Evaluation of the effects of secondary alveolar bone grafting in both adults and children with cleft lip and palate.

The preliminary results in a prospective study to evaluate the effects of secondary alveolar bone grafting in adults and children with cleft lip and palate is presented. Bone grafting was performed in 67 patients using two different surgical approaches. Autogenous cancellous bone from the iliac crest was used in all the cases. The patients were divided into three groups according to the state of eruption of the canine and the age group, whether adult or children. The minimum period for observation of the cases following the bone grafting is three months, the maximum one and a half years. The initial findings indicated that better results were obtained when the bone grafting was done in younger patients prior to the eruption of the canine.

Tissue expanders in reconstructive surgery.

The shortage of skin and soft tissue has been the limiting factor in many reconstructive problems. Tissue expanders (a temporary inflatable device) were recently introduced, to provide the solution. In the first 25 patients of our series of patients age 7 years to 46 years, a total of 30 expanders were used in different parts of the body. The expanders used were made by Cox-Uphoff (CUI) and varied in size from 30 ml to 1000 ml, with an attached remote valve. These expanders were placed subcutaneously, subfascially or submuscularly. Expansion was carried out as an outpatient procedure over a period of 6 weeks. 21 of the 25 patients underwent successful treatment. Failures were due to technical problems, extrusion of expanders, haematoma and infection. Tissue expanders are a new dimension in reconstructive surgery. They cause stretching of skin and soft tissues over a period of time and permit resurfacing of skin defects in difficult situations. Complications may be avoided with experience and further refinements in the construction of the expanders.

Microsurgical reconstruction of the upper and lower limbs.

The availability of a wide variety of free tissue transfers has revolutionised and simplified the reconstruction of the upper and lower limbs. These microsurgical procedures will continue to provide alternatives for salvaging limbs, controlling some infections and resurfacing extensive soft and bony tissue defects. Thirteen free tissue transfers consisting of toe pulp, scapular cutaneous and latissimus dorsi myocutaneous flaps were utilised to reconstruct the thumb, hand, soft and bony defects of the lower tibial and heel regions. One latissimus dorsi flap failed because of severe infections.

Congenital cleft lip and palate in Singapore.

Cleft lip and palate is the most common congenital abnormality seen in the Department of Plastic Surgery, Singapore General Hospital. A total of 461 operative cases seen over a period of 5 years (1977 to 1981) is analysed. There is a relatively high incidence in the Singapore population of 2.0 per 1000 live births. In both cleft lip and cleft palate deformities, there is no significant difference in the sex distribution. The cleft patterns show that 78% of the cases are unilateral clefts, 53% are left sided, 25% are right sided and 22% of the cases are bilateral clefts.
