[Reoperations in primary hyperparathyroidism].

We report on 14 patients suffering from persistent (n = 13) and truly recurrent (n = 1) primary hyperparathyroidism. Reoperations were successful in 13 cases. Cervical reexploration was only required for 5 patients who had previous operations by unexperienced surgeons in other clinics. In 5 out of 7 patients operated by us, the causes of the persistent primary hyperparathyroidism were 4 adenomas and one hyperplastic gland in the mediastinum found by median sternotomy. The remaining cases were a rare ectopic location of the inferior glands and an inadequate resection in four-gland-hyperplasia. Preoperative localization procedures were carried out for most patients. 201-Tl/99m-Tc-subtraction scintigraphy has proved for localization in both the cervical region and the mediastinum to be a highly sensitive method. Reoperative parathyroid surgery can be reduced with thorough initial cervical exploration by experienced surgeons in specialized clinics.

Preoperative ultrasonic of common bile duct stones and its importance to surgical management--result of a prospective controlled trial.

The biliary tract was examined by sonography in 192 nonselected surgical patients with clinically suspected gallbladder stone disease. This study was carried out to assess the potentiality of ultrasonic examination in the detection of common bile duct stones. All patients were operated on with sonographic monitoring during surgery. Common bile duct stones can be detected with high accuracy only in patients with an obstructive jaundice and a dilated common duct. In these, the sensitivity was found to be 70%, whereas that for the whole group of patients was as low as 28%. Accuracy in the ultrasonic detection of common bile duct stones is limited, and operative cholangiography seems to retain its position as the most reliable method of exploring common bile duct stones.

[Value of ultrasound tomography in the diagnosis and follow-up of fatty liver].

In a prospective study 187 patients with diffuse fatty infiltration of the liver, estimated by ultrasound tomography, were investigated. In any case the history as well as the results of clinical investigation and the lab findings were known to the observer. Liver blind punctions were performed after a short time, on an average of 11 days. In 91.4% the sonographic diagnosis "fatty infiltration of the liver" could be confirmed by histological findings, while in the other cases the diagnosis was incorrect or incomplete respectively. Following sonographic criterions any results were graduated into three groups and compared with the percentage of fat infiltrated liver cells found by histological investigation. There was a high accuracy in patient with fatty liver: 90.9% could be established by ultrasound tomography. Only in those cases, in which the sonographic diagnosis "fatty infiltration" is not confirmed by case history, clinical and lab findings, the liver biopsy is to be performed.

[Functional muscular abdominal distension--a case report].

The rare clinical picture of the functional muscular distension must be demarcated from organic changes which are connected with an increase of the abdominal volume such as intraabdominal tumours, accumulations of fluid or gatherings of gas or pregnancy. In this symptom a functional disturbance is concerned which appears in patients with vegetative lability or psychic peculiarities and by changes of tension of the musculature surrounding the abdominal space leads to the intermittent or chronic protrusion of the abdomen. By means of an anaesthesia the presence of a functional cause can be made evident: after injection of anaesthetics the protruding abdominal wall is subsiding. The knowledge of the picture of the disease saves long diagnostic ways which contribute to the fact to fix labile patients extremely to the observation of their abdomen and to favour the manifestation of a neurosis. The often unsatisfying therapeutic long-term effect is in contrast to the harmlessness of this symptom.

[Imaging procedures in the diagnosis of nodular goiter].

A prospective examination of 80 patients with struma nodosa resulted in an approximately equally high reliability of scintigraphy (98.8%) and sonography (97.5%) with regard to the evidence of focal changes. Due to the well-known advantages and the present availability the sonography should above all be used in children, adolescents, pregnant women and iodine-contaminated patients. Since from the scintigram additionally can be reported on the functional condition (autonomous adenoma), this technique cannot generally be replaced by sonography. The advantages and the limits should be known to the physician, in order to avoid unnecessary manifold examinations.

[Ultrasound diagnosis of malignant lymphomas].

The most effective treatment of malignant lymphomas requires an exact diagnosis of the spreading of the tumour in the organism (staging). In this respect the combination of various radiological methods, actually including both sonography and computed tomography stood the test. However, in order to put the ultrasound into the right place of the diagnostic-step programme one has to know the advantages as well as the limitations of this method: the sonography ought to be used at the beginning of the diagnosis and in controlling the success of the treatment of malignant lymphomas.

[Early diagnosis of diabetes mellitus--studies on the effectiveness and value of diabetes screening actions].

Actions to found out cases of diabetes, which exclusively worked with a proof of glucosuria using the biophan-test stripe, were always loaded by a high percentage of falsely pathologic, but also by a not insignificant number of falsely negative investigation results. -- Therefore a multiphasic screening was carried out in 5,424 persons at the age of 20 and more years, which tested the problems mentioned and judged the possibilities of an improved diagnosis.
