. There is a place for prophylactic radiotherapy to the testis. Testicular biopsy should be considered early in high risk groups.

HLA system in Chinese children with insulin-dependent diabetes mellitus.

HLA in 12 unrelated Chinese paediatric patients with insulin dependent diabetes mellitus (IDDM) were found to have an increased frequency of BW22, B17 and AW33. BW22 was observed in 5/12 (41.7%) of IDDM patients compared to 40/330 normal unrelated Chinese controls (p less than 0.005, rr = 5.2). AW33 and B17 were observed in 6/12 (50.0%) and 7/12 (58.3%) of IDDM patients respectively, compared to 36/330 and 46/330 in the normal controls respectively (AW33: corrected p less than 0.0026, RR = 8.2, B17: corrected p less than 0.0026, rr = 8.6). HLA B8, B15 and B18 did not demonstrate any significant association with IDDM in this series of patients. The results of this study further emphasize the well recognized race specificity in HLA antigen distribution in normal population as well as disease states.
